Notes: Abstract Phagocyte (neutrophil and monocyte) function was evaluated in a boy with glycogen storage disease type Ib. Neutrophils were found to be defective in motility and respiratory burst and monocytes showed a defect in respiratory burst but not in motility. These results suggested that the glucose-6-phosphate transport system plays a role in the function of neutrophils and monocytes and that these two phagocytes are different from each other in their energy metabolism for motility.

Notes: Summary ¶ An unruptured internal carotid artery (ICA) aneurysm arising at the origin of a hyperplastic anomalous AchoA was identified together with a second unruptured middle cerebral artery aneurysm during angiography performed to investigate a striatal and intraventricular haemorrhage in a 55-years-old woman. The anomalous hyperplastic AchoA supplied the left temporal and occipital lobes, and the aneurysm arose proximal to its origin. The patient underwent clipping of the aneurysms, and intra-operative observation revealed that several perforating branches arose directly from the ICA between the AchoA and the ICA bifurcation.

Description / Table of Contents: Zusammenfassung Hämatit aus Pyrophyllitlagerstätten des Shobara Distriktes, SW Japan Hämatit ist ein typisches Mineral in Liesegang-Gesteinen aus Pyrophyllitlagerstätten des Shobara Distriktes, südwestliches Japan.
Notes: Summary Hematite is a characteristic mineral of Liesegang rock in the pyrophyllite ore deposits, Shobara district, southwestern Japan. The mineral intimately coexists with fine-grained pyrophyllite and quartz. The hematite crystals are characteristically needle-shaped to fibrous, and several microns in length, and can only be observed by high-magnification reflected light microscopy with oil immersion lens. They occur as ring-shaped aggregates several tens of microns in diameter. Moreover, the crystals are often arranged radially within the rings. Microprobe analyses reveal that hematite crystals are almost pure iron oxide, with less than 0.9 wt.% Ti. X-ray powder diffraction patterns and reflection intensities indicate that hematite is the only iron mineral in the Liesegang rock, i.e., no hydrous iron oxides are present. This observation was also confirmed by examinations of water content and infrared spectroscopy.

Notes: Abstract Bacterial adherence to the surfaces of indwelling urethral (Foley) catheters is briefly reviewed. In addition, crystal adhesion to the catheters is described. Using scanning electron microscopy, micro-organisms including fungi and crystals are frequently observed in association with fibrillar material. Based on our study using the immunofluorescence method, some of the fibrillar material is considered to be fibrin. On the surfaces of catheters, especially those indwelling for long periods, microorganisms are often embedded in biofilms, which are composed of an amorphous matrix. Urease-producing bacteria adherent to the catheters are considered to play an important role in the formation of the calculous encrustation.

Notes: Abstract Although candiduria is not an infrequent finding, its clinical significance is uncertain. Herein the pertinent literature including ours on candiduria and related conditions are reviewed, with emphasis on methods of differentiating true urinary candidiasis from simple colonization, using colony counts in urine specimens, antibody-coated bacteria test, serologic diagnosis including serum antibody titer, Candida antigen and metabolite assays. The predisposing factors and treatment in relation to antifungal susceptibility tests are also presented.

Notes: Abstract Four patients with pontine gliomas were treated with radiation and intra-arterial chemotherapy using ACNU, cisplatin, and/or carboplatin. All underwent biopsy and were histologically proven to have gliomas. Even though three were benign histologically, all appeared malignant on neuroimaging. Two patients had complete remissions; the others had partial responses; three patients died, 36, 14, and 15 months after the primary diagnosis. One patient, now in complete remission, has been alive for 24 months. There was no neurotoxicity except for transient nausea and vomiting. Intra-arterial chemotherapy is effective against brain stem gliomas, producing remission with minimum side effects.

Notes: Abstract The diagnostic value of determination of serum mannan levels was studied in 41 patients with candiduria and 20 control patients without candiduria. The patients with candiduria comprised 18 asymptomatic afebrile patients (group 1) and 23 febrile patients with suspectedCandida pyelonephritis (group 2). Mannan antigenemia (≥0.5 ng/ml) was detected in 14 patients in group 2, this rate being significantly higher than the rate of four patients in group 1 (p〈0.05) and none in the control group (p〈0.01). Serum mannan levels decreased gradually and became undetectable in six patients in whom therapy with an anticandidal agent was effective. Determination of serum mannan levels may be a useful parameter in diagnosingCandida pyelonephritis and deciding on effective treatment.

Notes: Abstract To evaluate serum markers for the diagnosis ofCandida pyelonephritis, levels of D-arabinitol, candidal mannan antigen and candidal protein antigen were measured using an enzymatic fluorometric assay, an enzyme immunoassay and a latex agglutination assay, respectively. The study group comprised 36 patients with candiduria (11 disseminated candidiasis, 9Candida pyelonephritis and 16 colonization) and 27 without candiduria. The D-arabinitol/creatinine ratio was the only marker that differentiated betweenCandida pyelonephritis and colonization (p〈0.01). Using the cut-off values (1.4 µmol/mg of D-arabinitol/creatinine ratio, 0.28 of mannan/creatinine ratio (×104) and 1:4 of protein antigen titer) D-arabinitol/creatinine ratio showed the highest sensitivity inCandida pyelonephritis (77.8 %) as compared to mannan/creatinine ratio (55.6 %) and protein antigen titer (11.1 %) although there were no differences in specificity, positive predictive value or negative predictive value. These results suggest that D-arabinitol/creatinine ratio is the most useful marker ofCandida pyelonephritis.
