    Description / Table of Contents: Zusammenfassung In vitro-Untersuchungen haben ergeben, daß die Adsorptionskapazität von Aktivkohle (‘Kohle-Compretten’, ‘Ultracarbon’, E. Merck, Darmstadt, FRG), ebenso groß ist, wie diejenige von ‘Fuller's Earth’ (Surrey powder, Laporte Ind. Ltd., Luton, GB) und ‘Bentonit BP W.B.’ (Steetly Minerals Ltd., Milton Keynes, GB). ‘Fullererde’ einer anderen Firma hatte so geringe Adsorptionsfähigkeit, daß sie zur Behandlung der Paraquat-Vergiftung nicht geeignet ist. In Tierversuchen konnte gezeigt werden, daß der kurative Effekt von Aktivkohle, die 0,5, 1, 2 und 3 Stunden nach der Ingestion von 200 bzw. 300 mg Paraquat pro kg Körpergewicht verabfolgt wurde, ebenso gut oder sogar besser ist, als diejenige von ‘Fuller's Earth’ oder ‘Bentonit BP W.B.’. Aktivkohle ist ein gleichwertiger Ersatz der Mineralerden.
    Notes: Summary In vitro investigations have shown that the adsorption capacity of activated charcoal (‘Kohle-Compretten’, ‘Ultracarbon’, E. Merck, Darmstadt, FRG) is just as high as that of ‘Fuller's earth’ (Surrey powder, Laporte Industries Ltd., Luton, GB) or ‘Bentonite BP W.B.’ (Steetley Minerals Ltd., Milton Keynes, GB). Fuller's earth (‘Fullererde’) from another manufacturer has had very poor adsorption properties and is thus not suitable for the treatment of paraquat poisoning. Animal experiments have shown that the curative effect of activated charcoal given 0.5, 1, 2, and 3 h after ingestion of 200 and 300 mg paraquat/kg body weight is equally as good or even better than that of ‘Fuller's earth’ or ‘Bentonite BP W.B.’. Activated charcoal is a substitute of equal value to these mineral soils.

    Description / Table of Contents: Zusammenfassung Die von Chang entwickelte Methode, artefizielle Zellen — in Kollodium und Albumin verkapselte Aktivkohle — zur Hämoperfusion einzusetzen, wurde in vitro studiert. Geprüft wurde die Kapazität zur Adsorption von schlecht wie gut dialysablen Substanzen (Barbiturate, Diazepam, Parathion) und einem endogen toxischen Metaboliten (p-Hydroxyphenylessigsäure). — Mit der Adsorptions-Einheit lassen sich in 90 min 80–90% der Ausgangskonzentrationen von Barbiturat, Diazepam und p-Hydroxyphenylacetat eliminieren, von Parathion etwa 50%, Bromid und Ammoniak werden nicht adsorbiert. Versuche, durch den Einbau von Heparin in die Hülle der Kohlegranula die regionale Heparinisierung unnötig zu machen, waren erfolglos. Bei sachgemäßer Herstellung des Shunt-Systems, Einhaltung steriler Kautelen und regionaler Heparinisierung ist der Einsatz des „Entgiftungs-Shunts“ eine vielversprechende Möglichkeit zur Therapie schwerer exogener und endogener Intoxikationen, die einer konventionellen Therapie nicht zugänglich sind.
    Notes: Summary The method developed by Chang for using in collodion and albumin encapsuled charcoal as artificial cells for hemoperfusion has been studied in vitro. The capacity for the absorption of more or less dialysable exogenous substances (barbiturate, Diazepam, Parathion) and an endogenous toxic metabolite (p-hydroxyphenyl acetic acid) has been examined. With the absorption unit one can eliminate 80 to 90 per cent of the initial concentration of barbiturate, Diazepam and p-hydroxyphenyl acetic acid in about 90 min, of Parathion about 50 per cent in the same time, whereas bromide and ammonia are not absorbed. Attempts to avoid regional heparinisation by the incorporation of heparin into the capsule of charcoal granules were not successful. Assuming correct preparation of the hemoperfusion system, observance of steril precoutions and regional heparinisation, the use of the “detoxication shunt” is a promising means in the treatment of severe exogenous and endogenous intoxications which are not accessible by conventional therapeutic means.

