    Notes: Summary In sixteen patients with arterial hypertension and differing degrees of renal function the pharmacokinetics and haemodynamic effects of nitrendipine have been studied after treatment for 7 days. The AUC (0–24) and the elimination half-life of nitrendipine were significantly increased; the AUC (0–24) in patients with renal failure (median creatinine clearance 27.1 ml × min−1) was 196 ng × ml−1 × h compared to 97.8 ng × ml−1 × h in control subjects (median creatinine clearance 94.4 ml × min−1). The corresponding elimination half-lives were 13.5 h in renal failure and 4.4 h in the controls. The haemodynamic effects of nitrendipine were not enhanced in the patients.

    Description / Table of Contents: Summary The successful late outcome after cardiac transplantation is hampered mainly by the occurence of either chronic rejection, presenting as cardiac allograft vasculopathy or malignant tumors. With 4% in a population of organ recipients the incidence of malignancies is increased significantly when compared to the normal population. After solid organ grafting, mainly lmyphoma and tumors of the skin are seen. We herein report the case of an extragonadal seminoma occuring after cardiac transplantation. Treatment by reduction of immunosuppression in conjunction with monochemotherapy was successful. The underlying pathogenetic processes are discussed.
    Notes: Zusammenfassung Der Spätverlauf nach einer erfolgreichen Herztransplantation wird im wesentlichen von der Entwicklung einer chronischen Abstoßung mit Transplantatvaskulopathie und dem Auftreten maligner Neubildungen beeinflußt. Die Inzidenz maligner Tumore ist mit 4% in einer Organempfängerpopulation deutlich gegenüber der Normalbevölkerung gesteigert. Besonders maligne Lymphome und Hauttumore treten gehäuft auf. Wir berichten hier den Fall eines extragonadalen Seminoms nach Herztransplantation, das durch Reduktion der Immunsuppression und Monochemotherapie erfolgreich behandelt werden konnte und diskutieren die möglichen pathogenetischen Zusammenhänge.

    Notes: Summary This study compared the pharmacokinetics of orally administered isosorbide-5-mononitrate (20 mg) in patients with and without signs of ischemic heart failure after acute myocardial infarction. The central venous pressure was used as a parameter of global myocardial function and to separate 17 patients into two groups with normal (group I: CVP〈6 cmH2O; Killip class I;n=9) and elevated (group II: CVP〉-6 cmH2O; Killip class II–III;n=8) pressure. As compared to subjects with normal CVP patients with hemodynamic impairment showed a markedly lower peak concentration of isosorbide-5-mononitrate levels (475±32 vs 663±38 ngml−1;p〈0.01; mean±SEM) which occurred delayed (32±6 vs 55±9 s;p〈0.05). Reduced cardiac function also resulted in a prolonged elimination of isosorbide-5-mononitrate as was suggested by the diminished elimination rate constant (0.14±0.01 vs 0.18±0.01 h−1;p〈0.05). Thus, in patients with ischemic heart failure cardiac performance influences both the absorption and apparent elimination phase of oral isosorbide-5-mononitrate.
