    Notes: Summary We analyzed the results of clinical studies on the therapeutic efficacy of hormone monotherapy with tamoxifen, medroxyprogesterone acetate, and aminoglutethimide in metastatic breast cancer, which were published between 1971 and 1986 and involved altogether 7000 patients. The overall response rates in patients treated with these hormonal single agents at various dose levels ranged from 31%–42%. When only estrogen receptor-positive patients were considered, the response rates lay between 41% and 54% in groups which were treated with the antiestrogenic agents tamoxifen or aminoglutethimide. The duration of remission was 12 months for tamoxifen- and aminoglutethimide-treated women, whereas medroxy-progesterone acetate effected remissions lasting from 6–16 months. The overall mean survival from start of therapy in tamoxifen- and aminoglutethimide-treated groups was 20 months, whereas information concerning this therapeutic parameter was available only in a minority of medroxyprogesterone acetate-treated groups. With respect to the response by site of metastatic lesions, all three agents caused a significantly higher degree of remissions in the soft tissue as compared to visceral disease.

    Notes: Objective To define the normal ranges of umbilical cord blood oxygen saturation (SaO2) and acid-base status at birth and to evaluate the effect of gestational age on cord blood values in vigorous newborn infants following spontaneous vaginal birth from a vertex position.Design Prospective study.Setting Department of Obstetrics and Gynaecology, University of Graz, Austria.Sample Cord blood samples from 1281 vigorous newborn infants.Methods Cord blood sampling was performed following on newborn infants following spontaneous vaginal birth in a vertex position. SaO2 was measured directly by a spectrophotometer and pH, base excess, pCO2 and pO2 by a pH/blood-gas analyser. Infants with a 5-minute Apgar score ≥ 7 were considered vigorous. Subgroups were classified according to the gestational age: preterm, term and postterm (〈 37, 37–42 and 〉 42 weeks, respectively).Results The median umbilical artery SaO2 was 24.3% and the 2.5th centile was as low as 2.7%. The median umbilical artery values were pH = 7.25, base excess =−4.3 mmol/L and pO2= 16 mmHg. The 2.5th centiles were 7.08, −11.1 mmol/L and 5 mmHg, respectively. The median umbilical artery pCO2 was 50 mmHg and the 97.5th centile was 75 mmHg. The mean umbilical artery and vein SaO2 values were not significantly influenced by gestational age. The umbilical artery SaO2 and base excess values were strongly skewed. The mean umbilical artery pH values in preterm infants were higher than in other subgroups. The mean umbilical artery and vein base excess values were lower in post-term newborn infants than in other subgroups.Conclusions The physiological range of oxygen saturation in umbilical cord of vigorous newborn infants at birth is wide and skewed. In contrast to pH and base excess, umbilical cord blood oxygen saturation is not influenced significantly by gestational age at birth.

    Notes: To evaluate the efficacy of computed tomography (CT) in 175 patients scheduled to undergo surgery for a suspected ovarian mass, we compared preoperative CT findings with the findings at surgery and histology. At surgery, 115 patients were found to have epithelial ovarian malignancies (20 stage 1, 7 stage II, 50 stage III and 38 stage IV) while 60 had benign pelvic tumors. CT findings were suggestive of malignancy in 4 (7%) of 60 patients with benign tumors and suggestive of benign disease in 3 (3%) of 115 patients with malignant ovarian tumors; 30% of all scans were inconclusive. The sensitivity of CT for predicting pelvic or para-aortic lymph node involvement in 42 patients who underwent lymphadenectomy was 62 and 70%, respectively; the specificity was 100%. The results of this series suggest that the routine use of CT in the preoperative evaluation of patients does not seem justified.

    Notes: Whether the size of a retroperitoneal lymph node reflects its status is not clear. We measured the size of 125 positive and 160 negative pelvic lymph nodes in 32 consecutive patients with node-positive endometrial cancer. The measurements were compared with those of 143 pelvic lymph nodes of five randomly selected patients with endometrial cancer without node involvement. Overall, positive lymph nodes were larger than negative lymph nodes in both node-positive patients and node- negative controls (P 〈 0.01). There was a positive correlation between the size of positive lymph nodes and the size of the metastasis therein (P 〈 0.01). However, 68 of 125 (54%) positive lymph nodes measured less than 10 mm in maximum diameter, while 46 of 160 (29%) negative lymph nodes in node-positive patients measured more than 10 mm in maximum diameter. The metastasis was detected in more than 50% of step-serial sections in only 74% of positive lymph nodes. These data suggest that the size of a lymph node does not reliably reflect its status. Thus, these nodes may be missed if only enlarged nodes are removed. If only one section of a lymph node is performed, at least 26% of metastases will be missed.

    Notes: Abstract. Thirty patients with tumors of the ovarian stroma (28 granulosa cell tumors, two androblastomas) and 12 women with germ-cell tumors (nine dysgerminomas, two teratomas, one endodermal sinus tumor) were treated at the Department of Obstetrics and Gynecology of the University in Graz between 1972 and 1989. Of the 28 patients with granulosa-cell tumors, 24 (86%) had stage I disease, 27 were primarily treated by surgery, and seven (25%) developed a recurrence (three stage I, three stage III and one stage IV). There were three recurrences in the peritoneal cavity and two in the paraaortic nodes; six of the seven patients died of recurrent disease.Four of nine patients with stage I dysgerminomas developed recurrences, two of which were located in the para-aortic nodes. Three patients with recurrence after stage Ia disease are currently alive without evidence of disease; one patient with stage Ib disease died 11 months after a paraaortic recurrence.Wider use of cytostatic combinations and careful surgical staging, including lymphadenectomy, may improve the survival of patients with stromal or germ-cell tumors.

    Notes: Summary This methodological study describes three surgical prodedures for locoregional and systemic drug administration, which are based on a similar experimental design. Cannulation of the arterial and portal access to the liver in comparison to the general venous system, and arterial access to the large intestine through a permanently implantable system, suitable for serial bolus injections and infusions in unrestrained rats, is presented (experiment I). Furthermore, an infusion system for longterm administration (experiment II) and a method for blood sampling during locoregional or systemic infusion procedures (experiment III) have been developed. The positioning and free flow of the catheters were checked by means of scintigraphy, administration of fluorescein under UV light and angiography in animals of experimental series I. After 7 days, no obstruction was detected. On day 15 and 30 following implantation 73.3% and 58.3% of the animals, respectively, showed unimpeded flow through the catheter system. The methods described here were well tolerated by the animals without alteration of their general condition and are currently in use in a series of chemotherapeutic and pharmacokinetic investigations.

    Notes: Summary The results of clinical studies dealing with first and second line chemotherapy of metastatic breast cancer published between 1975 and early 1986 which involved 9350 women were reviewed. Our special aim was to evaluate combination chemotherapy and its influence on overall survival in late stage breast cancer patients. No significant improvement in overall survival times was found in this selected group of patients who were treated with intense palliative chemotherapy.
