    Description / Table of Contents: Zusammenfassung Bei 5 Patienten mit essentieller Hypertonie wurde der Einfluß einer einmaligen Injektion von 12,1 mg Guanethidinbase auf Blutdruck und Katecholaminausscheidung im Harn verglichen. Nach der Injektion des Antihypertensivums kam es zu einem Abfall der Noradrenalinausscheidung bei unveränderter Adrenalinausscheidung. Der am liegenden und am stehenden Patienten gemessene systolische und diastolische Blutdruck fiel nach Injektion des Pharmakons deutlich ab. Die Kreatininausscheidung im Harn änderte sich nicht. Bei 6 Hypertonikern wurde der Einfluß einer 3 Wochen dauernden Behandlung mit Guanethidin auf die Ausscheidung von Noradrenalin und Adrenalin im Urin geprüft. Die verabreichten Dosen zwischen 20,2 und 80,8 mg Guanethidinbase pro Tag senkten den am liegenden und am stehenden Patienten gemessenen systolischen und diastolischen Blutdruck. Unter Guanethidintherapie nahm die Noradrenalinausscheidung im Harn ab, während die Ausscheidung von Adrenalin und von Kreatinin sich nicht änderte. Die geschilderten Befunde sowie der nach Guanethidin-injektion beobachtete Zeitverlauf von Blutdrucksenkung einerseits und Veränderung der Noradrenalinausscheidung andererseits sind vereinbar mit der Vorstellung, daß Guanethidin auch beim Menschen durch Verminderung der Noradrenalinfreisetzung aus peripheren sympathischen Nerven blutdrucksenkend wirkt. Weder nach einmaliger intravenöser Injektion noch bei einer 3 Wochen dauernden Behandlung mit oralen Guanethidindosen wurde bei den untersuchten Hypertonikern eine vermehrte Katecholaminausscheidung im Harn festgestellt. Eine Änderung der Ausscheidung von Noradrenalin und Adrenalin, welche ein Phäochromocytom vortäuschen könnte, trat demnach bei Verabreichung von Guanethidin nicht ein.
    Notes: Summary The effects of a single intravenous injection of 12.1 mg guanethidine base on both blood pressure and urinary catecholamine excretion were compared in 5 patients with essential hypertension. The intravenous administration of guanethidine decreased norepinephrine excretion, whereas epinephrine excretion in the urine remained unchanged. Diastolic blood pressure in supine and standing position was considerably lowered after the injection of the drug. Urinary creatinine excretion was not altered. In 6 hypertensive patients, the influence of a 3-week's treatment with oral doses of guanethidine on the urinary excretion of norepinephrine and epinephrine was studied. At doses ranging from 20.2 to 80.8 mg/day, blood pressure in supine and standing position as well as urinary norepinephrine excretion were decreased. Urinary excretion of epinephrine and creatinine, however, remained unchanged. These data as well as the time course of the effect on blood pressure and on norepinephrine excretion are compatible with the hypothesis that also in man the antihypertensive effect of guanethidine is due to a decrease of norepinephrine release from peripheral sympathetic nerves. Neither after a single intravenous injection nor after a 3-week's oral treatment with guanethidine was there an increase in urinary catecholamine excretion simulating phaeochromocytoma.

    Description / Table of Contents: Summary In 8 normotensive subjects and in 8 patients with essential hypertension total catecholamine content and dopamine-β-hydroxylase (DBH) activity in plasma were measured at rest and during physical exercise. Catecholamine levels at rest were significantly higher in the hypertensive than in the normotensive persons whereas the 2 groups of subjects did not differ in DBH activity at rest. At a work load of 150 watts there was a more pronounced increase of the catecholamine concentrations and of the DBH activity in the hypertensive patients than in the normotensives. One may conclude from the data that there is an increased sympathetic nerve activity in patients with essential hypertension.