    Description / Table of Contents: Summary “Continuous hemoperfusion” (8 h/day for 2–3 weeks) was performed in two patients suffering from severe paraquat intoxication. On account of paraquat plasma concentrations a fatal outcome due to pulmonary fibrosis would have been expected in both cases. However, both patients survived following “continuous hemoperfusion” therapy. Coated activated charcoal seems to have a higher affinity for paraquat than lung tissue.
    Notes: Zusammenfassung „Kontinuierliche Hämoperfusion“ (ca. 8 h täglich während 2–3 Wochen) wurde wegen Paraquatvergiftung an zwei Patienten durchgeführt, bei denen aufgrund der Höhe der Paraquat-Plasma-Spiegel mit der Entwicklung einer letalen Lungenfibrose gerechnet werden mußte. Beide Patienten konnten durch diese Maßnahme der „Kontinuierlichen Hämoperfusion“ gerettet werden. Beschichtete Aktivkohle scheint eine größere Affinität für Paraquat zu haben als die Lungen.

    Description / Table of Contents: Zusammenfassung Bei einer 38jährigen Patientin mit einer schweren peroralen Lauge-Intoxikation ohne Schock entwickelte sich trotz Substitutions-Therapie eine über 8 Tage andauernde metabolische Azidose. Die metabolische Azidose wurde durch eine Akkummulation von Milchsäure verursacht. Die gesteigerte Laktatproduktion in den Randbezirken verätzter Areale des Verdauungstrakts sowie ein gestörter hepatischer Laktatabbau sind mit großer Wahrscheinlichkeit für die Laktazidose bei Lauge-Intoxikation verantwortlich.
    Notes: Summary A woman 38 years of age suffering from severe peroral lye intoxication, without shock, developed prolonged metabolic acidosis lasting for 8 days despite substitution therapy. Metabolic acidosis was caused by lactic acid. The increased lactic acid production in the marginal regions of the corroded upper digestive tract as well as the impaired lactate removal by the injured liver were most probably responsible for this lactic acidosis.

    Description / Table of Contents: Summary Since there is no widely used causal means of reducing the severity of massive digitalis intoxication the capability of hemoperfusion with coated activated charcoal to remove toxicologically relevant amounts of digoxin and digitoxin was evaluated in vitro and in man. At a blood flow rate of 100 ml/min the digoxin clearance by hemoperfusion in vitro was 51±8 ml/min in comparison to 24.3±11.3 ml/min by hemodialysis. The average hemoperfusion clearance of digitoxin was 31.7±13.4 ml/min, whereas almost no digitoxin was removed by hemodialysis. These clearance values point to the ability of hemoperfusion of eliminating digitalis glycosides from the blood. They do not clarify the essential question whether it is possible to lower the toxic concentrations in the tissues. In two patients being on hemoperfusion for other reasons, 0.5 mg digoxin were injected intravenously as a bolus 1 h prior to the beginning of hemoperfusion and 0.125 mg/h were infused continuously over 4h simultaneously with hemoperfusion. By an average digoxin clearance of 77 ml/min, only 5 and 4.5% of the dose given were removed by this procedure. In 2 patients receiving digitoxin under the same trial conditions an average of 24% of the digitoxin load were eliminated by 4 to 6 h hemoperfusion period although the clearance values obtained were below the clearance for digoxin. The lack of effectiveness in eliminating toxicological relevant amounts of digoxin is due to the quite high distribution volume which results in high tissue concentrations and low blood concentrations of the drug. On the other hand the effective removal of digitoxin is due to the appreciably smaller distribution volume and suggests that hemoperfusion may be a valuable method of rapid reversal of advanced digitoxin toxicity in man.
    Notes: Zusammenfassung Bisher gibt es keine allgemein anwendbare kausale Therapie lebensbedrohlicher Digitalis-Intoxikationen. Daher wurde in vitro und in vivo beim Menschen untersucht, ob die Hämoperfusion mit beschichteter Aktivkohle toxikologisch relevante Mengen von Herzglykosiden zu eliminieren vermag. Bei einer Blutumlaufgeschwindigkeit von 100 ml/min betrug in vitro die Digoxin-Clearance durch Hämoperfusion 51±8 ml/min, im Vergleich zu 24,3±11,3 ml/min durch die Hämodialyse. Für Digitoxin fand sich eine Clearance von 31,7±13,4 ml/min durch Hämoperfusion, während bei Hämodialyse kein meßbarer Anteil des Glykosids eliminiert wurde. Diese Clearence-Werte lassen erwarten, daß die Herzglykoside durch die Hämoperfusion aus dem Blut eliminiert werden, ohne daß allerdings durch die in vitro-Untersuchung eine Aussage möglich ist, ob toxische Glykosidkonzentrationen im Gewebe hierdurch gesenkt werden können. Bei 2 Patienten, bei denen aus anderen Gründen eine Hämoperfusion durchgeführt wurde, wurde Digoxin nach einer Bolusinjektion von 0,5 mg mit einer konstanten Geschwindigkeit von 0,125 ml/h infundiert und der Effekt der Hämoperfusion ermittelt. Bei einer mittleren Digoxin-Clearance von 77 ml/min wurden durch eine 4- bzw. 6-stündige Hämoperfusion nur 5% und 4,5% der applizierten Dosis eliminiert. Dahingegen wurden bei gleichem Vorgehen bei 2 anderen Patienten im Mittel 24% der verabreichten Digitoxin-Dosis durch eine 4- bzw. 6stündige Hämoperfusion eliminiert, obwohl die Digitoxin-Clearance deutlich unter der Digoxin-Clearance lag. Die geringe Effektivität der Hämoperfusion bei der Digoxinelimination ist auf das große Verteilungsvolumen dieses Glykosids zurückzuführen, welches durch hohe Gewebe-und niedrige Plasmakonzentrationen gekennzeichnet ist. Andererseits ist die gute Wirkung der Hämoperfusion in der Elimination von Digitoxin auf das deutlich kleinere Verteilungsvolumen zurückzuführen und läßt den Einsatz der Hämoperfusion bei lebensbedrohlicher Digitoxin-Intoxikation sinnvoll erscheinen.

