    Description / Table of Contents: Summary One case of LED and myastenia gravis (Patient aged 33 years), two cases of LED and Thyroiditis (patients aged 42 and 35 years), and immunopathological phenomena of 13 patients with LED were reported. — 3 of 4 patients with LED and positive rheumatoid factors had in the preclinical phase of LED a history of rheumatoid arthritis. — There are leukocytagglutinins in 3 and plateletagglutinins in 2 patients. Both agglutinins were present in these patients. The cytopenia therefore seemed to be caused by autoantibodies. — Factors against species-unspecific cytoplasmatic fractions of rat and pig liver were found by complement fixation-test only in one case using the microsome and mitochondria fractions as antigen. — Thyroid-antibodies were directed against human thyreoglobulin in one case, and against the microsome fractions of normal and pathological human thyroid as well as thyreoglobulin of colloid goitre in another one. — The rare observation of LED and myastenia gravis gave rise to the discussion of the relation between autoimmun diseases and thymus function.
    Notes: Zusammenfassung Über immunpathologische Begleiterkrankungen und -phänomene beim LED wird anhand eines Falles von LED und Myasthenia gravis (33jährige Patientin), 2 Fällen von LED und Thyreoiditis (42- bzw. 35jährige Patientinnen) und 7 bzw. 13 weiteren LED-Patienten berichtet. — Von 4 LED-Patienten mit positiven Rheumafaktoren waren 3 aus einer chronischen Polyarthritis rheumatica hervorgegangen. — Die nachgewiesenen Leuko- bzw. Thrombocytenagglutinine bei 3 bzw. 2 Patienten traten in 2 Fällen im Sinne autoantikörperbedingter Cytopenien kombiniert auf. — Faktoren gegen speciesunspezifische cytoplasmatische Fraktionen von Ratten-und Schweineleber waren mit der KBR nur in einem Falle unter Verwendung der Mikrosomen bzw. Mitochondrienfraktion als Antigen nachweisbar. — Die Schilddrüsenantikörper waren in dem einen Fall gegen menschliches Thyreoglobulin, im anderen Fall gegen die Mikrosomenfraktionen gesunder und kranker menschlicher Schilddrüsen sowie Thyreoglobulin einer Kolloidstruma gerichtet. — Die seltene Beobachtung eines LED, kombiniert mit Myasthenia gravis war Anlaß, die Beziehungen von Autoimmunleiden zur Thymusfunktion zu diskutieren.

    Description / Table of Contents: Summary Liver biopsies of patients with inflammatory liver diseases and clinically healthy HB s Ag-carriers were examined for presence of intracellular HB s Ag, HB c Ag and IgG by direct immunofluorescence. The studies revealed the following results: 1. In most cases healthy HB s Ag-carriers had HB s Ag in the cytoplasm, but they did never show HB c Ag in the nuclei of hepatocytes. 2. In the early phase some patients with HB s Ag-positive acute hepatitis had HB c Ag and/or HB s Ag in their hepatocytes. In a normal course with complete recovery the immunoelimination may clear either phenomenon at variable stages of the disease. 3. Cases one year after complete recovery of acute virus B-hepatitis had no HB-components in their liver tissue. 2 cases without immunoelimination of HB s Ag developed chronic active hepatitis within one year and had HB c Ag in their liver cell nuclei. 4. Patients with HB s Ag-positive CAH and highly inflammatory activity had HB c Ag in the nuclei and a low percentage of cells with HB s Ag in the cytoplasm of hepatocytes. HB s Ag-negative cases with CAH never had HB-components in their tissue. 5. Patients with HB s Ag-positive and -negative CAH in complete remission never had HB c Ag and HB s Ag in their hepatocytes. 6. Most cases with HB s Ag-positive acute hepatitis and chronic active hepatitis positive for HB c Ag had also IgG in the same liver cell nuclei. The coincidence of this finding gives strong evidence for the presence of anti-HB c in these liver cell nuclei. The importance of this finding for the course of the disease is unknown.
    Notes: Zusammenfassung Wir untersuchten mit der direkten Immunfluorescenztechnik die Leberbiopsien von Patienten mit entzündlichen Lebererkrankungen auf intracelluläres HB s Ag, HB c Ag und IgG. Die Untersuchungen haben folgende Ergebnisse erbracht: 1. Klinisch gesunde HB s Ag-Träger hatten in den meisten Fällen HB s Ag im Cytoplasma, jedoch niemals HB c Ag in den Leberzellkernen. 2. Patienten mit HB s Ag-positiver akuter Hepatitis hatten in der Frühphase HB c Ag und/oder HB s Ag in den Hepatocyten. Unsere Befunde sprechen bei normalem Verlauf für eine komplette Immunelimination der HB-Komponenten zu verschiedenen Stadien der Erkrankung mit völliger Wiederherstellung. 3. Kontrollpunktate 1 Jahr nach Beginn einer HB s Ag-positiven Hepatitis zeigten bei völlig gesunden Patienten kein HB s Ag oder HB c Ag. Bei zwei Patienten ohne Immunelimination von HB s Ag entwickelte sich innerhalb eines Jahres eine chronisch aktive Hepatitis. Diese Patienten hatten HB c Ag in den Leberzellkernen. 4. Patienten mit HB s Ag-positiver CAH und Zeichen entzündlicher Aktivität hatten HB c Ag in den Zellkernen und in geringem Prozentsatz HB s Ag im Cytoplasma. HB s Ag-negative Fälle zeigten niemals HB-Komponenten im Gewebe. 5. In keinem Fall einer HB s Ag-positiven und -negativen CAH in Remission ließ sich HB s Ag und HB c Ag in Hepatocyten nachweisen. 6. In den meisten Fällen von HB s Ag-positiver AH und CAH, die HB c Ag-positiv waren, fand sich in den gleichen Zellkernen IgG. Die strenge Koinzidenz von HB c Ag und IgG läßt an das Vorliegen von anti-HB c in den Zellkernen denken. Die Bedeutung dieses Antikörpers in den Zellkernen für den weiteren Verlauf der Erkrankung ist unbekannt.

