    Notes: summary  The aim of the present study was to evaluate the mechanical strength of the Empress2 system, which is based on the use of a high-strength glass--ceramic core of lithium disilicate, and the fracture resistance of fixed partial dentures fabricated with this material. To evaluate mechanical strength, four types of ceramic materials were tested for four-point flexural strength and diametral tensile strength: Empress2 core material, Empress2 layering porcelain, conventional Empress material and Dicor. Then, using Empress2, conventional Empress and Dicor, actual clinical type anterior fixed partial dentures were fabricated for fracture testing. The results showed that the Empress2 core material, at 329 MPa, has more than twice the flexural strength of conventional materials and at 271 MPa, more than four times the diametral tensile strength of conventional materials. Furthermore, fixed partial dentures fabricated with Empress2 had a fracture resistance of 1424 N. That is, they were more than twice as fracture resistant as fixed partial dentures made with conventional materials.

    Notes: summary The purpose of this study was to evaluate the stress distribution under various loading conditions within posterior all-ceramic crowns. A three-dimensional finite element model representing a lower first molar was constructed. Variations of the model had two types of single layer all-ceramic crowns (Dicor and Empress) and two types of double layer all-ceramic crowns (In-Ceram and Empress2) cemented. A load of 600 N, simulating the maximum bite force, was applied vertically to the crowns. Loads of 225 N, simulating masticatory force, were applied from three directions (vertically, at a 45° angle, and horizontally). In the test simulating maximum bite force, the maximum tensile stresses on all crowns (17·4–19·4 MPa) concentrated around the loading points. In the masticatory force simulation test, the specimens experienced maximum tensile stresses of 19·7–27·0 MPa under a horizontal load and 10·8–10·9 MPa under a vertical load. When the load was applied horizontally, the maximum tensile stress was observed around the loading points on the surface in the case of the single layer crowns, and of the cervical area of the inner core of the double layer crowns. Within the limitation of this study, it was found that the strength of occlusal contact points is important to the integrity of posterior all-ceramic crowns and that bite forces applied from the horizontal direction are a critical factor.

    Notes: Abstract. To determine the rates of change in bone mineral density (BMD) at the spine in healthy Japanese women, longitudinal measurements of spinal BMD using dual X-ray absorptiometry were collected from 984 women over 17 years of age (mean age 51.6) at eight medical research centers. They were followed up for 20.9 months on average without any treatment influencing bone and calcium metabolism. Measurements of BMD obtained by two different scanners were converted into standardized BMD (sBMD) values. The multiple linear regression model predicts that spinal sBMD increases up to about 23 years of age: the estimated average rates of increases were 0.13%/year for women aged 20 years. After the age of 23, the sBMD began decreasing: the rates of loss increased by 0.045%/year for each year increase in age among premenopausal women. In perimenopausal women, the rate of loss was 2.1%/year. In postnatural menopausal women, the rates of loss decreased exponentially with increasing years since menopause. The rates of loss increased by 0.04%/year for 1 kg decrease in body weight or by 0.1%/year for 1 kg/m2 decrease in body mass index. No significant differences in changes in sBMD were found between scanners and between centers after multiple adjustment. We conclude that the rates of change in spinal sBMD are associated with age in premenopausal women, and with years since menopause and weight or BMI in postmenopausal women. Caution is needed, however, when using data from different densitometers to evaluate rates in bone loss in multicenter trials.

Objective: To clarify whether plasma concentrations of soluble P-selectin, platelet factor-4 (PF-4) and β-thromboglobulin (βTG) are altered in patients with septic shock and whether polymyxin B-immobilized fiber (PMX-F) treatment affects these changes.¶Subjects: Thirty patients with septic shock who were treated with PMX-F (group A), 20 such patients who received conventional therapies (group B) and 20 healthy control subjects (group C).¶Methods: ELISA using commercial kits. Endotoxin elimination by direct hemoperfusion using PMX-F.¶Results: Blood endotoxin levels decreased significantly from 49.4 ± 8.8 pg/ml to 13.0 ± 4.5 pg/ml after PMX-F treatment. The pretreatment plasma concentrations of soluble P-selectin, PF-4 and βTG in patients in groups A and B were significantly higher than those in group C (p 〈 0.001). Plasma concentrations of these factors decreased significantly in group A after PMX-F treatment (p 〈 0.01); however, the concentrations in group B were not altered after conventional treatment. The survival rate of group A (60%) was higher than that of group B (30%).¶Conclusions: Our findings suggest that soluble P-selectin, PF-4 and βTG may be associated with septic shock and that PMX-F is effective in reducing these markers in patients with septic shock.

Objective and Design: To evaluate the effect of a newly developed inhibitor of matrix metalloproteinases (MMPs), ONO-4817, on the degradation of cartilage in the guinea pig arthritis model.¶Materials: 42 guinea pigs were used in the arthritis model.¶Treatment: Lipopolysaccharide (LPS) was injected into guinea pig knee joints. The content of proteoglycan released in synovial cavity was measured as a marker of cartilage degradation. ONO-4817, dexamethasone or indomethacin were administered orally to the animals.¶Results: ONO-4817 showed a broad inhibitory activity against MMPs except MMP-1 and MMP-7. The oral administration of ONO-4817 dose-dependently suppressed the release of proteoglycan from the cartilage of the knee joints.¶Conclusion: This study suggests the possibility that a novel MMP inhibitor, ONO-4817 may have a therapeutic utility for MMP-related diseases.
