    Description / Table of Contents: Summary We report the case of a 56-years old patient with clinical symptoms of an unresolved neuromuscular disease. Thelight microscopic studies of a muscle biopsy from the m. triceps shows the picture of a diffuse muscular atrophy. Byelectron microscopy, myelin-like degeneration zones with tubular-filamentous inclusions can be shown in the cytoplasma of the atrophic muscle cells. These filamentous structures correspond morphologically to the nucleocapside ofparamyxoviruses. These results lead, even without the proof of inflammatory cells, to the diagnosis of an “inclusion body” myositis also taking into account the clinical and electrophysiological findings.
    Notes: Zusammenfassung Es wird über einen 56jährigen Patienten mit den klinischen Symptomen einer unklaren neuromuskulären Erkrankung berichtet. Dielichtmikroskopische Untersuchung einer Muskelbiopsie aus dem m. triceps ergibt das Bild einer diffusen Muskelzellatrophie.Elektronenmikroskopisch werden im Cytoplasma der atropischen Muskelzellen myelinartige Degenerationsherde mit tubulär-filamentösen Einschlüssen nachgewiesen. Diese Filamentstrukturen entsprechen morphologisch dem Nucleocapsid von Paramyxoviren. Dieser Befund führt auch ohne den Nachweis interstitieller Entzündungszellen gemeinsam mit den klinischen und elektrophysiologischen Ergebnissen zur Diagnose einer „inclusion body myositis“.

    Notes: Summary The role of calcium accumulation in the pathogenesis of Duchenne muscular dystrophy (DMD) has already been discussed. Several trials with different calcium-blocking drugs have revealed no clinical benefit. In addition, the present study includes histological investigations and computer tomography to verify therapeutic effects. In a randomized placebo-controlled double-blind study, 13 DMD patients aged from 3–10 years (mean, 7 years) were treated with 5 mg/kg diltiazem daily for 1 year. Compared with before therapy, the number of calcium-positive muscular fibres was remarkably reduced in the treated DMD patients, but not in the placebo group. The evaluation of all other biochemical and clinical parameters revealed no significant effects of the diltiazem therapy. The muscularX-ray density measured by computer tomography decreased under treatment. After the evaluation of the double-blind study, the code was broken. Therapy, however, was continued in the treated group and started in the placebo group. After 3 years of diltiazem therapy the clinical status of all 26 patients of the study and 20 additional DMD patients who were treated with diltiazem was compared with 46 untreated DMD patients of the same age and stage in our department. No obvious clinical benefit of diltiazem therapy could be observed.

    Notes: Abstract Using a confocal microscope with a single-photon avalanche photodiode as detector, we studied photon bursts of single Rhodamine 6G (R6G) and Rhodamin B-zwitterion (RB) molecules in aqueous solution by excitation of the lowest excited singlet stateS 1 with a frequency-doubled titanium: sapphire laser. Multichannel scaler traces, the fluorescence autocorrelation function and fluorescence decay times determined by time-correlated single-photon counting have been measured simultaneously. The time-resolved fluorescence signals were analyzed with a maximum likelihood estimator. Fluorescence lifetime patterns in steps of 100 ps were generated by convolution with the excitation pulse. The lifetime of theS 1 state was derived from the Kullback-Leibler minimum discrimination information. We are able to demonstrate for the first time identification of two different single dye molecules via their characteristic fluorescence lifetimes of 1.79 ± 0.33 ns (RB) and 3.79 ± 0.38 ns (R6G) in aqueous solution.

    Notes: Abstract A few models of nonlinear optical systems, known experimentally to possess both stable and unstable dynamical modes, are approximated by different dynamical models and integrated by different numerical methods. It is shown that the onset of instabilities and chaotic behavior in the same physical system may be dependent on the model used and on the numerical method applied. Finite order difference schemes should be applied with caution to infinite dimensional dynamical systems displaying irregular behavior.

