    Description / Table of Contents: Zusammenfassung Bei Patienten mit benigner essentieller Hypertonie konnte eine Regulationsstörung der Aldosteronexkretion nachgewiesen werden: 55% der Patienten zeigten im Vergleich mit Kontrollpersonen eine eingeschränkte Stimulationsfähigkeit der Aldosteronexkretion nach Natriumverarmung und 45% eine unvollständige Supprimierbarkeit der Aldosteronexkretion nach Natriumbelastung. Die Regulationsstörung der Aldosteronexkretion in Abhängigkeit von der Natriumzufuhr ist bei Hypertonikern 2. Grades deutlicher ausgeprägt als bei Hypertonikern 1. Grades und ist am stärksten bei Patienten mit supprimierter Plasmareninaktivität zu beobachten. Große Änderungen der Natriumzufuhr bedingen bei diesen Patienten in der Regel nur geringe Schwankungen der Aldosteronexkretion. Die mangelhafte Supprimierbarkeit der Aldosteronexkretion scheint Auswirkungen im Sinne eines Hyperaldosteronismus zu haben: Patienten mit unvollständiger Suppression der Aldosteronexkretion wiesen nach Natriumbelastung eine signifikant niedrigere Serumkaliumkonzentration auf. Außerdem gehören zu dieser Gruppe mit einer Ausnahme alle Patienten mit supprimierter Plasmareninaktivität.
    Notes: Summary In patients with benign essential hypertension we could demonstrate a defect in aldosterone regulation: In 55% of the hypertensive patients studied the stimulation of aldosterone excretion by sodium deficiency was less marked and in 45% of the patients, the suppressibility of aldosterone excretion by sodium loading was incomplete compared with controls. The regulatory defect of the hypertensive patients is less pronounced in patients with first grade hypertension than in those with second grade hypertension and can be best demonstrated in patients with suppressed plasma-renin activity. In the latter, considerable changes of sodium intake did induce only minor changes of aldosterone excretion. Patients with incomplete suppression of aldosterone excretion had a significantly lower serum potassium concentration during sodium loading compared with the rest of the hypertensive subjects. All patients with suppressed plasma renin activity, except one, also belonged to this group of essential hypertensives.

    Description / Table of Contents: Zusammenfassung Um Hinweise auf den drucksenkenden Mechanismus einer Langzeit-β-Blockade zu erhalten, wurden Plasma- und Urinproben von 22 essentiellen Hypertonikern wöchentlich vor, während und nach β-Blockade (Pindolol 10–20 mg täglich) im Rahmen einer 11 Wochen dauernden Studie untersucht. Es konnte gezeigt werden, daß sowohl die Serum-Elektrolyte, die Plasma-Noradrenalin-Konzentration und die Plasma-Renin-Aktivität als auch die Exkretion von Natrium, Kalium und Noradrenalin diskrete Veränderungen während mehrwöchiger β-Blockade durchlaufen. Der Exkretionsquotient von Natrium zu Noradrenalin kehrte im Laufe der Behandlung seine Beziehung zum mittleren arteriellen Druck um (Placebor=−0.34, Pindololr=+0,31). Zwischen Blutdruck und Natrium-Exkretion bestand eine signifikante Korrelation (p〈0,03) während Placebogabe, nicht jedoch während β-Blockade. Zwischen Blutdruck und Noradrenalin-Exkretion bestand eine signifikante Korrelation (p〈0,003) während β-Blockade, nicht jedoch während Placebogabe. Im Laufe der Langzeit-β-Blockade kam es zu einer signifikanten positiven Korrelation zwischen der Natrium-Exkretion und der Noradrenalin-Exkretion, welche das Ende der Behandlung überdauerte. Aus den Ergebnissen wird der Schluß gezogen, daß der Gesamt-Bilanz von Natrium und Noradrenalin mehr Aufmerksamkeit zugewendet werden sollte, um die bei der essentiellen Hypertonie zugrundeliegenden Regulationsstörungen zu erfassen.
