    Notes: Trichilemmoma is considered a benign neoplasm derived from the outer root sheath of the hair follicle. Although the histogenesis of the lesion is unknown, a relationship between human papillomavirus (HPV) infection and development of these tumors has been suggested on morphologic observations. In order to determine whether HPV is, in fact, present in these lesions, we have analyzed sections from 25 formalin-fixed, paraffin-embedded trichilemmomas for the presence of occult HPV DNA using ultrasensitive DNA amplification technology. Under carefully controlled conditions normally effective in detecting HPV from condyloma or verrucae, we were unable to detect an HPV-induced amplimer in agarose gels or their corresponding Southern blots using radiolabeled probes. Our results do not support the hypothesis that trichilemmomas arise from HPV infection.

    Notes: Tissue obtained from formalin-fixed, paraffin-embedded blocks containing molluscum contagiosum virus was used as a source of antigen to generate a polyclonal antibody against molluscum contagiosum virus. This antibody recognized an antigen in molluscum bodies which was preserved during routine fixation and embedding. The antibody can be used with standard immunohistochemical methods to identify molluscum contagiosuminfection in inflamed and distorted tissue specimens. Our data indicate that paraffin-embedded, formalin-fixed material can be used as an antigenic source to generate antibodies with diagnostic potential.

    Notes: Langerhans cells (LC) were identified and quantitated in non-inflamed keratoacanthoma (KA), inflamed KA, non-inflamed squamous cell carcinoma (SCC), and inflamed SCC, by their content of S100 protein. The number of LCs per high-power field was markedly increased in inflamed KA when compared to the other groups. Using similar methods on frozen sections, the expression of HLA-DR was identified on keratinocytes in KA in areas of inflammation but not in other lesions under study. We hypothesize that increased numbers of LCs in inflamed KA are part of the process which results in tumor regression.

    Notes: Cutaneous papillomavirus infection is common in patients who are immunosupprcsscd. We describe swollen keratinocytes in the granular layer in lesions from four patients who had human immunodeficiency virus infection. These cells were similar to those described in skin lesions of epidermodysplasia verruciformis. Amplification of DNA from the lesions revealed an amplimer for human papillomavirus using a consensus primer for a highly conserved region of the LI open reading frame: however, specific binding was not noted when radiolabelled probes for human papillomavirus types 6, 11, 16, 18, and 33 were used. We conclude that the presence of these distinctive swollen cells strongly suggests immunosuppression and quite possibly infection by a less common papillomavirus type.

    Notes: Using standard immunohistochemical methods, routinely processed sections, and a polyclonal antibody to Type IV collagen, we have determined the location of Type IV collagen, a substance located in the lamina densa of basement membranes, in a spectrum of acquired subepidermal bullous diseases. Type IV collagen was attached to the blister roof in five cases of well-established epidermolysis bullosa acquisita and to the blister base in 25 cases of bullous pemphigoid, four cases of dermatitis herpetiformis and 12 cases of porphyria cutanca tarda. Immunohistochemical localization of Type IV collagen in epidermal-dermal basement membrane is a simple, rapid and reliable technique which can be utilized to exclude and possibly to confirm the diagnosis of epidermolysis bullosa acquisita in routinely fixed paraffin-embedded tissues.

    Notes: Using a rabbit anti-human liver ferritin antibody, we examined the binding patterns of this reagent in normal skin and observed a unique binding pattern limited to the outermost layer of the eccrine duct. Examination of a variety of sweat gland neoplasms revealed 2 distinct patterns. One was the binding of this antibody to the outermost layer of cells in the epithelial cords of syringoma, producing a characteristic ring when seen in cross-section. This pattern of binding did not occur in other neoplasms known to be related to the eccrine duct such as dermal duct tumor and eccrine poroma. Only sparse sporadic binding occurred in other eccrine and apocrine neoplasms. A second characteristic binding pattern, not related to that noted in syringoma and diffuse in pattern, was seen in acrospiroma and in a number of adnexal carcinomas. Diffuse ferritin expression has been described in malignant neoplasms in tissues other than skin. Diffuse ferritin staining of certain sweat gland neoplasms may be an indication of biologic activity and potential aggressivity of these neoplasms.

    Notes: Factor XIIIa (FXIIIa), a blood and intracellularly produced coagulation factor, has been found in a variety of cell types including fibroblast-like mesenchymal cells, and has been shown to stimulate the proliferation of fibroblasts and some neoplastic cells in vitro. We have already shown that the dendritic fibroblasts composing the fibrous papule contain this factor. We hypothesized that histopathologically similar fibrovascular tumors may also express FXIIIa and, in this report, show that the large stellate fibroblasts found in acquired digital fibrokeratomas, angiofibromas (adenoma sebaceum of Pringle), and oral fibroma (oral fibrous hyperplasia) also express FXIIIa. We postulate that FXIIIa, possibly acting as a growth factor, may be a common denominator in the pathogenesis of these tumors. Another possibility is that these tumors may be the consequence of a local overproduction of FXIIIa in response to an, as yet, unidentified stimulus.

    Notes: Biopsies of morpheaform basal cell carcinomas (MBCCs) were separated into 2 groups based on histological features: 1) a pure morpheaform group with narrow elongated strands of basaloid cells in a dense fibrous stroma; and 2) a complex morpheaform group with pure morpheaform regions as well as regions with other histological patterns (nodular, etc.). Analysis by sex and body location revealed a female predominance in the pure morpheaform group. Lesions were found primarily on the mid-face in women with pure MBCCs. Using a monoclonal antibody which identifies eccrine duct in routinely fixed tissues, we could not identify eccrine ductal differentiation in MBCCs.

    Notes: Human hair follicles (HF) and sebaceous glands (SG) were assessed for the presence and distribution of the cytochrome P-450-aromatase (AR) and 3B-hydroxysteroid dehydrogenase (3B-HSD) enzymes. Immunohistochemical methods were used to examine both enzymes in male and female human skin specimens at various ages and different body sites. AR was found in the external root sheath of anagen, terminal HF, and in SG, whereas the 3B-HSD was found only in the SG. AR was rarely found in telogen HF. The expression of both enzymes, AR and 3B-HSD, did not vary with body site or sex. Localizing AR in the external root sheath of anagen HF suggests that AR may have a function in the HF cycle. We hypothesize that AR may be one of many enzymes or factors that play a role in the HF cycle by regulating the level of androgens formed locally, whereas 3B-HSD is localized in SG, converting weak androgen precursors to potent androgens, stimulating lipogenesis.

    Notes: Medical knowledge has been significantly expanded by the techniques of molecular genetics. A new technology, the polymerase chain reaction (PCR) (1–6), has produced a quantum leap in the field of molecular genetics. PCR uses an in vitro chemical reaction to amplify impure DNA, either fragmented or intact. A defined DNA fragment can be amplified a millionfold in a relatively brief incubation (a few hours). The ability to amplify minute quantities of crude DNA gives the method extraordinary power and sensitivity, DNA can be amplified from fixed pathologic specimens (7), buccal cells from mouth washes (8), a single human hair (9), and solitary cells (10). Once amplified, large DNA samples can be studied by electrophoresis, Southern and slot blot, and other techniques.
