Abstract:
Zusammenfassung Bei 156 nachgewiesenen Akustikus-Neurinomen wurden die Röntgen-Übersichtsaufnahmen noch einmal ausgewertet. Die diagnostische Genauigkeit betrug 75%. Es wird hervorgehoben, daß eine Untersuchung in 3 Ebenen (Towne-projection, Aufnahmen im seitlichen und sagittalen Strahlengang) ausreichend ist und daß die Tomographie für eine Screening-Untersuchung nur wenig neue Gesichtspunkte ergibt.

Summary A retrospective study of 156 surgically and pathologically proven cases of acoustic neuroma is reported. The plain skull films were assessed by a method which was designed to minimize observer bias and normal controls were included amongst the cases. The internal auditory canal and porus acousticus were examined for porosis, widening, erosion or any combination of these. In nearly every case of acoustic neuroma with radiological abnormality erosion plus widening was present. The results indicate that a Towne view and an AP view (through the orbits) together enabled detection of 75% of acoustic neuromas, and that the other additional films did not improve in this figure. It is noted that this diagnostic accuracy approximates that which has been reported using tomography, but in practice, neither examination influences the subsequent investigation of the patient. The relatively minor role of both plain films and tomography in the diagnosis of acoustic neuroma is emphasized. It is suggested that a three film examination (Towne's, Perorbital and Lateral) is adequate, and that tomography has little value in the initial screening examinations.
