    Notes: Misplacement of a tracheal tube in the oesophagus remains a significant cause of mortality and morbidity in anaesthesia, despite decades of effort aimed at prevention, or perhaps more importantly, detection, of such an event. We have evaluated a cheap, simple and quick device which relies mainly on the reflation or otherwise of an Ellick's evacuator applied to the supposed ‘tracheal’ tube. Identical tracheal tubes were passed into the trachea and oesophagus of 100 patients; the left and right position in the mouth was chosen at random. The test was conducted by a second anaesthetist, not present at intubation, and unaware of which tube was in the trachea. There were no false positive results and the correct deduction of which was the tracheal tube was reached in 100 tests using this device. Its use is recommended for widespread evaluation as a valuable adjunct to existing methods of detecting misplacement.

    Notes: The interaction of nitrous oxide and vitamin B12 and its implications are not the exclusive territory of any one discipline. The initial discovery was by a chemist but it is of obvious relevance to anaesthetists and intensivists; some complications are neurological others haematological. The interaction provides an extremely important research tool as the first easily available B12-deficient animal model. Finally there are implications for exposure to contaminated atmospheres in hospitals and in industry.

    Notes: Previously reported figures for the incidence of Adult Respiratory Distress Syndrome, based largely on American results, would suggest that the average British Health District of 200 000 people could expect to see 130 cases per year. This figure is greatly in excess of that expected from our clinical experience. We have undertaken a retrospective survey of patients within a British Health Region (population, 3 599 400). Results indicate an incidence of nine for an average Health District of 200 000 population, or 2.5% of all admissions to intensive therapy units, or 0.045 per 1000 resident population.

    Notes: The accuracy of 21 anaesthetists in estimating tidal volumes from reservoir bag movements was assessed using a model lung apparatus. The breathing system configuration (Mapleson A or D), the grade of anaesthetist, and the years of anaesthetic experience had no effect on accuracy. Greater precision of tidal volume estimation was observed with larger tidal volumes and lower fresh gas flows. The mean systematic error of 18 of the 21 anaesthetists was greater than zero, indicating a general tendency to overestimate tidal volume. This study therefore strengthens the view that clinical observations should be supplemented with information from continuous monitoring devices.

    Notes: A prototype size 3 laryngeal mask was used in 100 patients by 18 anaesthetists with no previous experience of its use. A clear and unobstructed airway was obtained in 98% of patients, without requiring support of the jaw, thus leaving the anaesthetists' hands entirely free. The patency of the airway did not deteriorate during the course of the anaesthetic. In 10 patients there was obstruction of the airway at the first attempt to place it without the introducer and this obstruction appeared to be as a result of downfolding of the epiglottis. Subsequent attempts at passage were successful in all 10 patients. The seal between the mask and the larynx was adequate for artifical ventilation of the patients, but the meun leak pressure was 1.7 k Pa.