    Notes: Summary In 22 patients with essential hypertension plasma levels and urine excretions of sodium and noradrenaline were studied before, during and after long-term β-blockade with pindolol. The relation between mean blood pressure and the quotient of sodium-/noradrenaline-excretion changed during treatment (placebor=−0.34; pindololr=+0.31). During placebo there existed a significant (p〈0.03) correlation between blood pressure and sodium-excretion which disappeared during β-blockade. No correlation between blood pressure and noradrenaline was seen during placebo, whereas during β-blockade a significant (p〈0.003) correlation was observed. In contrast to the placebo period there was a significant positive correlation between sodium- and noradrenaline-excretion during long-term treatment with pindolol. It is concluded that whole-body balance of sodium and noradrenaline is an important factor in essential hypertension.

    Description / Table of Contents: Summary Active renin was detected in 6 of 7 anephric patients (mean value: 0.72 ± 0.27 µU/ml,n=7; normals: 19.7 ± 2.4 µU/ml,n=10), using an assay method without intrinsic acid- or cryoactivation. Prorenin, measured as the difference between plasma renin concentration (PRC) before and after acid-activation in vitro, was present in considerable amounts in all patients (32.4 ± 3.5 µU/ml; normals: 80.7 ± 9.7 µU/ml). PRC after cryoactivation at −5°C during 1 week was insignificantly lower than after acid-activation in the patients. There was a linear correlation between PRC after either activation procedure (p 〈 0.01). Supine or upright posture did not influence active or inactive renin in the anephric subjects. Our studies show that low but significant levels of active renin and a considerable amount of prorenin are detectable in anephric patients. The data suggest that prorenin in normal subjects in a significant proportion is of extrarenal origin. Orthostasis has no effect on extrarenal active or inactive renin.
    Notes: Zusammenfassung Bei 6 von 7 anephrischen Patienten waren niedrige Reninspiegel im Plasma nachweisbar: 0,72 ± 0,27 µU/ml,n=7; (Normalpersonen: 19,7 ± 2,4 µU/ml,n=10). Prorenin, eine inaktive Reninvorstufe, die sich aus der Differenz der Plasma-Renin-Konzentration (PRC) vor und nach Säureaktivierung in vitro ergibt, war im Plasma aller anephrischen Patienten vorhanden: 32,4 ± 3,5 µU/ml (Normalplasmen: 80,7 ± 9,7 µU/ml). Nach Cryoaktivierung bei −5°C während 1 Woche war die PRC nur geringfügig weniger angestiegen als nach Säureaktivierung; zwischen beiden Aktivierungsverfahren bestand eine signifikante Korrelation (p 〈 0.01). Liegende Körperhaltung oder Orthostasebedingungen beeinflußten bei den anephrischen Patienten weder aktives noch inaktives Renin. Diese Untersuchungen zeigen, daß im Plasma anephrischer Personen niedrige, aber signifikante Reninspiegel und beträchtliche Prorenin-Konzentrationen vorhanden sind. Die Befunde lassen annehmen, daß Prorenin im Plasma von Gesunden zu einem wesentlichen Teil extrarenalen Ursprungs ist. Orthostase stimuliert offenbar nur renales Renin und hat keinen Effekt auf extrarenales Renin oder Prorenin.

    Description / Table of Contents: Summary In healthy volunteers, the effects of prenalterol, a new β1-adrenoceptor agonist, on renal hemodynamics, excretory function, plasma-renin-activity, plasma cAMP concentration, and plasma and urinary norepinephrine were studied. Besides an increase in blood pressure, which was adjusted to about 20 mmHg above the resting values, and an increase of heart rate, prenalterol induced only transient decreases of urinary volume and free water clearance and in increase of sodium excretion. The other parameters measured did not change. Thus prenalterol mainly exerts positiv inotropic and chronotropic effects and does not affect renal circulation.