    Description / Table of Contents: Zusammenfassung Eine Analyse der Literatur und eigene Befunde über die Häufigkeit von 23 HLA-Antigenen bei Patienten mit Hepatitis-Oberflächen-Antigen (HBsAg)-positiver und -negativer chronisch aktiver Hepatitis (CAH) und gesunden Trägern von HBsAg und Antikörpern gegen HBsAg (Anti-HBs) erlauben die folgenden Schlußfolgerungen: 1. Bei Patienten mit CAH und HBsAg-Persistenz findet sich eine normale Häufigkeit von HLA-B8 (und -A1). 2. Auch bei Patienten mit HBsAg-negativen Formen einer CAH ohne Autoimmunphänomene (AutoAk) ist die HLA-B8-Häufigkeit normal. 3. Nur Patienten mit HBsAg-negativer CAH mit AutoAk haben eine statistisch signifikant größere Häufigkeit von HLA-B8 (und -A1) (p 〈 0,01 nach Korrektion für die Zahl der getesteten HLA-Antigene und Patientengruppen). 4. Es bestehen keine signifikanten Unterschiede in der Häufigkeit der getesteten HLA-Antigene bei CAH-Patienten mit HBsAg-Persistenz und gesunden Trägern von HBsAg und Anti-HBs. Somit konnten keine Hinweise für HLA-assoziierte Faktoren für das unterschiedliche Verhalten gegen das Hepatitis-B-Virus gefunden werden.
    Notes: Summary New own data and a survey of published data concerning the frequencies of 23 HLA antigens in patients with HBsAg positive and negative chronic active hepatitis (CAH) and healthy carriers of Hepatitis-surface antigen (HBsAg) and high titers of antibodies to HBsAg (Anti-HBs) allow to conclude as follows: 1. Patients with CAH and persistence of HBsAg show a normal frequency of HLA-B8 2. There is no increased frequency of HLA-B8 in HBsAg negative CAH without autoimmune antibodies. 3. Only in patients with HBsAg negative CAH with autoimmune antibodies is the frequency of HLA-B8 is statistically significantly increased (p 〈 0.01 after correction for the number of antigens and different groups of patients compared). These are patients with the autoimmune form of CAH. 4. There exist no significant differences in the frequencies of the HLA antigens tested in patients with HBsAg positive CAH and healthy carriers of HBsAg and Anti-HBs. Thus no indications could be found for HLA-associated factors in the different behaviour to the hepatitis-B virus.

