    Description / Table of Contents: Zusammenfassung Die vorliegende Studie befaßt sich mit der Wertbestimmung der TNM-Klassifikation des Mundhöhlencarcinoms nach den Definitionen der UICC. Sie basiert auf dem kollektiven Vergleich, wonach das Krankengut sowohl nach den UICC-Vorschlägen wie nach eigenen Vorschlägen (Spiessl/Fries) klassifiziert worden ist. Als Parameter für die Beurteilung, welche Definitionen in der Praxis am brauchbarsten sind, diente die Absterbekurve, errechnet nach der Minimal Survival Rate. Das gereinigte Krankengut (975 Fälle) wurde nach den beiden T-Kategorien (UICC und Spiessl/Fries) aufgegliedert. Hier zeigte sich, daß die Klassifizierung der Tumoren nach der Spiessl/Fries-Variante den praktischen Beobachtungen eher entspricht als die Klassifizierung nach dem UICC-System. Bei der Auswertung der T-Definitionen stellte sich heraus, daß die Angabe der Infiltrationsgrade für die Prognosebestimmung von geringem Wert ist. Allein die Größe des Tumors in cm ist ausschlaggebend für die Einzelabstufung der T-Kategorien. Unter Berücksichtigung dieser Beobachtungen werden künftig nur noch 3 T-Kategorien statt 4 empfohlen. Hingegen wird vorgeschlagen, die N-Definitionen beizubehalten.
    Notes: Summary The present study deals with the value determination of the TNM-classification of the carcinoma of the buccal cavity according to the UICC definitions. It is based on a joint comparison whereby the total number of cases were classified according to both the UICC-propositions and those by Spiessl /Fries. The lethal curve calculated according to the minimum survival rate was used as a parameter to assess which definitions would prove to be most useful in practice. The screened number of cases (975 all told) was divided according to both T-categories (UICC and Spiessl/Fries). The result showed that tumour classifications in accordance with the Spiessl/Fries variant corresponded much more to practical observations than the classifications according to the UICC-system. On evaluation of the T-definitions it was found that information about infiltration degrees was of minor importance for prognostic purposes. It is the size of the tumour given in cms which is determining the individual graduation of T-categories. Considering these results only three T-categories instead of four will be recommended in future. It is suggested, however, that the N-definitions should be adhered to.

    Description / Table of Contents: Summary In a complex animal model in sheep, polydioxanone (PDS®) and polylactic membranes were used for the reconstruction of large orbital-wall defects. In a long-term experiment over 1 year, polylactic implants alone showed the best performance as compared with combinations involving autogenous bone grafts and titanium miniplate fixation. As soon as these polylactic implants are approved for human surgery, they will be used to solve the still challenging problem of anatomical reconstruction of large comminuted fractures of more than one orbital wall.
    Notes: Zusammenfassung In einem experimentellen Modell wurden biodegradierbare Membranen aus Polydioxanon (PDS ® ) resp. Polylaktat zur Rekonstruktion großer Orbitawanddefekte bei Schafen verwendet. Im Langzeitversuch über ein Jahr zeigten diejenigen Rekonstruktionen die besten Resultate, bei welchen ausschließlich biodegradierbare Implantate aus Polylaktat zur Anwendung kamen. Bei gleichzeitiger Verwendung von autologen Knochentransplantaten und Titan-Miniplatten-Osteosynthesen war die anatomische Form der rekonstruierten Orbita weniger zuverlässig gewährleistet. Es darf erwartet werden, dass die klinische Zulassung der Polylaktat-Implantate die chirurgische Wiederherstellung ausgedehnter Trümmerfrakturen mehrerer Orbitawände wesentlich erleichtern wird und zu vorhersehbaren, anatomischen Rekonstruktionen führt.