    Notes: Zusammenfassung Bei 8 Normotonikern sowie bei 8 Patienten mit essentieller Hypertonie wurden die Konzentrationen der Gesamtkatecholamine und die Aktivität der Dopamin-β-Hydroxylase (DBH) im Plasma unter Ruhebedingungen und während körperlicher Belastung gemessen. Bei den Hypertonikern waren die Ruhewerte der Katecholamine signifikant höher als bei den Normotonikern, während die DBH-Aktivität unter diesen Bedingungen bei beiden Gruppen gleich war. Bei einer Ergometerbelastung von 150 Watt stiegen die Katecholaminspiegel und die DHB-Aktivität bei den Hochdruckkranken stärker an als bei den Probanden mit normalem Blutdruck. Aus den Untersuchungsergebnissen kann man schließen, daß bei Patienten mit essentieller Hypertonie eine erhöhte Aktivität sympathischer Nerven besteht.

    Description / Table of Contents: Summary As pheochromocytoma sometimes is accompanied by medullary thyroid carcinoma (in the sense of multiple endocrine adonomatosis type II=Sipple-Syndrome), serum calcitonin (CT) was measured by radioimmunoassay in 4 patients with pheochromocytoma. Before extirpation of the adreno-medullary tumor, serum CT was distinctly increased to 3 and 30 ng/ml in 2 of 4 patients, respectively. After removal of the tumor, serum CT was normal in the patients and pentagastrin stimulation produced no exaggerated CT response. In hydrochloric acid extracts from the two corresponding pheochromocytoma tissues, immunoreactive-Calcitonin (IR-CT) was detected, the concentrations amounting 1 and 4 ng/mg wet tissue. These findings suggest that hypercalcitonism in patients with pheochromocytoma cannot always be ascribed to the thyroid, i.e. increased calcitonin levels do not necessarily indicate a medullary carcinoma of this organ.
    Notes: Zusammenfassung Da Phäochromocytome in Kombination mit einem medullären Schilddrüsen-Carzinom auftreten können (multiple endokrine Adenomatose Typ II=Sipple-Syndrom), wurden bei 4 Patienten mit Phäochromocytomen die Serumcalcitoninspiegel radioimmunologisch bestimmt. Bei 2 der 4 Patienten fanden sich vor Operation des Nebennierenmarktumors deutlich erhöhte Calcitoninspiegel von 3 und 30 ng/ml. Nach Entfernung des Tumors normalisierten sich die Calcitoninspiegel, ebenso erzeugte die Pentagastrin-Stimulation keinen pathologischen Anstieg des Calcitonins. In salzsauren Extrakten aus den entsprechenden beiden Phäochromocytomen fand sich radioimmunologisch bestimmbares Calcitonin; die Werte betrugen 1und 4 ng/mg Frischgewicht. Der Nachweis einer Hypercalcitoninämie bei Patienten mit Phäochromocytomen läßt daher nicht unbedingt den Schluß auf ein gleichzeitig bestehendes medulläres Schilddrüsen-Carcinom zu, da offensichtlich Nebennierenmarktumoren zu Calcitoninproduktion und -sekretion fähig sind.

    Description / Table of Contents: Summary The pharmacological evidence for the classification of adrenergic α- and β-receptors is presented. Drugs blocking α-receptors have been used therapeutically for many years. β-adreneric receptor blocking agents were introduced only recently; they are discussed in more detail. The pharmacological actions, side effects and therapeutic uses of the receptor blocking drugs are summarized.
    Notes: Zusammenfassung Die wichtigsten pharmakologischen Befunde, die zur Klassifizierung der adrenergen α- und β-Receptoren geführt haben, werden besprochen. Hemmstoffe der α-Receptoren werden bereits seit längerer Zeit zu therapeutischen Zwecken verwandt. Ausführlicher werden die Hemmstoffe der β-Receptoren behandelt, die erst in den letzten Jahren Eingang in die Klinik gefunden haben. Die pharmakologischen Wirkungen, Nebenwirkungen und therapeutischen Indikationen der Receptorenhemmstoffe werden besprochen.

