    Description / Table of Contents: Résumé Une connaissance très précise de l'anatomie cérébrale est nécessaire à l'appréciation d'une lésion à ce niveau. Si les sillons permettent l'identification très précise des aires corticales, il en est tout autrement pour les faisceaux de substance blanche. Le but de ce travail est un essai d'évaluation topographique de certains grands faisceaux en s'aidant de l'imagerie 3-D en IRM (étude faite avec un sigma 1.5 T GE et le programme Voxtool de General Electric pour l'évaluation de la surface du cerveau). Cette étude est intéressante lorsque des anomalies de la substance blanche sont observées chez des patients présentant un syndrome de dysconnexion ou des déficits neuropsychologiques visuels ou du langage. Elle peut aussi servir à titre pédagogique dans les domaines clinique, anatomique ou radiologique. Elle devrait aussi aider à la compréhension de certaines activités cérébrales en pathologie ou pour comprendre le résultat des explorations fonctionnelles (PET, IRM fonctionnelle). En effet, il est exceptionnel qu'une activité corticale soit isolée et la mise en jeu de zones corticales différentes ne peut se faire que par le jeu des connexions établies par les fibres de la substance blanche du cerveau. Le but de ce travail est de proposer un petit atlas 3-D qui en montre la topographie pour les plus importants d'entre eux.
    Notes: Summary An accurate knowledge of cerebral anatomy is important in order to evaluate the precise location of a cerebral lesion. Cortical structures are identified by knowledge of the adjacent gyri and sulci; however, white matter tracts are difficult to differentiate from one another due to the lack of clear anatomic landmarks. Therefore, even if MRI shows obvious white matter abnormalities, in some cases it is difficult accurately to localize the lesion. The purpose of this study is to evaluate the location of the main white matter tracts by using three-dimensional MR imaging. MRI study was performed by 1.5 Tesla (Signa: General Electric). Computer assisted analysis with Voxtool software (General Electric) was used to generate both surface brain and tomographic images. The exact anatomic basis of white matter signal abnormalities is important when analyzing patients with disconnective syndromes or neuropsychological deficits such as conduction aphasia, visuospatial deficit etc. This preliminary attempt at constructing a three-dimensional MRI white matter atlas of the brain may be helpful for evaluating the anatomico-clinical correlations in these patients, and also as teaching materials for the clinical (neurologic, neurosurgical), anatomic and radiographic disciplines.

    Notes: Summary The authors report two cases of a rare tumor in adults which were inserted on the fornix and caused a frontal syndrome. By light microscopy, the tumors, highly calcified, were composed of small clear cells forming dense areas in a patchy fibrillary stroma. Electron microscopy revealed a striking neuronal differentiation with numerous synapses. These tumors, for which the term neurocytomas was proposed, were compared with the other CNS neuronal tumors described in the literature.

    Notes: Summary Stereoimpedoencephalography (SIEG) is routinely used by the authors during stereotactic investigations of glial brain tumours prior to stereotactic implantation of isotopes or prior to conventional surgery: the brain tissue impedance is measured along a probe track according to the four electrodes impedance technique. The correlation between the tissue structures and the corresponding impedance values has been studied by comparison with the CATScan data reported on the stereotactic schedules; impedance values are influenced by changes in arterial pCO2 and also by other unknown factors; in order to avoid these uninterpretable changes of the impedance, the recorded values have been normalized with respect to the average impedance along a control track in the normal hemisphere. As a rule, the impedance profile exhibits a “hole” at the site of the pathological processes like tumours as well as surrounding oedema, and the delimitation of the pathological area is very similar to that obtained by the CATScan. There is a good linear correlation between the values of normalized impedance and the X-ray absorption as given by the computer listing of the CATScan. Lastly, statistical values of normalized impedances are determined for the main structures of the brain, showing that the tumour tissue has an impedance equal to half the impedance of the normal tissue.

    Description / Table of Contents: Résumé A partir d'une exploration IRM de la fosse postérieure de 30 sujets volontaires sains une corrélation anatomoradiologique a été effectuée. L'examen fut pratiqué sur un appareil CGR Magniscan de 0,5 Tesla avec une séquence d'inversion récupération (TI: 21 ms, Tr. 2 000 ms, Ti: 500 ms). Cette séquence nous paraît être celle qui visualise le mieux la substance grise, la substance blanche et le liquide cérébrospinal. Afin d'étudier la configuration des principales structures du tronc cérébral 8 plans de coupes horizontales et 3 sagittales sont proposés.
    Notes: Summary Magnetic resonance imaging of 30 normal volunteers' posterior fossae was performed on a CGR Magniscan machine with a magnetic field strength of 0,5 Tesla. We chose the inversion recovery signal with a Tr of 2,000 ms, a TE of 21 ms and an inverse time of 500 ms. This sequence gives a better definition of the grey matter, the white matter and the CSF. To study the brain stem we suggest 8 horizontal and 3 sagittal sections.

    Notes: Résumé Les descriptions données par les différents auteurs sur la radioanatomie de l'artère cérébrale postérieure (arteria cerebri posterior CP) sont assez variables autant en ce qui concerne la nomenclature des branches que pour les types de division, le calibre et le trajet des artères individualisées sur l'angiographie conventionnelle. Sur la base de l'étude anatomo-radiologique de 76 hémisphères injectés dont 24 ont été disséqués en conditions stéréotaxiques, les auteurs présentent une analyse statistique des branches de la CP et proposent une nomenclature régionale en fonction des territoires artériels. La relation topographique des vaisseaux et des circonvolutions ne peut être pleinement mise en valeur que par une étude tridimensionnelle, stéréoangiographique, même si certains éléments apparaissent déjà sur l'angiogramme conventionnel. Un bref rappel de l'anatomie de la face interne et inféro-interne des hémisphères permet de dégager les grandes lignes de plissement de l'écorce, communes à la partie antérieure fronto-pariétale et la partie postérieure, temporo-occipitale: au centre, des sillons concentriques entourent les circonvolutions limbiques, tandis qu'en périphérie des sillons radiaires sont disposés perpendiculairement aux structures limbiques et au bord de l'hémisphère. Vus en trois dimensions, les sillons correspondent à des surfaces invaginées très étendues. Leurs équivalents angiographiques sont des lames vasculaires profondes, composées par tous les vaisseaux cheminant sur leur parois. Tenant compte du trajet tridimensionnel des vaisseaux et remplaçant les artères individuelles très variables par le concept des lames vasculaires profondes, la configuration constante des sillons et circonvolutions peut être facilement reconnue sur les images stéréoangiographiques de la CP. La topographie vasculaire des sillons choroïdien, collatéral, temporal inférieur, calcarin et pariéto-occipital est décrite et illustrée.
