Wir berichten über einen 30jährigen Diabetiker mit dem von Shulman 1974 definierten Syndrom. Eine Kortikosteroidtherapie führte zur Remission der Krankheitssymptome, jedoch zeigte eine Rebiopsie, daß die Therapie keinen Einfluß auf den pathomorphologischen Befund hatte. Anhand einer Literaturübersicht unter Berücksichtigung von 118 Fällen werden die Gemeinsamkeiten und Unterschiede von EF und der systemischen Sklerodermie diskutiert.
    Notes: Summary Scleroderma like skin indurations without internal organ involvement, joint contractures, eosinophilia, and inflammatory infiltration of the fascia between subcutis and muscle are considered as characteristic features of eosinophilic fasciitis (EF). We report a further case of a 30 years old diabetic confirming to the syndrome defined by Shulman 1974. Corticosteroid therapy resulted in remission of disease symptoms, however a rebiopsy revealed no effect of therapy on histopathologic changes. A critical review of 118 cases in the literature is presented to explore the relationship of EF to scleroderma.

    Notes: Summary The westernblot analysis of 170 patients with HIV-1-infection demonstrated that 47% of the patients in latent stage, 58% of the patients with lymphadenopathy-syndrome and only 25% of the patients with the full-blown picture of AIDS showed the complete pattern of HIV-specific antibody response. This antibody response is mainly directed against the env-encoded envelope proteins gp160, gp120 and gp41, against the gag-encoded core proteins p55, p24 and p17 as well as against the pol-encoded enzymatic proteins p66, p51 and p31. Antibodies against gp160 and gp120 were present in nearly all patients, whereas the prevalence of the other antibodies decreased with the stage of the disease. Statistical significant differences were found particularly between patients with LAS or AIDS respectively. Antibodies against p17 were detected in 74% of the patients with LAS but only in 25% of the patients with AIDS. The lack of antibodies against p17, p24 or p51 was significantly associated with lower mean CD4/CD8-ratios (p〈0,007) and higher mean serum levels of IgA (p〈0,001) and beta-2-microglobulin (p〈0.001). One third of the patients with LAS and this reduced pattern of antibody response developed AIDS within six months. These results demonstrate that the detection of antibodies against p17, p24 or p51 is of prognostic importance. A serological profile which lacks the antibody response against at least two of those three viral antigens indicates a progression of the disease activity.

    Notes: Summary A 31-year-old woman with chronic myeloid leukemia developed bullous skin changes not responding to antibiotic therapy. A biopsy showed a sterile predominantly neutrophilic infiltrate consistent with Sweet's syndrome. These skin changes responded well to methylprednisolon. Seven month later the patient died in a blast crisis.

    Notes: Summary We investigated the effect of UV light (320–460 nm) on total hemolytic CH50 activity and C3 cleavage in sera obtained from 14 patients with porphyria cutanea tarda. Irradiation with 5, 10, or 50 J/cm2 resulted in a 12%–60% loss of CH50 and a 5%–30% cleavage of native C3 as estimated by planimetric evaluation of the immunoelectrophoretic C3 pattern. The complement changes were most pronounced in sera from patients with active disease and were minimal or absent in patients who were in remission. In all cases, the decrease of CH50 and C3 cleavage was proportional to the plasma-porphyrin concentration and the dose of radiation. After exposure to 320- to 460-nm light, similar changes were seen in normal human serum (NHS) to which exogenous uroporphyrin had been added. Beta-carotene and chloroquine had no inhibitory effect on the photodynamic complement activation. The C3 cleavage in irradiated NHS containing uroporphyrin was not affected by 10 mM EGTA, but was partially inhibited in the presence of 30 mM EDTA, thus indicating that the interaction of photoexcited uroporphyrin with the complement system differs from classical-pathway complement activation.

    Notes:   Topical Calcineurin Inhibitors (TCIs) used for the treatment of atopic eczema modify the immune regulatory function of the skin and may have the potential to enhance immunosuppressive ultraviolet (UV) effects. Current recommendations on UV protection in eczema patients treated with PCIs are inconsistent and have given rise to uncertainty and anxiety in patients. Therefore, the European Dermatology Forum (EDF) developed a position statement which reviews critically the available data with regard to the problem, especially analysing and commenting the limitations of rodent models for the human situation. There is no conclusive evidence from rodent trials to indicate that long-term application of TCIs is photococarcinogenic. There is a need for further studies to investigate the validity of mouse models as well as long-term cohort studies in patients using TCIs. Available data suggest that long-term application of TCIs is safe, that there is no evidence of increased skin cancer risk and that it is ethical to treat patients with TCIs when indicated.

    Notes: In the context of our investigation, we found information on 432 (76%) of the 569 Jewish dermatologists in Germany. There is evidence that 57 (10%) of the Jewish dermatologists were murdered in concentration camps, 61 (10.7%) died a natural death, 13 (2%) committed suicide, and 25 (4%) survived the Third Reich in Germany. After 1933, 276 (49%) Jewish dermatologists were able to leave Germany; the United States of America was the main destination and 107 (or 41%) emigrated there. A total of 34 (13%) Jewish dermatologists emigrated from Germany to Palestine and 16 to Latin America. Regarding emigration to other European countries, 20 of the Jewish dermatologists from Germany went to Great Britain (including Walter Freudenthal, 1893–1952, and Ernst Sklarz, 1894–1975), and 24 emigrated to other European countries, such as France (Rudolf Mayer, 1895–1962), Sweden (Carl Lennhoff, 1883–1963), and the Netherlands (Otto Schlein, 1895–1944).

    Notes: UVB irradiation of cultured human keratinocytes induces both the conversion of 7-dehydrocholesterol (7-DHC) to calcitriol (1α,25(OH)2D3) and the release of tumor necrosis factor-a (TNF-α) in these cells. Calcitriol synthesis in human keratinocytes was reduced in the presence of a neutralizing polyclonal antibody directed against human TNF-α. On the other hand, we found a 1.7-fold higher stimulatory effect of UVB on liberation of TNF-α in cultured keratinocytes enriched with 7-DHC compared with irradiated cell cultures in absence of 7-DHC. These observations argue in favor of a synergetic relationship between generation of TNF-α and calcitriol in UVB irradiated keratinocytes. In addition, we found that TNF-α potently increases the conversion rate of vitamin D3(cholecalciferol) to calcitriol this cell system. The UVB-triggered formation of both TNF-α and calcitriol in cultured keratinocytes as wavelength-, time- and dose-dependent. Maximum formation of TNF-α and calcitriol was found at 300 nm and UVB doses of 30 mJ/cm2. The enhancement of both, the formation of TNF-α and calcitriol in keratinocytes by UVB may be of relevance for regulation of growth and apoptosis in light-exposed epidermal cells and, in addition, may play a role in the UVB treatment of deseased skin, including psoriasis.
