    Description / Table of Contents: Zusammenfassung Bei 100 Patienten wurden entlang der kleinen Kurvatur mit einem Gastroskop mit Biopsievorrichtung 8 Partikel in annähernd gleichem Abstand entnommen (Stufenbiopsie). Eine histologische Klassifizierung der Schleimhautveränderungen erfolgte in geringgradige, mäßig starke und ausgeprägte Oberflächengastritis sowie in eine chronisch atrophische Gastritis mit und ohne intestinale Metaplasie. Bei 44 Patienten wurde darüber hinaus Schleimhaut aus dem Bereich der großen Kurvatur entnommen. — Nur bei 16 Patienten fand sich eine unauffällige Schleimhaut, bei 20 Patienten eine diffuse Gastritis mit gleichartigem Entzündungsprozeß in allen Biopsiepartikeln. 26 Patienten wiesen eine isolierte Antrumgastritis auf, bei 13 Untersuchten war der Entzündungsgrad der Magenschleimhaut im Antrumbereich am stärksten ausgeprägt und nahm zur Kardia hin ab, so daß man von einer pyloro-kardialen Expansion sprechen konnte. 15 Patienten boten umgekehrte Verhältnisse, d. h. der Entzündungsprozeß war um so ausgeprägter, je kardianaher der Partikel entnommen worden war (kardia-pylorische Expansion). Eine letzte Gruppe von 10 Patienten zeigte keine eindeutige Ausbreitungstendenz der gastritischen Veränderungen; bei 8 waren Antrum und Kardia im Vergleich zur Angulusregion deutlicher entzündlich verändert, bei 2 war die Gastritis am Angulus am intensivsten. — Bei einer Korrelation der Fundusbiopsien aus großer und kleiner Kurvatur wies letztere mit einer Ausnahme immer stärkere entzündliche Veränderungen auf. Die Methode der blinden Saugbiopsie des Magens, mit der man glaubte, diffuse Schleimhautveränderungen repräsentativ zu erfassen, muß auf Grund der lokalisationsbedingten diskrepanten Befunde in Frage gestellt werden.
    Notes: Summary Eight samples are obtained with the Japanese fiberscope GFB at different levels of the minor curvature in 100 patients. Histological classification distinguishes between slight, moderate and severe superficial gastritis and chronic atrophic gastritis with and without intestinal metaplasia. In 44 patients biopsies were also performed on the greater curvature. — Only 16 patients showed normal mucosa, 20 had diffuse gastritis with identical findings in all specimens. 26 patients had isolated antral gastritis, in 13 others the inflammatory process was most severe in the prepyloric antrum, with decreasing intensity towards the cardia (pyloric-cardial expansion). In 15 subjects the reverse was true: the antrum showed only minor signs of gastritis compared to fundus and cardia (cardia-pyloric expansion). Classification was difficult in a group of 10 patients: 8 showed gastritis of the antrum and cardia whereas the gastroscopic angle was almost free of cellular infiltration, in two only was the angle most severely affected. — The correlation of fundus mucosa biopsies from the minor and greater curvature indicates that the gastritis-type changes were more pronounced in the specimens from the minor curvature, except for one case. Blind suction biopsy supposed to give a representative idea of gastric changes has to be challenged. The hypothesis that gastritis is a diffusely spreading process seems unjustified.

    Description / Table of Contents: Zusammenfassung Bei 13 unausgewählten Klinikpatienten wurde der Einfluß von Theophyllin auf die durch Pentagastrin submaximal stimulierte gastrale HCl-Sekretion geprüft. Die statistische Analyse der Untersuchungsergebnisse ergab, daß Theophyllin die durch Pentagastrin stimulierte Sekretion der Elektrolyte H+, Cl− und K+ signifikant steigert. Dieser Effekt kann als Indiz dafür angesehen werden, daß die Wirkung von Pentagastrin durch zAMP vermittelt wird. Bevor dieser Wirkungsmechanismus als gesichert angesehen werden kann, sind weitere Untersuchungen erforderlich.
    Notes: Summary In 13 randomly chosen patients of our clinic, the effects of theophylline on gastric acid secretion was studied after submaximal pentagastrin stimulation. Statistical analysis of the results showed that theophylline significantly increases the secretion of the electrolytes H+, Cl− and K+. This effect on electrolyte secretion already stimulated by pentagastrin may be interpreted as evidence that the effect of pentagastrin is mediated by cyclic AMP. Further investigations aren/necessary to support this hypothesis.

    Description / Table of Contents: Zusammenfassung Bei 13 unausgewählten Probanden wurde der Effekt induzierter akuter Hypercalcämie auf die durch Theophyllin submaximal stimulierte gastrale HCl-Sekretion geprüft. Die statistische Analyse der Untersuchungsergebnisse zeigte, daß die durch Theophyllin submaximal stimulierte HCl-Sekretion durch akute Hypercalcämie gesteigert wird. Dieser potenzierende Effekt akuter Hypercalcämie könnte zumindest partiell durch Freisetzung von Gastrin in der Antrumschleimhaut vermittelt werden.
    Notes: Summary In 13 unselected in-patients, the effect of induced acute hypercalcaemia on gastric acid secretion stimulated submaximally by theophylline was examined. Statistical analysis of the results showed that acid secretion stimulated submaximally by theophylline is increased by acute hypercalcaemia. This additive effect of acute hypercalcaemia could be mediated at least partially by liberation of gastrin from the antral mucosa.

