    Notes: Summary 70 patients were treated for spinal dural arteriovenous fistula in the same centre, during a period of 10 years. Conus medullaris and cauda equina syndromes were observed in all patients as the clinical stereotyped presentation. Diagnosis was based on myelography in the first instance and then on angiography. 40 patients were treated by intravascular neuroradiological embolization, whereas the other 30 were operated on. Surgery was proposed because embolization was contraindicated (7 cases of hazardous catheterisation) of inefficient (23 cases=38%). The results of the operative series are presented, and compared with those of embolization. Improvement was observed in 50% of the 20 patients with follow up, but a complete recovery to an asymptomatic state was never obtained. For the other patients (47%) complete stabilization of the disease could be obtained, whereas in one of the patients (3%), who was operated upon because of failure of embolization, surgery was also completely ineffectual. The long-term results of patients treated surgically are comparable with those patients efficiently embolized. 5 patients of the operative series were submitted to MRI before and after surgery: the results and the place of MRI are discussed.

    Description / Table of Contents: Summary There is evidence that an immunoregulatory circuit integrated by immune-derived cytokines and the hypothalamus-pituitary-adrenal (HPA) axis operates during certain pathological conditions. For example, it has been shown that IL-1 is the main mediator of the increase in ACTH and corticosterone blood levels detected in models of viral infection and bacterial endotoxins. This endocrine response has protective effects during septic shock. In experimental models of tumors, there are also clear indications that the increase in glucocorticoid levels detected during the growth of a lymphoma is mediated by immune-derived products and contributes to the inhibition of the inflammatory response. The disruption of the cytokine-HPA axis circuit can predispose to autoimmunity. This has been shown in animal models of spontaneous autoimmune thyroiditis, lupus-like disease, and experimental arthritis. More recently, it has been shown that the proper operation of this circuit contributes to preventing or moderating autoimmunity. The recovery of animals from experimental autoimmune encephalomyelitis (EAE) is clearly dependent on an increase in endogenous glucocorticoid levels. It has been recently shown that this endocrine response is, at least in part, triggered by the immune response to the encephalitogenic antigen and medicated by the endogenous IL-1 produced during the disease. These examples support the concept that the cytokine-HPA circuit plays a protective role during certain pathologies and that its disruption can lead to predisposition to or aggravation of autoimmune diseases.
    Notes: Zusammenfassung Es gilt als gesichert, dass ein immunregulatorischer Regelkreis von Zytokinen mit der Achse Hypothalamus-Hypophyse-Nebennierenrinde /HPA) während verschiedenster pathologischer Abläufe wirksam ist. So konnte beispielsweise gezeigt werden, dass IL-1 der wichtigste Mediator des Anstiegs von ACTH und Kortikosteron im Blut in Tiermodellen viraler Infektion und bakterieller Endotoxine ist. Diese endokrinologische Antwort hat protektive Effekte im septischen Schock. In experimentellen Tumormodellen ergaben sich ebenfalls klare Hinweise, dass der Glukokortikoidanstieg im Blut, welcher während des Wachstums von Lymphomen beobachtet wird, durch Mediatoren des Immunsystems vermittelt ist und den begleitenden Entzündungsprozess hemmt. Die Unterbrechung des Zytokin-HPA-Achse-Regelkreises kann für die Autoimmunerkrankungen prädisponieren. Dies konnte für die Tiermodelle der spontanen Autoimmunthyreoiditis, Lupus-ähnlicher Erkrankungen und experimenteller Arthritiden gezeigt werden. Erst kürzlich wurde demonstriert, dass der intakte Regelkreis zur Vermeidung oder mindestens Abschwächung autoimmunologischer Reaktionen beiträgt. Die Erholung von Tieren mit experimenteller Autoimmunenzephalitis (EAE) eindeutig abhängig vom Anstieg endogener Glukokortikoide. So konnte gezeigt werden, dass diese endokrine Antwort zumindest teilweise durch die Immunantwort gegen das Enzephalitis-auslösende Antigen vermittelt ist und zwar durch das IL-1, welches während der Erkrankung gebildet wird. Diese Beispiele verdeutlichen das Konzept einer protektiven Rolle, die der Zytokin-HPA-Achsen-Regelkreis während verschiedener pathologischer Zustände spielt. Seine Störung kann zur Auslösung von Autoimmunerkrankungen beitragen oder ihren Verlauf ungünstig beeinflussen.

    Description / Table of Contents: Summary Due to their anti-inflammatory effects and their capacity to affect the synthesis of most cytokines and the Th1/Th2 balance, endogenous glucocorticoids contribute to control the basal operation of immune cells. Immune processes result in the production of cytokines, such as IL-1, IL-2, IL-6, IL-11, IL-12, TNF-α, and INF-γ, which can activate the HPA axis. In turn, the resulting increase in glucocorticoid blood levels can affect immune cell activity directly or by controlling the production of cytokines, suggesting the existence of an immunoregulatory cytokine-HPA axis circuit. There is evidence that this circuit plays a relevant role in controlling excessive inflammatory reactions and the non-specific expansion of immune cells with no or low affinity for the antigen that triggers an immune response.
    Notes: Zusammenfassung Wegen ihrer anti-inflammatorischen Effekte und ihrer Potenz, die Synthese der meisten Zytokine und die Balance Th1/Th2 zu beeinflussen, tragen die endogenen Glukokortikoide zur basalen Regulation der Immunzellen bei. Immunologische Prozesse verursachen die Produktion von Zytokinen, wie IL-1, IL-2, IL-6, IL-11, IL-12, TNF-α, INF-γ, welche ihrerseits die HPA Achse stimulieren können. Der dadurch vermittelte Anstieg von Glukokortikoiden wirkt entweder direkt oder über die Beeinflussung der Produktion von Zytokinen auf immunologische Zellen, was die Existenz eines zytokinvermittelten immunregulatorischen Regelkreises mit der HPA Achse nahelegt. Es gilt als gesichert, dass dieser Regelkreis eine bedeutsame Rolle spielt bei der Kontrolle einer überschießenden Immunantwort und der Unterdrückung der Expansion von Immunzellen mit fehlender oder nur geringer Affinität zu Antigenen, welche die spezifische Immunantwort auslösen.