    Notes: Zusammenfassung Bei gesunden Freiwilligen wurde der Einfluß des neuen β1-Rezeptoren Agonisten Prenalterol auf die renale Hämodynamik und Ausscheidungsfunktion, auf die Reninfreisetzung, zyklisches AMP im Plasma sowie die Noradrenalin-Konzentration und -Ausscheidung untersucht. Außer einem Anstieg des Blutdrukkes, der auf Grund der gewählten Prenalterol-Dosierung etwa 20 mmHg betrug, und einer Zunahme der Herzfrequenz, bewirkte Prenalterol einen nur vorübergehenden Abfall des Harnminutenvolumens und der Freiwasser-Clearance sowie einen Anstieg der Natrium-Ausscheidung. Die übrigen gemessenen Größen veränderten sich nicht. Somit besitzt Prenalterol überwiegend positiv inotrope und chronotrope Effekte und beeinflußt die Nierenfunktion nicht.

    Notes: Summary Oral tryptophan loading and serotonin (5-HT) uptake by platelets were investigated as an approach to the study of central serotonergic mechanisms in patients with essential hypertension. Single oral doses of L-tryptophan (50 mg/kg body weight) lowered blood pressure significantly 90–120 min after administration in 14 patients with essential hypertension, but not in normotensive controls. Baseline measurements (without tryptophan loading) of 5-HT uptake by platelets did not differ between hypertensive and normotensive persons. Whereas L-tryptophan changed the uptake kinetics and increased 5-HT uptake in normal controls, these effects were not observed — or occurred to a much lesser degree — in hypertensive patients. It is suggested that in human essential hypertension central serotonergic mechanisms are involved in pathogenetic mechanisms. The tryptophan-in-duced lowering of blood pressure could be attributable to the enhancement of central 5-HT synthesis.

    Description / Table of Contents: Summary Secretion rate (SR), metabolic clearance rate (MCR), mean plasma concentration (MPC=SR/MCR) and renal excretion (E) of aldosterone were determined by double isotope derivative assay (Kliman andPeterson) in patients with congestive heart failure of various etiology. Mean plasma concentration of aldosterone (MPC) was found to be increased in 6 out of 15 cases studied. This increase was due to (1) either an increase of aldosterone secretion (SR), (2) a decrease of the metabolic clearance rate (MCR) or (3) both (1) and (2). No hyperaldosteronism was discovered in approximately 2/3 of our patients. Some of those cases revealed reduction of MCR; normal plasma concentration (MPC) resulted from relatively low aldosterone secretion (SR). From these results it can be derived that hyperaldosteronism is apparently not essential in sustaining cardiac edema. It cannot be excluded however, that in an earlier phase of congestive heart failure aldosterone is a significant factor in the pathogenesis of sodium retention. Hyperaldosteronism is typically shown to be a consequence of saluretic therapy: In 9 out of 11 cases studied an increase of SR could be observed in the course of edema excretion, whereas MCR proved to be inconsistent. This induced hyperaldosteronism seemed to be pronounced in patients treated with spironolactone. The increase of SR in patients treated with thiacides was only slight. While increased sodium excretion is considered to be the cause of the stimulation of aldosterone secretion, further increase of SR is likely to occur as a consequence of potassium retention as produced by the action of spironolactone. The stimulatory action of negative sodium balance on aldosterone secretion can possibly be suppressed by increased potassium excretion. Various findings suggest that induced hyperaldosteronism in congestive heart failure is not mediated via the renin-angiotensin system.
