    Notes: Summary Enhanced activation of the clotting system has been recently implicated in the pathogenesis of vascular complications in patients with diabetes mellitus. Abnormalities of the anticoagulant system may constitute a potential trigger factor for the haemostatic activation observed in diabetic subjects. The current study aimed to evaluate anticoagulant activity in diabetic patients by assessing the plasma levels of activated protein C-protein C inhibitor complex; and by measuring the anticoagulant response to exogenous thrombomodulin. This study comprised 61 patients (34 men, 27 women) with non-insulin-dependent diabetes mellitus (NIDDM) of whom 22 showed microalbuminuria and 39 normoalbuminuria. Data obtained in 31 non-obese and non-diabetic subjects were available for comparison. The plasma levels of fibrinogen (p 〈 0.02), prothrombin fragment 1 + 2 (p 〈 0.05), fibrin monomer (p 〈 0.0001), protein C antigen (p 〈 0.005), total protein S antigen (p 〈 0.02), soluble thrombomodulin (p 〈 0.005) and soluble E-selectin (p 〈 0.005) were significantly higher in diabetic patients than in healthy subjects. The plasma level of activated protein C-protein C inhibitor complex (7.4 ± 3.8 vs 3.0 ± 0.4 pmol/l) was significantly higher (p 〈 0.0001) and the anticoagulant response to exogenous thrombomodulin (23.4 ± 2.6 vs 35.3 ± 3.0 ng/ml) was markedly lower (p = 0.005) in all diabetic patients than in healthy subjects. Cases with microalbuminuria presented low plasma levels of activated protein C-protein C inhibitor complex (5.5 ± 0.6 vs 8.6 ± 0.7 pmol/l, p 〈 0.05) and significantly decreased values of the anticoagulant response to exogenous thrombomodulin (16.5 ± 2.9 vs 23.4 ± 2.6 %, p = 0.03) as compared to those with normoalbuminuria. The present study suggests that the hyper-coagulable state in NIDDM is associated with an increased activation of protein C but with a poor plasma reactivity to the anticoagulant effect of thrombomodulin.     Notes: Summary The composition of urinary acidic glycosaminoglycans (AGAG) in 4 patients with Werner's syndrome was determined by an enzymatic assay system using chondroitinases and hyaluronidase. In Werner's syndrome, the amount of hyaluronic acid and heparan sulfates in the total AGAG increases. A compositional change in the chondroitin sulfate isomers occurs. The change of urinary AGAG in Werner's syndrome appears to reflect age-related changes.

    Notes: Abstract Six to eight months after the Tokyo subway attack in March 1995, the neurophysiological effects of acute sarin poisoning were investigated in 18 passengers exposed to sarin (sarin cases) in the subways to ascertain the focal or functional brain deficits induced by sarin. The event-related and visual evoked potentials (P300 and VEP), brainstem auditory evoked potential, and electrocardiographic R-R interval variability (CVRR), together with the score on the posttraumatic stress disorder (PTSD) checklist, were measured in the sarin cases and the same number of control subjects matched for sex and age. None of the sarin cases had any obvious clinical abnormalities at the time of testing. The P300 and VEP (P100) latencies in the sarin cases were significantly prolonged compared with the matched controls. In the sarin cases, the CVRR was significantly related to serum cholinesterase (ChE) levels determined immediately after exposure; the PTSD score was not significantly associated with any neurophysiological data despite the high PTSD score in the sarin cases. These findings suggest that asymptomatic sequelae to sarin exposure, rather than PTSD, persist in the higher and visual nervous systems beyond the turnover period of ChE; sarin may have neurotoxic actions in addition to the inhibitory action on brain ChE.

    Notes: Summary Whole cells of Achromobacter butyri OUT 8004 having polyphosphate glucokinase activity were immobilized in polyacrylamide gel. The immobilized cells were activated by organic solvents, especially acetone. The immobilization resulted in increased stability of polyphosphate glucokinase. Continuous high yield production of G-6-P from glucose and metaphosphate was performed with an immobilized cell column, which had a half-life of approximately 20 days.

    Notes: Abstract Endoscopic ultrasound was performed in 56 patients with histologically or clinically proven mediastinal or lung diseases. These 56 patients comprised 12 with sarcoidosis, 8 with lymphoma, 20 with lung cancer, 10 with oesophageal cancer, and 6 with other diseases. A 7.5 MHz electronic linear-arrayed ultrasonic endoscope was used. In 38 of the 56 patients lymph nodes were detected by endoscopic ultrasound. Eleven patients with sarcoidosis showed lymph nodes in a “facet formation” (like stones in an old stone wall), while lymph nodes in the other 27 patients were round in shape and did not form facets. The “facet formation sign”, evaluated by endoscopic ultrasound, may be a characteristic finding in sarcoidosis.

    Notes: Summary Changes of specific response properties of single neurones in area 17 of the cat's cortex were studied during electrophoretic administration of glutamate and gamma-aminobutyric acid (GABA). Most cells were excited by glutamate. The diameter of the cells' discharge field (ERF) was enlarged by less than 1° in most cells, and the amplitude of the response within the ERF was increased. There was a weak correlation between the original size of the ERF and its enlargement during glutamate. Stimuli presented in non-optimal orientations and moved in the non-optimal direction became also effective in eliciting an excitatory response. When glutamate “doses” sufficiently high to increase background activity were used, inhibitory regions of the receptive fields could be revealed. GABA decreased the excitability of the cells until the response was totally blocked. In some cases, glutamate also decreased the excitability of cells. Simultaneous recordings from two cells suggested that the inhibitory effect may be indirect through activation of nearby inhibitory neurones. It is pointed out that extracellular recording combined with electrophoretic administration of glutamate can reveal information about a cell's properties which can otherwise only be obtained with intracellular recordings. The functional implications of the results are discussed.