    Description / Table of Contents: Zusammenfassung Wir haben 129 Blutspender, bei denen bei Routineuntersuchungen HBsAg im Serum festgestellt wurde, zur diagnostischen Klärung einer möglichen Lebererkrankung untersucht. Zwölf hatten, als sie zur klinischen und leberbioptischen Untersuchung aufgenommen wurden, schon kein HBsAg mehr im Serum. Keiner von diesen hatte klinisch oder histologisch Anhalt für eine entzündliche Lebererkrankung. Zwei der 129 Patienten hatten eine milde, ikterische Hepatitis, verloren das Antigen während des Verlaufes und wurden anti-HBs-positiv. Die übrigen 115 Patienten, die klinisch gesund erschienen und keine frühere ikterische Leberkrankheit hatten, blieben HBsAg-positiv während des bisherigen Verlaufes von im Mittel 17,3±3,0 Beobachtungsmonaten. 40 dieser Patienten hatten histologisch eine normale Leber und 37 geringe bis deutliche Verfettungen des Parenchyms ohne Entzündungszeichen. 11 Patienten hatten eine leichte unspezifische Mesenchymaktivierung ohne fokale Nekrosen, 16 Patienten hatten neben Einzelzellnekrosen mit mesenchymaler Reaktion milde portale Infiltrationen, 6 hatten eine chronisch persistierende Hepatitis, 4 eine chronisch aggressive Hepatitis und 1 Patient eine posthepatitische Cirrhose. Eine Anzahl biochemischer und immunologischer Werte dieser Patientengruppen wurden mit denen einer Gruppe von 88 HBsAg-negativen Blutspendern verglichen. Signifikant höhere SGOT-Werte wurden nur in der Gruppe der Patienten mit Verfettungen, der Gruppe mit Nekrosen und Reaktionen und der Gruppe mit chronisch aggressiver Hepatitis gefunden. Die SGPT-Werte waren nur in den beiden letzteren Gruppen signifikant höher. Der Mittelwert der gamma-GT war nur in der Patientengruppe mit Verfettungen signifikant höher als in der HBsAg-Trägergruppe mit normaler Histologie, nicht aber verglichen mit dem HBsAg-negativen Kontrollkollektiv. Signifikant erhöht waren auch die Bilirubinwerte in allen Gruppen außer der Gruppe mit chronisch aggressiver Hepatitis, jedoch blieben die Mittelwerte immer im Normbereich. Die relativen Häufigkeiten der HBsAg-Subtypen verteilten sich gleichmäßig auf die verschiedenen Gruppen. Bei 7 Trägern fanden wir auch Anti-HBs im Serum. Autoimmunphänomene wurden nur selten beobachtet. Mit der direkten fluorescierenden Antikörpertechnik wurde in 77% der Leberbiopsien intrazytoplasmatisches HBsAg mit einer gleichmäßigen Häufigkeitsverteilung in den Gruppen festgestellt. Intranucleäres HBcAg konnte aber nur bei Patienten gefunden werden, die entweder eine chronisch entzündliche Lebererkrankung hatten oder entwickelten, während sie im Verlauf zur Kontrolle biopsiert wurden. Die Ergebnisse beweisen schlüssig, daß es vollkommen gesunde HBsAg-Träger mit normaler Leberhistologie gibt, die sich in bezug auf ihre Leberfunktion und Häufigkeit von positiven Autoimmunphänomenen nicht von normalen Kontrollpersonen unterscheiden. Diese Träger haben häufig intrazytoplasmatisches HBsAg aber nie HBcAg oder IgG in Kernen von Hepatocyten.
    Notes: Summary 129 blood donors found to be HBsAg-positive on routine testing were studied for evidence of hepatic disease. Twelve had already lost the antigen from the serum when histologically examined. None of these has had clinical or histological evidence of inflammatory liver disease. Two of the 129 patients showed mild icteric hepatitis, cleared the antigen during the follow up and became anti-HBs positive. The remaining 115 patients who appeared clinically healthy and who had no history of previous icteric liver disease remained HBsAg positive during a mean follow up period of 17.3±3.0 months. Forty patients from these had a normal liver histology and 37 mild to distinct steatosis but no signs of inflammatory liver disease. 11 patients a mild nonspecific mesenchymal activity but no focal necrosis, 16 patients had mild infiltration in portal tracts and a few necrotic parenchymal cells with mesenchymal reaction, 6 patients had chronic persistent hepatitis, 4 chronic aggressive hepatitis, and 1 definite posthepatitic cirrhosis. A number of biochemical and immunological variables of these patients were compared to those of a group of 88 HBsAg-negative blood donors. Significant higher SGOT values were found only in the groups of patients with steatosis, necrosis of parenchymal cells, and chronic aggressive hepatitis. SGPT values were significantly higher only in the two latter groups. The mean value for the gamma GT was significantly higher only in the group with steatosis, compared to the HBsAg carrier group with normal liver histology. A significant difference could not be established when compared to HBsAg-negative controls. Significantly higher, but still within normal range, were bilirubin values in all groups except for the 4 patients with chronic aggressive hepatitis. The relative frequencies of HBsAg subtypes were evenly distributed among the different groups. 7 carriers also had anti-HBs detectable in their serum. Autoimmune phenomena were rarely found. With the direct fluorescent antibody technique intracytoplasmic HBsAg was found in 77% of the liver biopsy specimens with an even distribution of the frequencies among the groups. Intranuclear HBcAg however could only be found in patients who either had or developed a chronic inflammatory liver disease when rebiopsied during the follow up. The results definitely establish that there are completely healthy HBsAg carriers with normal liver histology who do not differ from normal controls in their liver function and frequencies of positive autoimmune phenomena. They frequently have intracycloplasmic HBsAg but never HBcAg or IgG in the nuclei of their hepatocytes.