    Description / Table of Contents: The reconstruction of extensive jaw defects is frequently only possible with microvascular bone flaps. Here we are presenting an operative technique using prefabricated fibular flaps and osseointegrated implants. In a first operation, the fibula is prepared with implants, split skin graft, and a nonresorbable membrane. The jaw defect is reconstructed 6 weeks later and can be treated directly with a prosthesis thanks to osseointegrated implants. The technique is described with reference to 5 patients already operated according to this technique and the initial findings are evaluated.
    Notes: Die Rekonstruktion von ausgedehnten Kieferdefekten ist häufig nur mit mikrovaskulären Knochentransplantaten möglich. Es wird hier eine Operationstechnik vorgestellt, bei der die Fibula präformiert wird. In der 1. Operation werden ITI-Implantate mit SLA-Oberfläche in die Fibula eingesetzt. Die Implantate und die Fibula werden mit Spalthaut und einer 1 mm dicken, nicht resorbierbaren Membran überdeckt. 6 Wochen nach der 1. Operation erfolgt in der 2. Operation die eigentliche Rekonstruktion des Kieferdefektes. Innerhalb dieser 6 Wochen sind die Implantate osseointegriert. Die Spalthaut, die wie bei einer Vestibulumplastik mit dem Periost verwachsen ist, bildet das stabile periimplantäre Weichgewebe. Die Implantate werden intraoperativ mit der vorbereiteten Suprastruktur verschraubt und gewähren durch die präoperativ bestimmte Okklusion die korrekte Positionierung des Transplantates. Postoperativ sind zudem die sofortige Funktion und Belastung möglich. Anhand von 5 Patienten werden diese Technik beschrieben und deren Resultate ausgewertet.

    Description / Table of Contents: Summary Within this chapter, tumors which produce bone matrix (osteoid) or other bone-forming tissues, like cartilage, connective tissue, and cementum or originate within the bone marrow are described. In addition, those lesions are mentioned which, according to the WHO, are typical for the jawbones and are therefore included within the 1992 WHO classification of odontogenic tumors. Vascular and epithelial tumors which may also be observed in the jaws are not described. All the lesions and tumors reported have in common a more or less similar clinical and radiological appearance. A precise diagnosis, therefore, can only be established by a sufficient biopsy which should be seen by a pathologist with experience in this field. In addition to clinical, radiological, and histological appearances, therapeutic necessities are described. The data from recent publications are taken into account. This presentation is based on the data collected within the reference registry of the German-Austrian-Swiss Study Group of Tumors of the Face and Jaws (DÖSAK) in Basel.
    Notes: Zusammenfassung Im vorliegenden Kapitel werden die Tumoren dargestellt, die in ihrer Differenzierung Knochenmatrix (Osteoid) und andere knochentypische Gewebe wie Knorpel, Bindegewebe und Zement bilden oder im Knochenmark vorkommen. Zusätzlich sind diejenigen Läsionen aufgeführt, welche von der WHO als für die Kieferknochen typisch bezeichnet und deshalb in die 1992 erschienene Klassifikation odontogener Tumoren aufgenommen worden sind. Vaskuläre und im Knochen vorkommende epitheliale Tumoren werden nicht behandelt. Den hier erwähnten Läsionen und Tumoren ist eine sowohl klinische als auch radiologische Symptomarmut gemeinsam. Eine zuverlässige Diagnose ist daher ohne ausreichende Biopsie, die von einem spezialisierten Pathologen untersucht werden sollte, nicht möglich. Neben der Klinik sind das radiologische und histologische Erscheinungsbild unter Berücksichtigung neuerer Literatur beschrieben, zudem wird das therapeutische Vorgehen erwähnt. Die vorliegende Darstellung der wichtigsten Knochentumoren und tumorähnlichen Läsionen im Kiefer basiert auf ¶den im Basler Referenzregister des Deutsch-Österreichisch-Schweizerischen Arbeitskreises für Tumoren im Kiefer-Gesichts-Bereich (DÖSAK) gesammelten Unterlagen.

    Description / Table of Contents: Zusammenfassung Innerhalb einer koordinierten Versorgung Polytraumatisierter bildet die funktionsstabile Osteosynthese der Gesichtsschädelfrakturen einen wesentlichen Bestandteil. Durch Verzicht auf intermaxilläre Ruhigstellung werden Beatmungs- und Ernährungsmöglichkeiten entscheidend verbessert und Tracheotomien unnötig. Oftmals irreparable Spätfolgen, wie Occlusionsstörungen, Dish face, Asymmetrien, Telecanthus und Hypertelorismus werden durch die Frühversorgung verhindert. Das im Vortrag beschriebene Basler Konzept lässt sich nur durch enge Zusammenarbeit aller Spezialdisziplinen verwirklichen.
    Notes: Summary A very important step within the coordinated multidisciplinary surgical treatment of polytraumatized patients is early, functionally stable osteosynthesis of the facial fractures. If this is performed, there is no need for intermaxillary fixation, which means no need for tracheostomies. Also, the patients are easier to feed. There are fewer surgical interventions and patients can be mobilized earlier. Besides these advantages during the early phase of treatment, late sequelae like disturbed occlusion, dish face, facial asymmetries, telecanthus, and hypertelorism can also be avoided.
