    Notes: Summary Enhanced activation of the clotting system has been recently implicated in the pathogenesis of vascular complications in patients with diabetes mellitus. Abnormalities of the anticoagulant system may constitute a potential trigger factor for the haemostatic activation observed in diabetic subjects. The current study aimed to evaluate anticoagulant activity in diabetic patients by assessing the plasma levels of activated protein C-protein C inhibitor complex; and by measuring the anticoagulant response to exogenous thrombomodulin. This study comprised 61 patients (34 men, 27 women) with non-insulin-dependent diabetes mellitus (NIDDM) of whom 22 showed microalbuminuria and 39 normoalbuminuria. Data obtained in 31 non-obese and non-diabetic subjects were available for comparison. The plasma levels of fibrinogen (p 〈 0.02), prothrombin fragment 1 + 2 (p 〈 0.05), fibrin monomer (p 〈 0.0001), protein C antigen (p 〈 0.005), total protein S antigen (p 〈 0.02), soluble thrombomodulin (p 〈 0.005) and soluble E-selectin (p 〈 0.005) were significantly higher in diabetic patients than in healthy subjects. The plasma level of activated protein C-protein C inhibitor complex (7.4 ± 3.8 vs 3.0 ± 0.4 pmol/l) was significantly higher (p 〈 0.0001) and the anticoagulant response to exogenous thrombomodulin (23.4 ± 2.6 vs 35.3 ± 3.0 ng/ml) was markedly lower (p = 0.005) in all diabetic patients than in healthy subjects. Cases with microalbuminuria presented low plasma levels of activated protein C-protein C inhibitor complex (5.5 ± 0.6 vs 8.6 ± 0.7 pmol/l, p 〈 0.05) and significantly decreased values of the anticoagulant response to exogenous thrombomodulin (16.5 ± 2.9 vs 23.4 ± 2.6 %, p = 0.03) as compared to those with normoalbuminuria. The present study suggests that the hyper-coagulable state in NIDDM is associated with an increased activation of protein C but with a poor plasma reactivity to the anticoagulant effect of thrombomodulin.     Notes: Summary Multiple intradural xanthomatous tumors developed in 48-year-old female with familial phytosterolemia. These tumors were restricted to the spinal denticulate ligaments. Histological and immunohistocheimical findings were fundamentally similar to those of tendinous xanthomas. The major cellular component of these tumors were identified as of mono-histiocytic origin because they possessed myeloid histiocytic antigen (Mac 387), CD11c and lysozyme but not CD15. Sitosterols, campesterols and cholestanols were recovered from the extract of the tumors and the lesions were confirmed to be phytosterolemic xanthomas. Schwann cells stained with anti-S100 protein were confined to the perivascular small nerve bundles and did not show xanthomatous change. Although immunohistochemical preparation of epithelial membrane antigen and desmoplakin I+II revealed the presence of non-neoplastic meningothelial cells in the superficial portion of the tumors, they were too few to play a significant role in the development of these xanthomas.

    Notes: Abstract  Laboratory culture experiments were performed to study the changes in size-fractionated Fe concentrations during the growth of the oceanic diatom Chaetoceros sp. Fe concentration was estimated for three size fractions: large labile particles (〉0.2 μm), small colloidal particles (0.2 μm to 200 kDa) and soluble species (〈200 kDa). The size-fractionated Fe concentration in the nutrient-enriched filtered seawater medium without diatom cells became stable within 4 d after the spike of FeCl3 solution. Light irradiation by white fluorescent tubes with a 14 h light:10 h dark cycle did not significantly alter concentrations of the size-fractionated Fe. For the phytoplankton culture experiment, Fe-starved diatom cells were inoculated into the nutrient-enriched media aged for 19 d after the addition of FeCl3. With the growth of diatom cells, total acid-labile Fe concentrations decreased from 0.60 to 0.46 nM during 7 d of incubation. However, only the concentration of the small colloidal particles showed a significant decrease; the concentration of the other size fractions remained relatively constant. Although the media still contained sufficient amounts Fe as large labile particles and soluble species, diatom cells appeared to be Fe-limited once Fe as small colloidal particles had been used up. These results suggest that Fe in the small colloidal particle fraction was the most dynamic size fraction during the growth of the diatom Chaetoceros sp. In addition, to better understand Fe dynamics in the ocean, we must consider the influence of phytoplankton growth on small colloidal Fe [which is typically included in the dissolved Fe fraction (〈0.2 μm)].

    Notes: Abstract We describe a successful treatment, intramedullary fibular grafting, for a patient with a supracondylar fracture of the femur following total knee arthroplasty in which the fracture region was comminuted and a segmental large bone defect was present. Free autogenous fibular was inserted into the medullary cavity from the intercondylar region and fixed to the proximal fragment of the femur with augmented fixation using a small plate and screws. Our procedure makes it easy to perform large segmental bone grafting and internal fixation at the same time for such a condition.