    Notes: Zusammenfassung Der Aldosteronstoffwechsel wurde bei Kranken mit kardialer Dekompensation verschiedener Ätiologie vor und zum Teil während Ödemelimination untersucht. Die Aldosteronanalysen wurden mit der Doppelisotopenmethode vonKliman undPeterson durchgeführt. Bestimmt wurden 1. Sekretionsrate (SR), 2. metabolische Clearancerate (=Plasmaclearancerate) (MCR) und 3. renale Exkretion (E). 4. Die mittlere Aldosteronplasmakonzentration ergab sich aus dem Quotienten SR/MCR. Gleichzeitige Analysen aller genannten Parameter konnten bei 15 Fällen von kardialer Dekompensation durchgeführt werden. Eine erhöhte Aldosteronplasmakonzentration fand sich etwa in einem Drittel der Kranken; sie ließ sich entweder 1. auf eine Zunahme der Aldosteronsekretion oder 2. eine Abnahme der metabolischen Clearancerate oder 3. schließlich auf die Kombination beider Vorgänge zurückführen. Bei zwei Drittel der Fälle war kein Hyperaldosteronismus nachweisbar. Bei einigen dieser Fälle lag eine mäßige Verminderung der MCR vor. Eine normale Plasmakonzentration ergab sich durch eine relativ niedrige Aldosteronsekretionsrate. Bei 9 von 11 untersuchten Kranken ließ sich im Ablauf der Rekompensation eine Zunahme der Aldosteronsekretionsrate nachweisen, während die metabolische Clearancerate keine gerichteten Änderungen erkennen ließ. Der gefundene induzierte Hyperaldosteronismus war bei den allein oder überwiegend saluretisch mit Spirolakton behandelten Kranken am stärksten ausgeprägt. Die durch Herzglykoside und/oder Thiazide erreichte Ödemelimination ging nur mit einem relativ geringen Anstieg der Aldosteronsekretion einher. Als primäre Ursache des beobachteten induzierten Aldosteronismus wird die durch die saluretische Behandlung hervorgerufene Natriumexkretion angesehen. Die Aldosteronsekretion kann offensichtlich durch die unter Spirolakton entstehende Kaliumretention gesteigert und bei Behandlung mit kaliuretisch wirkenden Pharmaka gehemmt werden. Verschiedene Befunde lassen vermuten, daß der therapeutisch induzierte Aldosteronismus nicht über das Renin-Angiotensinsystem vermittelt wird.

    Description / Table of Contents: Zusammenfassung In Untersuchungen an gesunden freiwilligen Probanden wurde versucht, die Plasma-Renin-Aktivität durch Gabe von Pharmaka zu stimulieren. Zur Anwendung kamen: das alpha-Sympathicomimeticum Phenylephrin (Visadron®), das beta-Sympathicomimeticum Orciprenalin (Alupent®) sowie das alpha-Sympathicolyticum Phentolamin (Regitin®) und das beta-Sympathicolyticum Propranolol (Dociton®). Außer der Plasma-Renin-Aktivität (PRA) wurden die Hämodynamik und Exkretionsfunktion der Nieren (CIn, CPAH, CNa+ und CK+), der arterielle Mitteldruck und die Herzfrequenz bestimmt. Folgende Ergebnisse konnten erzielt werden: Sowohl während Infusion des alpha-Sympathicomimeticums Phenylephrin als auch des beta-Sympathicomimeticums Orciprenalin ließ sich eine Blutdruckerhöhung und eine Einschränkung der renalen Hämodynamik und Exkretionsfunktion nachweisen. Im Gegensatz zu den Kreislaufgrößen waren letztere nach zusätzlicher Applikation des alpha- bzw. beta-Sympathicolyticums nicht vollständig reversibel. Zum Unterschied von diesen Änderungen war ein signifikanter Anstieg der PRA nur nach Gabe des beta-Sympathicomimeticums Orciprenalin zu beobachten, der durch Injektion von Propranolol umkehrbar war. Durch Propranolol-Gabe war gleichfalls ein durch aktive Orthostase erzielter Anstieg der PRA aufzuheben. Mit Phenylephrin ließ sich nur vereinzelt ein mäßiger Anstieg der PRA erzielen, der darüber hinaus nicht durch den spezifischen Antagonisten Phentolamin gehemmt werden konnte. Die Zufuhr von Phentolamin allein oder in Kombination mit Phenylephrin oder Orciprenalin führte ebenfalls zu einer Stimulation der Reninfreisetzung. Die vorgelegten Ergebnisse ermöglichen folgende Erklärung für die Regulation der Reninfreisetzung: Die Gabe beta-adrenerger Sympathicomimetica führt zu einer Aktivierung des Fermentes Adenylcyclase und zu einem Anstieg von cyclischem AMP, durch dessen Vermittlung die Reninfreisetzung aus dem juxtaglomerulären Apparat der Nieren gesteigert wird. Die Gabe alpha-adrenerger Sympathicomimetica führt hingegen zu einem Konzentrationsabfall des intracellulär lokalisierten cyclischen AMP. Die Ergebnisse deuten somit auf einen einheitlichen Mechanismus der Reninfreisetzung, selbst bei Anwendung von Stimuli mit unterschiedlichen Angriffspunkten.