    Description / Table of Contents: Summary In 33 patients, a comparison was made of the actions of the beta adrenergic blocking agents propranolol, 1-(3-methylphenoxy)-2-hydroxy-3-isopropylamino-propane (MHIP; Kö 592), and N-isopropyl-p-nitrophenylethanol amine (INPEA). The effects on the following parameters were studied: heart rate, cardiac output, and the positive chronotropic action of 2µg isoproterenol. All drugs were given intravenously. Propranolol (5 mg), MHIP (5 mg), and (−)-INPEA (50 to 75 mg) decreased heart rate, cardiac output, and the positive chronotropic action of isoproterenol. From the following results it is concluded that the beta adrenergic blocking activity is not related to the decrease of heart rate and cardiac output in man: 1. In doses ranging from 0.5 to 5 mg the dose-response curve of the beta adrenergic blocking actions of propranolol and MHIP had a much steeper slope than the dose-response curves of the effects on heart rate and cardiac output. 2. There was no correlation of the beta adrenergic blocking action of 50 mg (−)-INPEA with effects on heart rate and on cardiac output. 3. 50 to 75 mg (−)-INPEA antagonized the positive chronotropic effect of isoproterenol, whereas 50 to 75 mg (+)-INPEA had no beta receptor blocking activity. However, the effects of both optical isomers of INPEA on heart rate and on cardiac output did not differ.
    Notes: Zusammenfassung An insgesamt 33 Patienten wurde der Einfluß der i.v. verabreichtenβ-Receptoren blockierenden Substanzen Propranolol, 1-(3-Methylphenoxy)-2-hydroxy-3-isopropylamino-propan (MHIP; Kö 592) und N-Isopropyl-p-nitrophenyläthanolamin (INPEA) auf Herzfrequenz und Herzminutenvolumen untersucht und mit derβ-adrenolytischen Wirkung verglichen. Als Maß für denβ-adrenolytischen Effekt wurde die Hemmung der positiv chronotropen Wirkung von 2µg Isoproterenol i.v. bestimmt. Propanolol (5 mg), MHIP (5 mg) und (−)-INPEA (50–75 mg) antagonisierten die positiv chronotrope Isoproterenolwirkung und verminderten Herzfrequenz und Herzminutenvolumen. Folgende Untersuchungsergebnisse sprechen gegen einen Zusammenhang vonβ-adrenolytischen Wirkungen und dem Einfluß auf Herzfrequenz und Herzminutenvolumen derβ-Receptorenblocker: 1. In einem Dosisbereich von 0,5–5 mg verlief die Dosis-Wirkungs-Kurve für dieβ-adrenolytische Wirkung von Propranolol und von MHIP wesentlich steiler als die Dosis-Wirkungs-Kurven für den Einfluß der beidenβ-Receptorenblocker auf Herzfrequenz und Herzminutenvolumen. 2. Dieβ-adrenolytische Wirkung von 50 mg (−)-INPEA war nicht mit dem Einfluß auf die Herzfrequenz bzw. auf das Herzminutenvolumen korreliert. 3. 50 bis 75 mg (−)-INPEA hatte eine deutlicheβ-Receptoren blockierende Wirkung, während (+)-INPEA in der gleichen Dosierung nichtβ-adrenolytisch wirksam war. Beide optischen Isomere von INPEA senkten die Herzfrequenz und das Herzminutenvolumen jedoch gleich stark.