    Description / Table of Contents: Zusammenfassung Zur Bewertung des Ultraschallverfahrens in der Pankreasdiagnostik wurden Ultraschalluntersuchung und endoskopische retrograde Pankreasgangdarstellung bei 140 Patienten vergleichend durchgeführt. Während in 23 Fällen — meist Patienten mit nach Billroth II reseziertem Magen — die retrograde Pankreasgangdarstellung mißlang, war bei 20 Patienten infolge Luftüberlagerung oder ausgeprägter Adipositas mit Ultraschall keine Pankreasdarstellung möglich. In 15 Fällen ergab die Ultraschalluntersuchung einen Grenzbefund. Von 105 Ultraschallbefunden waren 3 falsch negativ und 5 falsch positiv, während die retrograde Pankreasdarstellung 9 falsch negative und 2 falsch positive Befunde ergab. In 50 von 67 Fällen einer sonographisch festgestellten Pankreaserkrankung war eine weitere Differenzierung zwischen malignem Tumor und benignem Pseudotumor (Pankreatitis oder Cyste) möglich. In der Diagnostik von Pankrescysten zeigte sich die Ultraschallmethode mit 23 (von 27) richtig erkannten Cysten der retrograden Pankreasgangdarstellung (12 von 27) überlegen. Im Vergleich zur endoskopischen Methode ist die Ungefährlichkeit und der geringe Aufwand der Ultraschalldiagnostik hervorzuheben. Beide Methoden ergänzen sich zu einer schr hohen Treffsicherheit (95%) in der Pankreasdiagnostik.
    Notes: Summary To evaluate the diagnostic efficacy of ultrasonography in pancreatic disorders the results were compared with endoscopic retrograde pancreaticography in 140 patients. While in 23 patients-in a vast majority due to a previous Billroth II resection-retrograde cannulation of Vaters' papilla was unsuccessful, examination of the pancreas was not possible in 20 patients with ultrasonography because of meteorism or severe adipositas. In 15 subjects the sonographic findings were equivocal. Out of 105 ultrasonic findings three were false-negative and 5 false-positive, whereas retrograde Wirsungography resulted in 9 false-negative and 2 false-positive interpretations. In 50 out of 67 cases of pancreatic disorders diagnosed by ultrasonography a further differentiation between malignant tumours and benigne pseudotumours (pancreatitis or cyst) was possible. With respect to the diagnosis of pancreatic cysts ultrasonography was superior by 23 of 27 to retrograde pancreaticography (12 out of 27) since many cysts do not communicate with the ductal system. In comparison with the endoscopic method the risklessness and the low expenses have to be stressed. Both methods supplement each other, so that a high diagnostic accuracy is reached (more than 95 per cent).

    Notes: Background : Pantoprazole is claimed to have a lower potential for drug interaction than other proton pump inhibitors.Aim : To estimate the frequency of adverse events and drug interactions reported to the Food and Drug Administration in patients receiving omeprazole, lansoprazole or pantoprazole.Methods : The study involved a search of the Food and Drug Administration's database for adverse events and drug interactions with omeprazole, lansoprazole or pantoprazole as primary or secondary suspect drug. An estimate of the amount of drug dispensed during the adverse event collection period (from US drug launch) was obtained from the International Medical Statistics health database.Results : Of the suspected drug interactions recorded, vitamin K antagonist interactions, although rare, were the most common. The frequency of vitamin K antagonist interactions was 0.09 per million packages for omeprazole and 0.11 per million packages for lansoprazole and pantoprazole. Interactions with benzodiazepines or phenytoin were even rarer, being reported in less than 10 patients on each proton pump inhibitor.Conclusion  The frequency of reported drug interactions was low for omeprazole, lansoprazole and pantoprazole and vitamin K antagonist interactions were by far the most common. These potentially important drug interactions, although rare, were no less frequent on pantoprazole than on omeprazole or lansoprazole, suggesting a class effect.

    Notes: Background : Despite a long-standing use of herbal drugs with dyspeptic symptoms, little attention has been paid to their clinical evaluation.Aim : To assess efficacy and safety of the herbal drug preparation STW 5 (containing, e.g. Iberis, peppermint, chamomile) in the treatment of functional dyspepsia.Methods : Research in electronic databases, consultation of experts and of the producer identified STW 5 (Iberogast) as descriptor in six randomized-controlled trials. The raw data of three placebo-controlled studies which met the selection criteria, were reanalysed and pooled for meta-analysis; one reference-controlled study supported the safety analysis (STW 5: n = 199, control: n = 198).Results : Pooled data showed verum (n = 138) to be more effective than placebo (n = 135) with regard to the severity of the most bothersome gastrointestinal symptom (P-value: 0.001, odds ratio: 0.22, 95% CI: 0.11–0.47). A fourth randomized-controlled trial showed no significant difference between STW 5 and cisapride. As to safety, adverse events were similar with verum and placebo; no serious adverse events occurred.Discussions : From the point of view of efficacy and safety, the herbal medicinal product STW 5 appears to be a valid therapeutic option for patients seeking phytotherapy for their symptoms of functional dyspepsia.