    Notes: Summary Plasma renin activity was stimulated pharmacologically in normal human volunteers by infusions of sympathomimetic (phenylephrine and orciprenaline) and sympatholytic drugs (phentolamine and propranolol). Besides plasma renin activity, renal hemodynamics, electrolyte excretion, mean arterial blood pressure and heart rate were measured. The following results could be obtained: During infusion of the alpha-adrenergic drug phenylephrine as well as during infusion of the beta-adrenergic drug orciprenaline the mean arterial blood pressure increased, while renal hemodynamics and electrolyte excretion decreased. The latter changes could not be reversed by application of either alpha- or beta-blocking substances. In contrast to these results, a significant increase of plasma renin activity only could be observed after application of the beta-adrenergic drug orciprenaline and it was reversibel by injection of propranolol. Propranolol also inhibited the increase of plasma renin activity after upright posture. Phenylephrine was not able to significantly augment plasma renin activity. The application of phentolamine alone or in combination with phenylephrine or orciprenaline also had a stimulating effect on plasma renin activity. These results may be explained as follows concerning the regulation of renin release: beta-adrenergic drugs lead to an activation of the adenyl cyclase and to augmentation of cyclic AMP, by which renin release from the juxtaglomerular apparatus will be increased, where as alpha-adrenergic drugs lead to a decrease of the cyclic AMP level. It appears that there is a unique mechanism responsible for renin release, even when using various stimuli.

    Description / Table of Contents: Zusammenfassung Es wird über diagnostische Erfahrungen an 10 Patienten mit primärem Hyperaldosteronismus berichtet, bei denen Serumelektrolyte, Plasma-Reninaktivität und Aldosteronsekretion während einer natriumarmen Periode und unter Natriumbelastung untersucht wurden. 7 der 10 Patienten mit klinisch gesichertem Hyperaldosteronismus wurden nach der Diagnosestellung operiert, wobei sich in 3 Fällen ein singuläres Nebennierenrindenadenom fand, in 3 Fällen eine beiderseitige mikronoduläre Hyperplasie und in einem Fall eine Kombination von beiden. Die Natriumbelastung über 7 Tage mit 200–250 mval Natrium pro Tag führte bei 7 von 10 Patienten zur Senkung der Aldosteronsekretion, jedoch in keinem Fall zu einer Normalisierung derselben. Die Plasma-Reninaktivität der Patienten war während der Natriumentzugsperiode in gleichem Maße supprimiert wie während der Natriumbelastungsphase. Bei 8 von 10 Patienten wurde unter NaClreicher Ernährung ein Abfall des Serumkaliums beobachtet im Unterschied zu 76 Patienten mit essentieller Hypertonie, bei denen Natriumbelastung nicht zu Veränderungen des Serumkaliumspiegels führte.
