    Notes: Summary A 50-year-old male patient had three episodes of acrally located purpura within 12 years without any other symptoms. The first two episodes which lasted several months coincided with the intake of beta-blockers; the third episode developed after a respiratory infection. A cryofibrinogenemia was found without any evidence of an underlying primary disease, especially not of a neoplasm. The clinical, histological, immunohistological, and electron microscopical findings, as well as the results obtained by intravital fluorescence videomicroscopy are presented.

    Notes: Eccrine syringofibroadenoma (ESFA) is a rare disorder that shows differentiation toward eccrine sweat apparatus. There is a controversy concerning the pathogenesis and differentiation of this tumour. We report a case of ESFA in a 63-year-old Japanese man. We review the literature presenting a classification, including a newly reported subtype. Clinically and pathogenically, ESFA is probably a group of heterogeneous disorders.

    Notes: Hybrid cyst is a rare cystic lesion that includes more than two components of the pilosebaceous units. To clarify the clinical and pathological features of hybrid cysts, we report two cases and review 15 cases of hybrid cyst in Japan.On the whole, the age range was 12–73 years with a 2.95 : 1 female predominance and predilection for the scalp and face (46.7%). Most of the tumours presented as a solitary lesion and the size range was 2–45 mm. The most frequent histological type was the combination of infundibular and trichilemmal cysts (60.0%). Studying the clinicopathological features of hybrid cysts helps us in understanding the pathogenesis of diseases arising from pilosebaceous units.

    Notes: We report the perinatal presentation and evolution of Netherton’s syndrome in siblings. The first patient, a female infant, presented with asphyxia at birth due to aspiration of desquamated scale, non-bullous ichthyosiform erythroderma, and hypernatraemic dehydration which she had for several days. Subsequently, she failed to thrive, with recurrent bacterial infections until 5 months of age, and very high serum IgE levels (1200 U/mL). Trichorrhexis invaginata and pili torti were identified at 18 months. The second patient was the younger brother of the first. Hydramnios and hyperechoic material in the amniotic fluid were observed by ultrasound at 35 weeks gestation, and he was delivered by elective Caesarian section at 40 weeks. At birth, no hair abnormality was demonstrated but, like his sister, his body was covered with thick caseous material, and he was erythrodermic. He failed to thrive, but serum IgE levels were normal until 5 months of age. Typical trichorrhexis invaginata was not observed until 7 months of age. Thus, the hair abnormality and high serum IgE levels in Netherton’s syndrome appear late relative to the ichthyosiform erythroderma.

    Notes: Background  Tumour necrosis factor (TNF)-α-converting enzyme (TACE) is a metalloproteinase-disintegrin that releases soluble TNF-α from cells by cleaving within the extracellular domain of membrane-bound pro-TNF-α. It was proposed that TNF-α is involved in the pathogenesis of psoriasis, and it is therefore suggested that TACE has important roles in psoriasis. However, it is unclear whether TACE is expressed in psoriatic tissue.Objectives  To clarify whether TACE is expressed in psoriatic lesions and whether expression levels of TACE mRNA are increased in lesional compared with nonlesional psoriatic skin.Methods  Skin biopsies were obtained from patients with psoriasis. We examined the expression of TACE in psoriatic tissues using a novel real-time quantitative reverse transcriptase–polymerase chain reaction method and immunohistochemical analysis.Results  There was a significant rise in the level of TACE mRNA expression in lesional psoriatic skin compared with nonlesional skin in all patients. There was a statistically significant rise in the level of TACE mRNA expression in lesional psoriatic skin compared with nonlesional skin (mean ± SD TACE/glyceraldehyde-3-phosphate dehydrogenase ratio 0·031 ± 0·012 vs. 0·009 ± 0·002, P 〈 0·05). In lesional psoriatic skin, immunostaining with anti-TACE antibody was present throughout all layers of the epidermis. TACE immunostaining was found in the cytoplasm of keratinocytes. There was staining associated with blood vessels in the papillary dermis and perivascular inflammatory cells. In particular, mast cells showed strong staining. They contained numerous granules that were stained for TACE in the cytoplasm.Conclusions  The findings of the present study suggest that elevation of TACE mRNA in psoriatic lesions is due to many cells, particularly mast cells, that function in lesional psoriatic skin as an important source of TNF-α and other cytokines.

TACE knockout mice display a range of epithelial abnormalities. However, the localization of TACE in normal human skin is unclear. In this study, we examined the expression of TACE in normal skin by immunohistochemical analysis. The expression of TACE was seen throughout all layers of the epidermis, the hair follicles, eccrine ducts and glands, and sebaceous glands. There was also staining of blood vessels in the dermis. In particular, TACE was localized predominantly in mast cells, suggesting that these cells are an important source of TNF-α.
