Zusammenfassung Bei 20 Fällen, bei denen wegen verschiedener intracranieller Erkrankungen eine Carotisangiographie ausgeführt werden mußte, wurden die Auswirkungen einer intracarotidialen Injektion von Amytal auf das EEG und das neurologische und psychische Verhalten untersucht. 1. Die Injektion des Pharrnakons in die Carotis verursacht regelmäßig vorübergehende EEG-Veränderungen. In Abhängigkeit von der Dosis (5 bis 100 mg) trat rasche Betha- oder langsame Delta-Aktivität auf. Die EEG-Veränderungen waren in der Regel auf der gleichen Seite; allerdings wurden manchmal auch doppelseitige Veränderungen beobachtet. 2. Die Injektion von Amytal in die Carotis verursacht vorübergehende motorische und sensorische Ausfälle. Zu diesen Symptomen kommt es nur dann, wenn im EEG langsame Wellen auftreten. Obwohl in der Regel kontralateral, werden diese Störungen dann doppelseitig beobachtet, wenn auch die EEG-Veränderungen bilateral sind. 3. Die Injektion von Amytal in die Carotis auf der Seite der dominanten Hemisphäre verursacht vorübergehende aphasische Symptome. Die Sprachstörungen sind nur dann vorhanden, wenn im EEG langsame Wellen auftreten. 4. Die intracarotidiale Injektion von Amytal kann vorübergehende psychische Veränderungen hervorrufen. Depressive Symptome werden bei Injektion auf der Seite der dominanten Hemisphäre ausgelöst, während der Injektion auf der nichtdominanten Seite euphorische Erscheinungen folgen können (Terzian und Cecotto, 1959). Diese psychischen Veränderungen sind nur dann feststellbar, wenn die Wirkung der Droge auf die Hemisphäre der injizierten Seite beschränkt bleibt. Sie sind begleitet von normaler oder rascherer EEG-Aktivität. Aber selbst unter diesen Bedingungen sind die psychischen Phänomene nicht immer zu erhalten. Diskussion der möglicherweise für die beobachteten Phänomene verantwortlichen Mechanismen.
Abstract:
Summary The EEG and behavioral effects of intracarotid injection of amytal (amobarbital sodium) have been studied in 20 patients who were undergoing carotid angiography for a variety of endocranic diseases. 1. The intracarotid injection of the drug constantly produces transitory EEG changes. According to the doses injected (5 to 100 mg.) fast beta activity or slow delta waves appear in the EEG. These EEG effects are usually ipsilateral, although sometimes bilateral changes are observed. 2. The intracarotid injection of amytal produces transitory motor and sensory defects. These defects are present only when slow waves occur in the EEG. Although usually contralateral, they are observed bilaterally when also the EEG changes are bilateral. 3. The intracarotid injection of amytal on the side of the dominant hemisphere produces transitory aphasic symptoms. The speech disturbances are present only when slow waves occur in the EEG. 4. The intracarotid injection of amytal may produce transitory psychic effects. Depressive symptoms may be elicited by injection on the side of the dominant hemisphere while euphoric symptoms may follow the injection on the side of the non-dominant hemisphere (Terzian and Cecotto, 1959). The psychic effects may be obtained only if the action of the drug remains confined to the injected hemisphere. They are accompanied by normal or by fast EEG activity. Even in these conditions, however, the psychic phenomena are not always present. The possible mechanisms responsible for the observed phenomena are discussed.

Riassunto In 20 soggetti adulti dei due sessi affetti da lesioni endocraniche di varia natura e che dovevano essere sottoposti ad esame radiologico carotidografico sono stati studiati gli effetti EEG e clinici della iniezione intracarotidea di amytal sodico. 1. L'iniezione intracarotidea del farmaco produce costantemente transitorie modificazioni dell'EEG, consistenti nella comparsa di attività rapida beta o di attività lenta delta, a seconda della dosi usate (da 5 a 100 mg). Questi effetti EEG sono in genere localizzati all'emicapo ipsilaterale al lato dell'iniezione; talvolta peró essi sono bilaterali. 2. L'iniezione intracarotidea di amytal produce transitori disturbi deficitari motori e sensitivi. Questi disturbi si osservano solamente quando nell'EEG compaiono le onde lente e sono generalmente controlaterali alla carotide iniettata; essi compaiono bilateralmente quando anche nell'EEG le alterazioni prodotte dal farmaco sono bilaterali. 3. Quando l'iniezione del farmaco è eseguita nella carotide corrispondente all'emisfero dominante compaiono transitori disturbi del linguaggio (sino a completa afasia). Come per i fenomeni deficitari motori e sensitivi anche le alterazioni del linguaggio si fanno evidenti solamente quando nell'EEG vi sono onde lente. 4. L'iniezione del farmaco nel circolo carotideo puó dar luogo ad una transitoria sintomatologia psichica. Le manifestazioni psichiche sono di tipo depressivo quando il farmaco è iniettato in corrispondenza dell'emisfero dominante, di tipo euforico quando è iniettato in corrispondenza dell'emisfero non dominante (Terzian e Cecotto, 1959). Questa sintomatologia compare solamente se l'azione dell'amytal rimane localizzata all'emisfero direttamente irrorato dalla carotide iniettata e quando nell'EEG o non vi sono alterazioni evidenti o vi è attività rapida; anche in queste condizioni, tuttavia, l'effetto non è costante. I possibili meccanismi responsabili dei fenomeni osservati vengono discussi.
