    Notes: Abstract. The etiology of avascular necrosis (AVN) is multifactorial. Independent of its etiology and localization it shows typical pathologies and radiological images. In the early stages localized subchondral edema is characteristic. In 50 % of all cases accompanying joint effusion may be found. Due to necrosis of the cells of bone marrow and bone fibrovascular, reactions with hyperemia can be delineated. These reactions allow us to visualize necrosis indirectly. The best imaging methods are MRI and, to a lesser extent, bone scintigraphy. In later stages calcification as well as new bone formation and microfractures are typically demonstrated and visualized best with plain X-rays and CT. Why reparations in many cases, particularly in the hip, are incomplete and may stop in any stage is unknown. Over years clinically complete silent AVNs are not an uncommon finding. Prognosis depends on the localization and size of the AVN. The number of repair mechanisms is best outlined with contrast-enhanced MRI and return of fatty marrow.

    Notes: Summary Quantitative histomorphometric and microradiographic analysis of iliac crest bone biopsy specimens of 10 unselected and untreated postmenopausal women with osteoporosis was performed and the results were correlated with the values measured by osteodensitometry on the right forearm. No positive correlation between histomorphometry or microradiography and osteodensitometry was observed. Moreover, the data of various histomorphometric parameters and those of the bone mineral content measured by microradiography revealed a significant negative correlation when compared to the values obtained by osteodensitometry. Our results suggest, that evaluation of one skeletal site is not necessarily representative of the entire skeleton in patients with postmenopausal osteoporosis.

    Description / Table of Contents: Zusammenfassung Es wird eine seltene komplexe Mißbildung eingehend beschrieben mit asymmetrischen Veränderungen im frontonasalen Bereich, einer beidseitig durchgehenden Lippen-Kiefer-Gaumen-Spalte Fehlbildungen des rechten Auges, des Unterkiefers, der Zunge, der äußeren Ohren, der hinteren Schädelgrube und des Kleinhirns sowie einer Dextroposition des Herzens und einer überzähligen Rippe beiderseits. Die Problematik der kausalen und formalen Genese derartiger Mißbildungen wird besprochen und gezeigt, daß eine Zuordnung zu einem der in Frage kommenden Syndrome eigentlich nicht möglich ist.
    Notes: Abstract A rare complex malformation is described consisting of asymmetrical changes in the fronto-nasal region, bilateral complete cleft lip, alveolar ridge, and palate, malformation of the right eye, the mandible, the tongue, the external ears, the posterior cerebral fossa and the cerebellum, as well as cardiac dextroposition and a supernumary rib on both sides. Problems of aetiology and structural genesis are discussed. It is shown that classification into one of the existing syndromes is not really possible.

    Notes: Normal Wistar rats, aged four and six month, were fed an atherogenic diet and the resulting periodontal changes were determined after six and eight weeks using histological, histochemical and histometrical methods. The changes of the alveolar bone remodelling adjacent to incisor teeth were compared with those around molar teeth. A diminished formation of new bone and an increased osteoclastic resorption were found in the alveolar bone of the rats fed the atherogenic diet. In addition gingival inflammation and the development of periodontal pockets were observed. Using histometrical evaluation it could be demonstrated that these alveolar bone changes were significantly more pronounced in the incisor teeth than in the molars. It is supposed that the latter finding is due to the more intense remodelling of the alveolar bone in the constantly growing teeth of rodents.

    Description / Table of Contents: Zusammenfassung Die Konstruktionsmerkmale von Endoprothesen aus Al2O3 werden vorgestellt. Die Befestigung am langen Röhrenknochen erfolgt mittels einer Kegelhülsenverbindung. Diese Prothesen-Knochenverbindung wurde mechanischen Festigkeitsprüfungen unterworfen and im Tierexperiment überprüft. Die Ergebnisse ermutigten zur klinischen Anwendung am Tumorpatienten, die an Hand verschiedener Fälle demonstriert wird. Auf die Möglichkeit, auch große Defekte zu überbrücken, wird hingewiesen.
    Notes: Summary Design characteristics of endoprostheses made of alumina are discussed. Fixation of implants on long bones is achieved by means of a conical sleeve. The bonding of bones to implants has been proved tested mechanically and in animal experiments. Good results have led to the use of bioceramic endoprostheses in tumor patients. Several implantations are shown. This technique makes it possible to bridge large defects.

    Description / Table of Contents: Zusammenfassung Der Zustand der dorsalen Haut und des darunterliegenden Muskelgewebes der Ratte wurde unmittelbar nach CO2-Laser-Schnitten mit Hilfe von licht- und elektronenmikroskopischen Methoden untersucht. In der subkutanen Skelettmuskelschicht konnte eine erhöhte Empfindlichkeit des Gewebes festgestellt werden, die in einer ausgeprägten Abfolge von Zellveränderungen ihren Ausdruck findet. Diese Schädigungen werden hauptsächlich den thermischen Effekten der Laserstrahlung zugeschrieben. Die Bedeutung dieser Untersuchungsergebnisse in Hinblick auf die Verwendung von Laser in der Chirurgie wird besprochen.
    Notes: Summary The immediate effect of CO2-laser incision to the dorsal skin and underlying muscular tissue of rats was studied by light and electron microscopical methods. In the subcutaneous layer of skeletal muscle cells an increased susceptibility was found, resulting in distinct zones of cellular changes which are attributed mainly to thermal effects of the laser beam. The importance of these findings for the surgical application of lasers is discussed.

    Notes: Abstract Bone marrow oedema syndrome (BMOS) of the hip includes severe hip joint pain, focal loss of radiodensity in radiographs, increased uptake in bone scintigrams and the pattern of bone marrow oedema in the femoral heads in MRI scans. In 15 patients (16 hip joints) we found the clinical and radiological signs of BMOS. On T1-weighted MRI images areas of low signal intensity could be observed in the head, neck and the intertrochanteric region of the femur in various extensions. These areas showed a significant increase in signal intensity on the T2-weighted images. Because pain was resistant to conservative therapy all these patients were treated by core decompression of the femoral head in a prospective study. Bone cores were evaluated histologically using undecalcified sections and quantitative microradiography. The existence of intramedullary oedema in exactly the regions exhibiting the MRI pattern of bone marrow oedema was verified histologically; however, bone and marrow changes similar to those of early avascular necrosis (AVN) were also visible. These findings support our assumption that BMOS might be a transitory initial phase of AVN. Therefore core decompression treatment for patients suffering pain from BMOS is recommended after excluding other diseases.