    Notes: Summary A fatal paraquat poisoning can occur when relatively large areas of skin are contaminated with a concentrated solution of paraquat (Gramoxone). A paraquat absorption takes place of the same magnitude as that with an equal dose per os.     Description / Table of Contents: Zusammenfassung Es wurde der Einfluß von Hypoxämie und Acidose auf den Gallenfluß, die hepatische Bilirubinelimination sowie die Konzentration von „direktem” und „indirektem” Bilirubin im Plasma Nitrostigmin-(E 605 forte®) vergifteter Ratten untersucht. Die Kombination von Hypoxämie und Acidose führt bei Nitrostigmin-vergifteten Batten zu einer signifikanten Abnahme des Gallenflusses und der hepatischen Bilirubinclearance. Die Nitrostigminvergiftung (4 μl/kg KG E 605 forte®) beeinträchtigt die exkretorische Leberfunktion nicht, wenn durch künstliche Hyperventilation (geringfügiger Anstieg des arteriellen pH-Wertes) physiologische pO2-Werte im arteriellen Blut aufrecht erhalten werden. Konsequenzen für die Behandlung von Vergiftungen mit Nitrostigmin in der Humanmedizin werden diskutiert.
    Notes: Abstract The influence of hypoxemia and respiratory acidosis on bile flow, hepatic elimination of bilirubin and the concentration of conjugated and unconjugated plasma bilirubin was studied in nitrostigmine (E 605 forte®) poisoned rats. The combination of hypoxemia and respiratory acidosis in rats poisoned with nitrostigmine results in a significant decrease in bile flow and hepatic clearance of bilirubin. Nitrostigmine poisoning (4 μl/kg b.w. E 605 forte®) does not adversely affect hepatic excretory function if arterial pO2 is maintained within the normal range by artificial hyperventilation (slight rise in arterial pH). The consequences of these studies for the treatment of human nitrostigmine intoxication are discussed.

    Description / Table of Contents: Zusammenfassung Klinische Untersuchungen und Laboratoriumsversuche wurden durchgeführt, um die Wirksamkeit von Darmspülung, Hämodialyse und Hämoperfusion bei Paraquat- und Deiquat-Vergiftungen zu prüfen. Bei einem Patienten wurde 30 Std nach Deiquat-Aufnahme durch Darmspülung 130mal mehr Deiquat entfernt als durch vollständige Aspiration des Mageninhaltes. In vitro-Versuche ergaben, daß bei Blutserumkonzentrationen von 1–2 ppm, die bei Vergiftungen oft gemessen werden, durch Hämodialyse keine toxikologisch relevanten Paraquat- oder Deiquat-Mengen entfernt werden können. Dagegen erwies sich die Hämodialyse bei 20 ppm und einer Blutumlaufgeschwindigkeit von 100 ml/min mit einer Clearance von 70 ml/min als wirksam. Die Hämoperfusion mit beschicheter Aktivkohle war in diesen Versuchen aber eindeutig überlegen, denn insbesondere bei Konzentrationen um 1–2 ppm waren die Clearance-Werte 5–7mal höher als bei der Hämodialyse. Die in vitro-Ergebnisse wurden bei einem Patienten mit einer Paraquat-Vergiftung bestätigt: Bei Konzentrationen unter 1 ppm war die Hämodialyse wirkungslos, während durch Hämoperfusion relativ hohe Clearance-Werte erreicht wurden, so daß der Serumspiegel rasch unter die Nachweisgrenze abfiel.
    Notes: Abstract Clinical and in vitro investigations were carried out to test the efficacy of gut lavage, hemodialysis, and hemoperfusion in the treatment of poisoning with paraquat or diquat. In a patient suffering from diquat intoxication 130 times more diquat was removed by gut lavage 30 h after ingestion than was removed by complete aspiration of the gastric contents. Determination of in vitro clearances for paraquat and diquat by hemodialysis showed that, at serum concentrations of 1–2 ppm, such as are frequently encountered in poisoning in man, toxicologically relevant quantities of herbicide cannot be removed from the body. At a concentration of 20 ppm, on the other hand, hemodialysis proved to be effective, the clearance being 70 ml/min at a blood flow rate of 100 ml/min. The efficacy of hemoperfusion with coated activated charcoal was on the whole better. Especially at concentrations around 1–2 ppm, the clearance values for hemoperfusion were some 5–7 times higher than those for hemodialysis. In a patient suffering from paraquat poisoning, both hemodialysis as well as hemoperfusion were carried out. The in vitro results could be confirmed: At serum concentrations of paraquat less than 1 ppm no clearance could be obtained by hemodialysis while by hemoperfusion with activated charcoal quite high clearance values were measured and the serum level dropped down to zero.

