    Notes: Summary Since there is a need for a widely applicable non-invasive test to assess gastric emptying in diabetic patients, we evaluated the sensitivity, specificity, and reproducibility of the [13C]octanoic acid breath test as compared with scintigraphy. Moreover, we examined the relationship between the breath test indices and gastric symptoms, cardiovascular autonomic function, and metabolic parameters. Forty healthy control subjects and 34 diabetic patients were studied. Three indices of gastric emptying, assessed by the breath test, were computed: half-emptying time (t1/2breath), gastric emptying coefficient (GEC), and lag phase. Furthermore, the half-emptying time, measured by scintigraphy (t1/2scint), was calculated and gastric symptoms and cardiovascular autonomic neuropathy (CAN) were scored. The coefficients of variation of day-to-day reproducibility in 10 healthy subjects were 29.6 % for t1/2breath, 7.4 % for GEC, and 46.5 % for lag phase. An abnormal delay for t1/2scint (〉 100 min) or t1/2breath (〉 200 min) was noted in 12 patients. Based on the results for t1/2scint, the sensitivity of t1/2breath and GEC was 75 % and the specificity was 86 %. Both t1/2breath (r s = 0.523; p 〈 0.05) and GEC (r 2 = − 0.594; p 〈 0.05) were significantly associated with the gastric symptom score. A significant relationship to the CAN score was demonstrated for t1/2breath (r s = 0.448; p 〈 0.05), GEC (r s = − 0.467; p 〈 0.05), and t1/2scint (r s = 0.602; p 〈 0.05). There were no significant associations of the breath test indices with the blood glucose levels during the test, HbA1c, age, and duration of diabetes. In patients with abnormal t1/2scint (n = 12) not only was t1/2breath significantly prolonged and GEC reduced, but also the scores of CAN and gastric symptoms were significantly increased as compared with those who had a normal t1/2scint (n = 22). We conclude that the [13C]octanoic acid breath test represents a suitable measure of delayed gastric emptying in diabetic patients which is associated with the severity of gastric symptoms and CAN but not affected by the blood glucose level. Im unselektierten Patientenmaterial (n=104) bestanden teilweise erhebliche Bestimmungsunterschiede; die mit dem EIA gemessenen Konzentrationen waren signifikant kleiner (1,09±0,99 ng/ml für EIA, 1,34±1,01 ng/ml für RIA,p〈0,01). Hämolytische, lipämische und urämische Seren hatten falsch negative Ergebnisse im EIA; nach Elimination dieser Seren resultierte eine gute Übereinstimmung der Meßwerte mit beiden Methoden (1,12±1,01 ng/ml für EIA, 1,12±1,02 ng/ml für RIA,r=0,95,n=63).
    Notes: Summary The results of parallel determinations of digoxin in the sera of non selected patients (n=104) by enzyme immunoassay (EMIT-EIA) and radioimmunoassay (J-125 labeled RIA) were compared with each other. The determinations revealed considerably different concentrations; the values determined by EIA were statistical lower (for EIA 1.09±0.99 ng/ml, for RIA1.34 ±1.01 ng/ml,p〈0.01). In sera with hemolysis and in sera of patients with hyperlipidemia and uremia false-negative results were found by EIA. After elimination of these sera an exact concordation of the values of both the methods was obtained (for EIA 1.12±1.01 ng/ml, for RIA 1.12±1.02 ng/ml,r=0.95).

    Notes: Abstract. A novel ultrasonic imaging method, wideband harmonic imaging, for nonlinear imaging of microbubble contrast agents is evaluated. In wideband harmonic mode, two pulses of alternate phase are send out. The image is then processed from the sum of both pulses, resulting in an image of nonlinear scatterers such as microbubbles. A prototype ultrasound system, Siemens Elegra, was evaluated with in vitro investigations and animal trials, using conventional, harmonic and wideband harmonic settings with the galactose based ultrasound contrast agent Levovist. Wideband harmonic imaging offers superior sensitivity for ultrasound contrast agents compared to conventional imaging and harmonic imaging. At low transmit power settings (MI 0.1–0.5) the nonlinear response is already sufficient to generate a image of the blood pool distribution of Levovist in the rabbit kidney including the microvasculature, with clear delineation of vessels and perfused parenchyma. At high transmit amplitudes, nonlinear tissue response reduced the apparent image contrast between contrast agent and tissue. The results suggest that wideband harmonic imaging is currently the most sensitive contrast imaging technique, maintaining highest spatial resolution. This may add to image quality and offer new clinical potential for the use of ultrasound contrast agents such as Levovist.

    Notes: Abstract Sapphirine-bearing rocks occur in three conformable, metre-size lenses in intrusive quartzo-feldspathic orthogneisses in the Curaçà valley of the Archaean Caraiba complex of Brazil. In the lenses there are six different sapphirine-bearing rock types, which have the following phases (each containing phlogopite in addition):A: Sapphirine, orthopyroxene;B: Sapphirine, cordierite, orthopyroxene, spinel;C: Sapphirine, cordierite;D: Sapphirine, cordierite, orthopyroxene, quartz;E: Sapphirine, cordierite, orthopyroxene, sillimanite, quartz;F: Sapphirine, cordierite, K-feldspar, quartz.Neither sapphirine and quartz nor orthopyroxene and sillimanite have been found in contact, however. During mylonitization, introduction of silica into the three quartz-free rocks (which represent relict protolith material) gave rise to the three cordierite and quartz-bearing rocks. Stable parageneses in the more magnesian rocks were sapphirine–orthopyroxene and sapphirine–cordierite. In more iron-rich rocks, sapphirine–cordierite, sapphirine-cordierite–sillimanite, cordierite–sillimanite, sapphirine–cordierite–spinel–magnetite and quartz–cordierite–orthopyroxene were stable. The iron oxide content in sapphirine of the six rocks increases from an average of 2.0 to 10.5 wt % (total Fe as FeO) in the order: C,F–A,D–B,E. With increase in Fe there is an increase in recalculated Fe2O3 in sapphirine.The four rock types associated with the sapphirine-bearing lenses are:I: Orthopyroxene, cordierite, biotite, quartz, feldspar tonalitic to grandioritic gneiss;II: Biotite, quartz, feldspar gneiss;III: Orthopyroxene, clinopyroxene, hornblende, plagioclase meta-norite;IV: Biotite, orthopyroxene, quartz, feldspar, garnet, cordierite, sillimanite granulite gneiss.The stable parageneses in type IV are orthopyroxene–cordierite–quartz, garnet–sillimanite–quartz and garnet–cordierite–sillimanite.Geothermobarometry suggests that the associated host rocks equilibrated at 720–750°C and 5.5–6.5 kbar. Petrogenetic grids for the FMASH and FMAFSH (FeO–MgO–Al2O3–Fe2O3–SiO2–H2O) model systems indicate that sapphirine-bearing assemblages without garnet were stabilized by a high Fe3+ content and a high XMg= (Mg/ (Mg+Fe2+)) under these P–T conditions.