    Description / Table of Contents: Zusammenfassung An 13 Hypertonikern wurde die Harnausscheidung von α-Methyldopa, α-Methyldopamin und Katecholammen während einer Dauerbehandlung oder nach einer Einzeldosis von α-Methyldopa untersucht. Die im Harn enthaltenen Substanzen werden durch Ionenaustausch- und Papierchromatographie voneinander getrennt und fluorometrisch oder biologisch bestimmt. — Von alien Patienten wurde außer freiem und konjugiertem α-Methyldopa und α-Methyldopamin auch α-Methylnoradrenalin ausgeschieden. Gleichzeitig nahm die Ausscheidung von Noradrenalin gegenüber der Vor-und Nachperiode bei fast allen Patienten ab. Es kann ausgeschlossen werden, daß die bei der Behandlung durch die Nieren ausgeschiedenen Mengen von α-Methyldopa, α-Methyldopamin oder α-Methylnoradrenalin die renale Ausscheidung von Noradrenalin herabsetzten. Die während der Behandlung ausgeschiedene Menge von Noradrenalin, Adrenalin und α-Methylnoradrenalin zusammengenommen unterschied sich nicht von der Katecholamin-Ausscheidung vor und nach der Behandlung. — Das biosynthetisch vom Menschen gebildete α-Methylnoradrenalin verhielt sich nach dem Ergebnis chemischer und pharmakologischer Tests wie Corbadrin, welches die Erythro-Konfiguration aufweist; α-Methylnoradrenalin ließ sich eindeutig von dem Diastereomeren von Corbadrin differenzieren, welches die Threo-Konfiguration besitzt. Im Harn der Patienten konnte α-Methyladrenalin nicht nachgewiesen werden. Gegen eine Beteiligung von α-Methyladrenalin, welches nach Tierversuchen durch Erregung von adrenergen β-Receptoren blutdrucksenkend wirkt, an der hypotensiven Wirkung von α-Methyldopa spricht ferner, daß die Blutdruckabnahme nach α-Methyldopa durch Propranolol nicht aufgehoben, sondern eher sogar verstärkt wurde. — Die systolische Blutdrucksenkung war sowohl der Abnahme der renalen Ausscheidung von Noradrenalin als auch dem Prozentsatz von α-Methylnoradrenalin an der gesamten Katecholamin-Ausscheidung significant korreliert; auch im Zeitverlauf ergab sich bei drei Patienten nach einer Einzeldosis von α-Methyldopa eine Korrelation zwischen hypotensivem Effekt einerseits und Abnahme der Noradrenalin- bzw. Zunahme der α-Methylnoradrenalin-Ausscheidung andererseits. — Die Ergebnisse machen es wahrscheinlich, daß der früher in Tierversuchen nach Verabreichung von α-Methyldopa nachgewiesene teilweise Ersatz des sympathischen Überträgerstoffs Noradrenalin durch α-Methylnoradrenalin auch unter therapeutischer Dosierung am Menschen stattfindet. Die Korrelationen von hypotensiven Effekten und Veränderungen im Aminstoffwechsel lassen den SchluB zu, daß α-Methyldopa den Blutdruek von Hypertonikern durch Freisetzung von α-Methylnoradrenalin als „falscher” Überträgersubstanz und entsprechende Verminderung der Noradrenalin-Freisetzung aus sympathischen Nervenfasern herabsetzt.