    Description / Table of Contents: Zusammenfassung Resorptionsversuche mit Obidoxim (Toxogonin®) an abgebundenen armschlingen der Ratte in situ sowie Bestimmungen der Blutkonzentration des Antidots nach Fütterung an nichtbetäubten Ratten zeigen, daß Obidoxim nur sehr schwer aus dem Magen-Darmtrakt resorbiert wird. Bei der Prüfung verschiedener Adjuvantien zur Resorptionsförderung erwies sich das Dinatriumsalz der Äthylendiamintetraessigsäure (Na2-EDTA) am wirksamsten: Na2-EDTA erhöht den Blutspiegel von Obidoxim um 50%. Die Resorption von Obidoxim findet nicht im Magen, sondern im Dünndarm statt. Dragées mit magenresistenter Decke, die sowohl Obidoxim als auch Na2-EDTA enthielten, führten weder an Hunden (mit einer Ausnahme) noch an 3 freiwilligen Versuchspersonen zu einem meßbaren Blutspiegel von Obidoxim. Die Vergiftung mit cholinesterasehemmenden Alkylphosphaten oder die Vorbehandlung mit dem Parasympathomimeticum Carbachol fördert die gastroenterale Resorption von Obidoxim an Ratten und Hunden in erheblichem Ausmaß.
    Notes: Summary Experiments in which the absorption of obidoxime was measured in situ in isolated loops of rat intestine as well as blood level determination of the antidote after feeding it to non-anesthetized rats demonstrated that obidoxime is absorbed with considerable difficulty from the gastrointestinal tract. The disodium salt of ethylendiaminetetraacetic acid (Na2 EDTA) proved to be the most effective of the various adjuvants of absorption tested: Na2 EDTA raised the blood level of obidoxime by 50%. The absorption of obidoxmie takes place in the small intestine and not in the stomach. Measurable blood levels of obidoxime could not be obtained in either dogs (with one exception) or 3 human volunteers after oral administration of tablets containing both obidoxime and Na2 EDTA and having a stomach resistent coating. The absorption of obidoxime from the gastrointestinal tract of rats and dogs is markedly enhanced by cholinesterase inhibiting organophosphates and by pretreatment with the parasympathomimetic agent carbacholum.

Der Reaktionsablauf wurde über die Abnahme der Somankonzentration und die Bildung von p-Nitrophenol, einem Reaktionsprodukt von Paraoxon, gaschromatographisch gemessen. Oximkonzentrationen von 5×10−4 M (HS-3, Obidoxim) bzw. 10−3 M (HS-6, 4-PAM) führen zu einer Halbwertzeit des Somanumsatzes von 14 bis 19 min bei pH 7.4 und 37 °C. Um entsprechende Halbwertszeiten des Paraoxonumsatzes mit Oximen zu erreichen, sind um 2 Zehnerpotenzen höhere Oximkonzentrationen nötig. Inwieweit diese Direktreaktion als Antidotprinzip therapeutische Bedeutung bei der Somanoder Paraoxonvergiftung hat wird diskutiert.
    Notes: Abstract The direct interaction of soman and paraoxon with pyridinium aldoximes was studied in vitro. The kinetics of reaction were measured gaschromatographically from decrease of soman concentration and rise of p-nitrophenol, a reaction product of paraoxon. Using oxime concentrations of 5×10−4 M (HS-3, Obidoxim) and 10−3 M (HS-6, 4-PAM) respectively, half time of soman is in the range of 14 to 19 min at pH 7.4 and 37 °C. To obtain similar half times for the reaction between paraoxon and oximes the concentration of oximes has to be increased two powers of ten. The question whether this direct interaction can be used as an antidote principle in therapy of soman or paraoxon intoxication is discussed.