    Notes: Summary In 10 patients with primary hyperaldosteronism, plasma electrolytes, plasma renin activity and aldosterone secretion were assessed during low and high sodium diet. 7 of 10 patients with the clinical syndrome of primary hyperaldosteronism underwent surgical exploration and treatment: in three cases, a singular adenoma was found in the adrenal cortex, three patients showed micronodular hyperplasia of the zona glomerulosa on both sides, and a combination of both was seen in one case. In 7 of 10 patients high sodium diet with 200–250 mval Na+/day for 7 days resulted in markedly lowered aldosterone secretion, but never was normal aldosterone secretion reached. The plasma renin activity of the group of patients with primary hyperaldosteronism was suppressed to a similar extent during low and during high sodium diet. High sodium diet induced a decrease in plasma potassium concentration in 8 of 10 patients which is in contrast with the findings in 76 patients with essential hypertension; in these patients the plasma potassium levels were not modified by changing the daily sodium intake.

    Description / Table of Contents: Zusammenfassung An 8 Gesunden und 21 Kranken mit kardialer Insuffizienz wurden Renin-Aktivität (PRA) und Angiotensin II-Konzentration (A II) im Plasma, Serumnatrium und -kalium und renale Na+-, K+- und Flüssigkeitsexkretion vor sowie 2, 4 und 6 Std nach i.v. Gabe von 0,3 mg Furosemid/kg untersucht. Die PRA wurde biologisch, A II radioimmunologisch bestimmt. Die Gesunden reagierten mit signifikanten Anstiegen der PRA- und A II-Werte (P〈0,005), die Kranken mit kardialer Insuffizienz zeigten 3 verschiedene Verhaltensmuster der PRA: Absinken unter den Ruhewert (4 Fälle), Anstieg (4 Fälle) und Gleichbleiben (14 Fälle, davon 11 ohne meßbare PRA). Eine mangelhafte Stimulierbarkeit der PRA ist somit auch bei kardialer Hydropsie möglich. Die Pat. mit abfallender bzw. gleichbleibender PRA waren vornehmlich Schwerkranke mit ausgeprägter Hydropsie; sie wiesen nach Furosemid im Mittel gleich große, im einzelnen z.T. weit höhere Na+-Verluste auf als die Gesunden. Dies könnte durch die Annahme eines (zentralnervösen) Systems erklärt werden, das die renale Na+-Verlustrate in Beziehung zum Na+-Gesamtbestand setzt und erst ab einer kritischen Relation gegenregulatorisch die Steigerung der Reninfreisetzung veranlaßt. Diese Vermutung wurde durch Untersuchung einer Patientin vor und nach Ödemausschwemmung wahrscheinlich gemacht. PRA- und A II-Werte verhielten sich konkordant (r=+0,627,P〈0,001).
    Notes: Summary In 21 patients with congestive heart failure and 8 normal controls plasma renin activity (PRA), angiotensin II (A II), sodium and potassium concentrations in plasma and renal Na+, K+, and water excretion were measured before and 2, 4, and 6 hours after an intravenous injection of Furosemide (0.3 mg/kg). PRA was determined biologically, A II by radioimmunoassay. The control group showed a significant increase of PRA and A II levels (P〈0.005). In patients with congestive heart failure, three different patterns of PRA were observed: decrease (4 cases), increase (4 cases) and no change (14 cases which comprise 11 patients without detectable PRA), compared to the individual control value. Thus, inadequate stimulation of PRA may occur in congestive heart failure, too. Particularly the patients with decreasing or unchanged PRA suffered from severe cardiac insufficiency with severe edema. After application of Furosemide, these patients showed in average the same, some of them a much higher Na+ loss than the controls. These results could be explained by the assumption of the presence of a control mechanism, possibly located in the brain, that detects the renal Na+ excretion in relation to the total body Na+ and induces the counterregulatory renin secretion not unless a certain reference value is attained. This hypothesis was supported by the results obtained in the same patient before and after loss of edema. PRA and A II values were concordant (r=+0.627,P〈0.001).