    Notes: Summary In 13 hypertensive patients the urinary excretion of α-methyldopa, α-methyldopamine und catechol amines was analyzed either during continuous administration or after a single dose of α-methyldopa. The compounds contained in the urine samples were separated by ion exchange and paper chromatography and determined fluorimetrically and biologically. — All patients excreted both free and conjugated α-methyldopa, α-methyldopamine, and small amounts of α-methylnoradrenaline. Compared with the pre- and postdrug period, in nearly all patients the noradrenaline excretion was decreased while α-methyldopa was administered. The experimental data obtained exclude the possibility that the noradrenaline excretion was diminished by the simultaneous excretion of α-methylnoradrenaline or of excessive amounts of α-methyldopa and α-methyldopamine. The combined quantities of noradrenaline, adrenaline and α-methylnoradrenaline excreted daily during administration of α-methyldopa did not differ significantly from the amount of noradrenaline and adrenaline excreted daily before drug treatment was started and after it was discontinued. —Evidence was obtained that α-methylnoradrenaline isolated from urine of patients given α-methyldopa behaved chemically and biologically in a way similar to (−)-corbadrine (erythro-configuration) but differed markedly from the diastereomer of corbadrine (threo-configuration). Hence, biosynthesis of α-methylnoradrenaline leads to the levorotatory erythro-isomer. — In animal experiments carried out previously α-methyladrenaline was found as another metabolite of α-methyldopa which decreased blood pressure by activation of adrenergic β-receptors. The patients given α-methyldopa did not excrete α-methyladrenaline. The method employed was sensitive enough to detect amounts of α-methyladrenaline less than 3 per cent of the α-methylnoradrenaline present. Furthermore, involvement of a β-adrenergic component in the response of the blood pressure to α-methyldopa in hypertensive patients was made unlikely by the observation that propranolol did not antagonize but rather enhanced the hypotensive effect of α-methyldopa. — There was a significant correlation between the fall of systolic blood pressure and both the decrease in urinary excretion of noradrenaline and the increase in the percentage of α-methylnoradrenaline of the total catecholamine output. Likewise, the time course of the depressor response to a single dose of α-methyldopa closely corresponded to the decrease in noradrenaline and the increase in α-methylnoradrenaline excretion. Conversely, the return of the blood pressure to the initial level was reflected by an increase in noradrenaline and a decrease in α-methylnoradrenaline excretion. — In animal experiments it was previously found that administration of α-methyldopa caused a partial displacement of noradrenaline by α-methylnoradrenaline which subsequently was released by sympathetic nerve stimulation. The present findings demonstrate that α-methylnoradrenaline is formed in man as well. It is concluded that α-methyldopa lowers the blood pressure of hypertensive patients by the release of α-methylnoradrenaline as a “false” neurotransmitter and the concomitant decrease in noradrenaline liberation from sympathetic nerve fibres.

    Notes: Summary In hypertensive patients, the effect of propranolol and of the optical isomers of N-isopropyl-p-nitrophenylethanolamine (INPEA) on blood pressure was studied. Single oral doses of 40 mg propranolol, 700 mg (−)-INPEA and 700 mg (+)-INPEA decreased supine systolic as well as standing systolic and diastolic blood pressure. Single intravenous doses of 75 mg (−)-INPEA and 75 mg (+)-INPEA lowered supine systolic and diastolic blood pressure. The antihypertensive action of the beta adrenergic blocking levorotatory isomer of INPEA and of the dextrorotatory isomer, which is devoid of beta adrenergic blocking activity, did not differ. It is concluded from these results that the antihypertensive action of beta adrenergic blocking agents is not related to their beta receptor blocking effect.

    Notes: Summary Parameters of the autonomic nervous system were studied in normotensive subjects (NT; standing blood pressure (BP)≤125/85 mmHg) and in subjects with borderline hypertension (BHT; 140/90≤standing BP〈60/100 mmHg). No differences in plasma noradrenaline and adrenaline levels were found between NT and BHT subjects, neither at rest nor during exercise at 75% of maximum work capacity. The dose of noradrenaline required to increase systolic BP by 10 mmHg was significantly higher in NT than in BHT subjects (5.13±0.42 vs 3.50±0.57 µg · min−1). No difference between NT and BHT subjects was found in the dose of isoprenaline required to increase heart rate by 20 beats · min−1 (1.21±0.12 vs 1.09±0.11 µg · min−1). Resting salivary flow was significantly lower in BHT than in NT subjects (0.39±0.06 vs 0.98±0.06 g · min−1), suggesting decreased parasympathetic activity in the former group. The enhanced pressor effect of noradrenaline, together with the decreased parasympathetic activity, could explain the elevated blood pressure and heart rate in subjects with borderline hypertension.
