    Description / Table of Contents: Résumé Pour parler d'une amélioration de la qualité de vie aprés chirurgie anti-reflux, le patient doit noter une amélioration ou même une disparition des symptômes qui l'ont conduit à se faire opérer. Il ne doit pas par ailleurs se plaindre de symptômes nouveaux résultant de la chirurgie anti-reflux. Les résultats publiés de la chirurgie anti-reflux sont extrêmement variables. Une part de cette variation est peut-être due à l'utilisation de méthodes non standardisées pour évaluer les symptômes. Une méthode d'évaluation des symptômes est proposée, qui tient compte de leur fréquence et de leur intensité. On espére que cette méthode permettra aux chirurgiens d'affiner et d'améliorer leurs techniques et leurs résultats.
    Abstract: Resumen Una operación antirreflujo está diseñada para corregir un estado fisiológico definido, o sèa un reflujo suficiente para producir síntomas y/o daño al esofago, laringe o pulmones. La mejoría de la calidad de la vida después de cirugía significa que deben desaparecer los síntomas del reflujo y que el paciente no debe adquirir nuevos síntomas como efecto secundario de la operación. Los resultados informados en la literatura varían en forma muy amplia. Parte de tal variación puede deberse a los métodos no estandarizados de valorar la sintomatología. Se propone un sistema de valoración de síntomas que considera tanto la intensidad como la frecuencia de los mismos. Se espera que este sistema permita a los cirujanos refinar y superar sus técnicas y sus resultados.
    Notes: Abstract To improve the quality of life after antireflux surgery, the patient should notice improvement or disappearance of the symptoms leading to the surgery and should not acquire any new symptoms resulting from antireflux surgery. Published accounts of antireflux surgery results vary widely. Part of this variation may be due to nonstandard methods of evaluating symptoms. A system of symptom evaluation is proposed which takes into account the intensity and frequency of symptoms. It is hoped that this system will allow surgeons to refine and improve their techniques and results.

    Notes: Abstract A system of magnetic field goniometry wasdeveloped for measuring the frequency of stomachcontractions. This technique uses a handheld, electroniccompass to measure the angular change in direction of a magnetic field generated by a small, ingestedmagnet. Measurements of gastric mechanical activity madeby goniometry were validated with simultaneousmeasurements using manometry and electrogastrography. The agreement between these differentmodalities was excellent. In this pilot study, magneticfield goniometry provided an easy, minimally invasive,and accurate method to measure the frequency of gastric contractions.

    Notes: Abstract Fifty-eight patients with angina-like chest pain had esophageal manometric testing. Forty-three had no evidence of coronary artery disease at the time of referral or at subsequent contact; 15 patients were proven to have coronary artery disease. High-amplitude contraction waves were the most frequently found manometric abnormality (15 patients). Less frequent were increased duration of contractions, achalasia, and diffuse esophageal spasm; the latter was present in only 3 patients. An approach to the interpretation of information obtained during manometry is presented. Using this approach, the esophagus was strongly implicated as the cause of the pain in 20 patients and was suspect in 18 others. Seven patients had results that exonerated the esophagus, and in the 13 remaining individuals, the esophagus was probably not the offending organ.

    Notes: Abstract Gastroesophageal reflux and clearance of the refluxed material can be measured by plotting a time-activity curve from an esophageal area of interest after 1 mCi of99mTc sulfur colloid is placed in the stomach. Control subjects do not have peaks exceeding a value twice that of the baseline count levels. Reflux patients exceed this value, either spontaneously or after Valsalva maneuvers. This technique has a sensitivity which is greater than that of barium and equal to the sensitivity of a pH probe in patients with both moderate and severe reflux. Scintigraphic reflux was shown in 62% of moderate refluxes and 85% of those with severe reflux as defined clinically. Clearing of the refluxed material occurs rapidly in most patients if measured by this scintigraphic technique. This test can be performed rapidly with minimal radiation exposure and is noninvasive.