    Notes: Summary Herpes simplex virus (HSV) has been associated with various neurological disorders. In contrast, HSV infection is very rarely found in acute polyneuroradiculitis. In this report, a patient is described with a severe course of Guillain-Barré syndrome (GBS). HSV IgM-specific antibodies and a rise of complement-fixation antibodies were detected. During the acute phase of neurologic syndrome, a nerve biopsy showed myelin damage and IgM deposits on the inner layer of the perineurium. Plasma exchange, in combination with immunosuppression, was successfully applied as a treatment in the relapsing course of GBS. Finally, after recovery, HSV-specific IgM antibodies disappeared.

    Notes: Summary Three cases with renal angiomyolipoma in Bourneville's disease are reported. In two patients advanced renal insufficiency developed, one being treated by chronic intermittent hemodialysis. In the third patient rupture of angiomyolipoma with severe retroperitoneal bleeding occurred. In one patient, tumorous enlargement initially was diagnosed only in the right kidney and preceded angiomyolipoma of the left kidney for several years. In this patient nephrectomy was performed because malignancy was assumed before diagnosis of Bourneville's disease was established. Occasionally, histology may suggest sarcomatous changes. However, the absence of distant metastasis and the overall long survival of patients with renal angiomyolipoma in Bourneville's disease underline the benign character of the tumor. Treatment should be conservative because surgical intervention with loss of kidney parenchyma may enhance progression to end-stage renal failure.

    Description / Table of Contents: Summary Investigations are reported, in which the presence of species-unspecific and organ-specific factors against at least one of the soluble proteins of liver tissue of 4 patients with chronic hepatitis is shown. These findings were confirmed by animal experiments. Rabbits, immunized with homogenates of rat liver, kidney, and thyroid formed mainly macroglobulinantibodies during the first 10 days following the completion of immunization. After the 20th day mainly heatstable (65° C) 7-s-antibodies were produced. In the antisera against rat liever homogenate 7-s-antibodies against at least one of the soluble proteins of rat liver were found. A detailed presentation of the findings will soon be published in the Deutsches Archiv für klinische Medizin.
    Notes: Zusammenfassung Es wird über den Nachweis speciesunspezifischer bzw. organspezifischer Faktoren gegen mindestens ein Protein der löslichen Proteinfraktionen von Lebergeweben bei vier Patienten mit chronischer Hepatitis berichtet. Die Befunde konnten tierexperimentell bestätigt werden. Mit Rattenleber-, -nieren- bzw. -schilddrüsenhomogenat immunisierte Kaninchen bildeten in den ersten 8–10 Tagen nach der letzten Immunisierung vorwiegend Makroglobulinantikörper, nach dem 20. Tag nach der Immunisierung vorwiegend bei 65° hitzestabile 7-S-Globulin-Antikörper. In den Leberantiseren ließen sich 7-S-Antikörper gegen mindestens ein Protein der löslichen Proteinfraktion von Rattenlebern abgrenzen. Eine ausführliche Darstellung der Einzelbefunde soll in Kürze im Deutschen Archiv für klinische Medizin erfolgen.

    Description / Table of Contents: Summary The effect of Azathioprine (AP, “Imuran”) during a peroral application on the antibody response of the immediately type was investigated by rabbit experiments. The primary immunological response was found to be only particulary suppressed by doses of 4 mg AP/kg/day. Doses of 6 mg AP/kg/day suppress the primary response completely. This dose had no great influence on the secundary response. That was possible only when given toxical doses of 15 mg AP/kg/day. The suppressive effectiveness of AP at a lower dose than 6 mg/kg/day increases with the antigenic stimulation.
    Notes: Zusammenfassung Die Wirkung von AP bei peroraler Applikation auf die Ak-Antwort vom Soforttyp wurde im Kaninchenexperiment geprüft. Die primärimmunologische Antwort ließ sich durch 4 mg AP/kg/Tag nur teilweise supprimieren. 6 mg waren voll wirksam hinsichtlich der Unterdrückung der Primärreaktion. Die Sekundärreaktion wurde von dieser AP-Dosis nicht wesentlich beeinflußt. Dies gelang erst mit toxischen Dosen von 15 mg AP/kg/Tag. Die Wirksamkeit von AP unterhalb von 6 mg/kg/Tag wächst mit dem Antigenstimulus.

