    Description / Table of Contents: Summary In a 36-year old subject with Zollinger-Ellison syndrome analyses of gastric HCl-secretion were performed preoperatively. These studies were carried out to evaluate the variability of basal hyperchlorhydria and the influence of exogenic factors upon endogenically stimulated secretion. In 7 analyses the basal HCl-secretion revealed only slight variations so that a nearly constant incretion of gastrin can be assumed. — The basal secretion increased of more than 25%, after parenteral application of betazoldihydrochloride, pentagastrin and regular insulin as well as after induced acute hypercalcemia. Acute hypermagnesemia as well as parenteral application of glucagone caused a decrease in basal HCl-secretion. “Medical Vagotomy” was followed by a marked decrease in basal HCl-secretion of more than 80%. These results demonstrate that the endogenically stimulated HCl-secretion e.g. by gastrin or gastrinlike substances in Zollinger-Ellison syndrome can be influenced principally in the same way by exogenic factors as the basal HCl-secretion in healthy subjects or subjects with duodenal ulcer..
    Notes: Zusammenfassung Bei einem 36jährigen Patienten mit gesichertem Zollinger-Ellison-Syndrom wurden präoperativ Analysen der gastralen HCl-Sekretion durchgeführt. Die Untersuchungen richteten sich auf die Variabilität der basalen Hyperchlorhydrie und auf die Beeinflußbarkeit der endogen stimulierten Sekretion durch exogene Faktoren. Die basale HCl-Sekretion zeigte bei insgesamt 7 Analysen bemerkenswert geringe Schwankungen, so daß eine weitgehend konstante Gastrininkretion angenommen wurde. Bei parenteraler Verabreichung von Betazoldihydrochlorid, Pentagastrin und Alt-Insulin sowie unter dem Einfluß induzierter akuter Hypercalcämie stieg die basale Sekretion um mehr als 25%. Akute Hypermagnesiämie bewirkte — ebenso wie parenterale Verabreichung von Glukagon — eine Hemmung der basalen Säuresekretion. „Medikamentöse Vagotomie“ hatte einen Abfall der basalen Sekretion um mehr als 80% zur Folge. Die Ergebnisse zeigen, daß die endogen — durch Gastrin oder gastrinanaloge Substanzen — stimulierte Sekretion bei Zollinger-Ellison-Syndrom durch exogene Faktoren grundsätzlich in gleichem Sinne zu beeinflussen ist wie die basale HCl-Sekretion bei gesunden Probanden oder Patienten mit Ulcus duodeni.

    Description / Table of Contents: Summary The production of acid by the parietal cells of the gastric mucosa can, by means of pH color indicators, serve to identify the proximal border of the antrum mucosa. Kongo red (color change at pH of 3), which stains the acid-producing gastric mucosa black while leaving the antrum region red, is particularly suited for this purpose; the border between the antrum and body portions of the stomach can be identified endoscopically and marked off by means of India ink injection. A preoperative demarcation of the antrum, especially for antrectomy, is thus possible.
    Notes: Zusammenfassung Die Funktion der Belegzellen in der Korpusschleimhaut, Säure zu sezernieren, kann zur Markierung der proximalen Antrumschleimhautgrenze mit Hilfe von pH-Farbstoffindicatoren genützt werden. Als Indicator eignet sich besonders Kongorot (Farbumschlag bei pH 3), das die Säure bildende Korpusschleimhaut schwärzlich färbt, während die Antrumschleimhaut rot bleibt. Endoskopisch läßt sich die Grenze zwischen Antrum und Korpus erkennen und durch submuköse Tuscheinjektionen markieren. Damit ist die Voraussetzung für eine exakte präoperative Abgrenzung des Antrum, also für eine Antrektomie, gegeben.

    Notes: Abstract A recombinant immunoblot was developed for detection of IgM and IgG antibodies in patients with Lyme borreliosis. The recombinant antigens were the chromosomal-encodedBorrelia burgdorferi proteins p100, the flagellin and an internal flagellin fragment thereof as well as the plasmid-encoded outer surface proteins A (OspA) and C (OspC). A panel of 144 sera from patients with Lyme borreliosis (erythema migrans,n = 31; neuroborreliosis state II,n = 60; Lyme arthritis,n = 24 and acrodermatitis chronica atrophicans,n =19) have been investigated and the results have been compared to the immunofluorescence absorption test (IFA-ABS) and to two different enzyme-linked immunosorbent assays [the flagellin ELISA and a newly developed ELISA (OGP-ELISA)]. The two ELISAs were comparable in sensitivity, whereas the IFA-ABS was less sensitive for IgM antibody but equally sensitive for IgG antibody detection. Immunoblot analysis revealed that IgG antibodies are mainly reactive with p 100 and the internal flagellin fragment (sensitivity 51% and 32%, respectively) and rarely with OspC (14%). All patients with late Lyme borreliosis had IgG antibodies against the p100. IgM antibodies were predominantly directed against OspC (43%) and in a lower extent against the internal flagellin fragment and p100 (15% and 13%, respectively). The complete flagellin was not useful due to a high number of unspecific reactions with control sera and the OspA was only exceptionally reactive in Lyme borreliosis patients. The sensitivity of IgM antibody detection could be increased in cases with early Lyme borreliosis from 46% to 65% when the OspC blot was performed in addition to the flagellin ELISA, or from 56% to 65% when performed in addition to the OGP-ELISA. The recombinant blot is, therefore, a valuable diagnostic test to increase sensitivity of early antibody detection and is regarded as a valuable confirmatory test also in late disease.

    Notes: Summary The disposition of Na2B12H11SH (BSH) in patients with malignant glioma has been investigated, in preparation for a Phase I clinical trial of boron neutron capture therapy. BSH was found to possess a linear disposition over the dosage interval investigated (up to 75 mg/kg). A bi-phasic blood pharmacokinetics was observed. Tumour-to-blood ratios showed variations between patients between 0.08 and 5.1. The data allow the definition of amount of BSH and timing of infusion for a Phase I clinical trial protocol.
