Summary Methemoglobin-reductase deficiency in the red cells has been found in a 11 years old girl. The enzymes of the glycolysis and pentosephosphatcyclus were normal. Further findings were decreased ATP and slightly decreased GSH in the red cells. Lactic acid production was increased. Methemoglobin reduction and CO-absorption were abnormal in the presence of glucose and lactic acid and was normalized by addition of toluidinblue. This abnormalities were considered as a consequence of the methemoglobin reduktase deficiency.

Zusammenfassung Bei einem jetzt lljährigen Mädchen besteht seit der Geburt eine Cyanose, bedingt durch eine kongenitale Methämoglobinämie mit Methämoglobinreduktasemangel. Dieser Enzymmangel ist im Erythrocyten als primäre Störung anzusehen, die anderen Enzyme der Glykolyse und des Pentosephosphatcyclus zeigen Normalwerte. Die weiteren gefundenen metabolischen Besonderheiten — ATP vermindert, GSH gering vermindert, Lactatbildung erhöht, verminderte Methämoglobinreduktion ohne Substrat, mit Glucose und Lactat, durch Toluidinblau normalisiert, ebenso die CO-Aufnahme — sind insgesamt Folgen dieses Enzymmangels.