Bei der chronisch aggressiven Hepatitis wurden HL-A 1 und 8 statistisch signifikant häufiger gefunden (p〈0,01).

    Description / Table of Contents: Summary In chronic active hepatitis (CAH,n=58) 70% of the HBsAg negative and 48% of the HBsAg positive cases showed a CMI against human liver specific proteins (HLPI). Using HBsAg as antigen only 12% of the HBsAg negative and 24% of the HBsAg positive cases gave a CMI response. On the basis of HBsAg and autoantibodies in the serum CAH patients could be divided into 4 subgroups. A close correlation between CMI against HLPI, sex, ANA and HL-A-8 could be detected. In a follow-up study of patients with acute virus B hepatitis (n=62) CMI against HBsAg was detected in 60% of the cases in the acute phase of the disease but in 15% only 3–6 months after the onset of the illness (n=40). In patients who developed a chronic HBsAg carrier status 3 of 5 cases remained persistently positive with HLPI as antigen in the migration inhibition test. — In non-hepatic diseases in which immunological abnormalities may be present (malignant diseasesn=46, diabetes mellitusn=27, active tuberculosis,n=18 and untreated systemic lupus erythematodes,n=5) only 26% of patients with malignant diseases showed a migration inhibition with HLPI. — Using different antigens such as human liver specific proteins (HLP), rabbit liver specific proteins (RLP), brucella suis antigen and tuberculin it was possible to demonstrate the validity of the two-step migration inhibition test to detect CMI. The results with different antigens in hepatic and non-hepatic diseases demonstrated that cell-mediated immunity to HLPI is an organ specific immune reaction which is associated with acute and chronic active liver diseases as a time limited or long-lasting phenomenon. Positive reactions in some tumor patients suggest that different mechanisms may elicit an autoimmune reaction against liver antigens.
    Notes: Zusammenfassung Bei Patienten mit chronisch-aktiver Hepatitis (CAH) (n=58) konnte bei 70% der HBsAg-negativen und 48% der HBsAg-positiven Fälle eine zelluläre Immunreaktion (CMI) gegenüber humanen leberspezifischen Proteinen (HLPI) aus Plasmamembranen nachgewiesen werden. Mit HBsAg als Antigen war nur bei 12% der HBsAg-negativen und 24% der HBsAg-positiven CAH eine CMI vorhanden. Unter Berücksichtigung von HBsAg und Autoantikörpern im Serum von Patienten mit CAH konnte eine Einteilung in 4 Untergruppen vorgenommen werden. Nur in Gruppe 1 konnte eine enge Korrelation zwischen CMI gegenüber HLPI, weiblichem Geschlecht, ANA und HLA-8 gefunden werden. — In einer prospektiven Studie bei Patienten mit akuter Virus B-Hepatitis (n=62) fand sich eine CMI gegenüber HBsAg in 60% in der akuten Phase der Erkrankung, jedoch nur in 15% der Fälle im Zeitintervall von 3–6 Wochen nach Beginn der Erkrankung (n=40). Bei Patienten, die einen chronischen HBsAg Carrier-Status entwickelten, blieb bei 3 von 5 Fällen konstant eine CMI gegenüber HLPI nachweisbar. — Bei nicht hepatischen Erkrankungen mit immunologischen Abnormitäten (malignen Erkrankungenn=46, Diabetes mellitusn=27, aktiven Tuberkulosenn=18 und unbehandelten Fälle mit Lupus erythematodes disseminatusn=5) konnte lediglich in 26% der Patienten mit malignen Erkrankungen eine Hemmung der Migration gegenüber HLPI gefunden werden. Unter Verwendung verschiedener Antigene, wie HLPI, R LPI, Brucella Suis-Antigen und Tuberkulin, war es möglich, die Validität des Zweiphasen-Migrations-Inhibitionstestes zum Nachweis einer CMI nachzuweisen. Die Ergebnisse bei hepatischen und nichthepatischen Erkrankungen zeigen, daß eine CMI gegenüber HLPI eine organ-spezifische Immunreaktion ist, die bei akuten und chronisch-aktiven Leberentzündungen passager oder persistierend auftreten kann. CMI bei einigen Tumorpatienten legen nahe, daß verschiedene Mechanismen für die Induktion einer Autoimmunreaktion gegenüber Leberantigenen verantwortlich sein können.