    Description / Table of Contents: Zusammenfassung Mit einer eigenen gaschromatographischen Methode bestimmten wir bei einer größeren Anzahl antikonvulsiv behandelter Kinder die Serumkonzentrationen der verwedenten Medikamente: Diphenylhydantoin, Primidon und Phenobarbital. Die Medikamente waren entweder als Monotherapie oder in Kombination miteinander oder mit anderen antikonvulsiv wirkenden Substanzen gegeben worden. An Hand der Regressionsgeraden ließ sich eine gute Korrelation zwischen der Menge der verabfolgten Medikamente (Gesamt-Tagesdosis) und den Serumkonzentrationen nachweisen. Die Regressionsgeraden für Diphenylhydantoin und Primidon wiesen jedoch keine Unterschiede auf, gleichgültig ob die Medikamente allein oder in Kombination gegeben worden waren. Auf Grund der Häufigkeit der Intoxikationserscheinungen sowie der Therapieversager stellten wir für Diphenylhydantoin und Primidon einen therapeutischen Bereich auf (Diphenylhydantoin 5–16 μg/ml, Primidon 4–14 μg/ml). Die erforderlichen Serumkonzentrationen können von 8–12 mg/kg Diphenylhydantoin und von 15–22 mg/kg Primidon erreicht werden. Trotz der guten Korrelation zwischen Gesamt-Tagesdosis und Serumspiegel weichen jedoch viele Patienten von diesem Normverhalten ab, besonders wenn Kombinationstherapien durchgeführt werden. Damit ist die Notwendigkeit zu routinemäßigen Serumspiegelkontrollen gegeben.
    Notes: Abstract With the aid of our own method of gas chromatography we determined serum concentrations of anticonvulsants in a large number of children who were being treated with diphenylhydantoin, primidone and phenobarbitone. The drugs were being prescribed either as monotherapy, or in combination with each other, or with other substances which have anticonvulsive activity. Regression lines showed good correlations between the quantity of drugs administered (total daily dose) and serum concentrations. The regression lines for diphenylhydantoin and primidone, however, showed no differences, irrespoective of whether they were being given alone or in combination. In view of the frequency of symptoms of intoxication and of non-responders, we established a therapeutic range for diphenylhydantoin and primidone (diphenylhydantoin: 5–16 mcg/ml; primidone: 4–14 mcg/ml). The required serum concentrations could be obtained by giving 8–12 mg/kg of diphenylhydantoin, and 15–22 mg/kg of primidone. In spite of the satisfactory correlation between total daily dose and serum concentrations, however, many patients showed departures from this normal behaviour, especially where combination treatments were being conducted. This demonstrates the necessity for routine controls of serum levels.

    Description / Table of Contents: Zusammenfassung Wenn sich auch bei der Behandlung mit antikonvulsiven Medikamenten eine gute Korrelation zwischen Gesamt-Tagesdosis und Serumkonzentrationen herstellen läßt, so weichen dennoch viele Patienten von diesem Verhalten ab. An Hand mehrerer Kinder, die unter Antikonvulsiva mit durchschnittlicher Dosierung standen und Überdosierungserscheinungen entwickelten oder ein Therapieversagen zeigten, sollen die verschiedenen Faktoren aufgezeigt werden, die die Serumkonzentrationen der verwendeten Medikamente zu beeinflussen vermögen. Vor allem ist bei gleichzeitiger Gabe mehrerer Medikamente eine Steigerung oder Hemmung des Metabolismus der verschiedenen Substanzen möglich, so daß zu geringe oder zu hohe Serumkonzentrationen resultieren. Bei ambulanter Behandlung muß andererseits auch immer mit einer ungenauen Einnahme der notwendigen Medikamente gerechnet werden. Dies sollte zur routinemäßigen Bestimmung der Serumspiegel antikonvulsiver Medikamente bei den Patienten führen.
    Notes: Abstract Although good correlation can be obtained between total daily dose and serum concentration in treatment with anticonvulsant drugs, many patients still show departures from this relation. The various factors which can influence serum concentrations of the administered drugs were to be demonstrated in a number of children who were receiving anticonvulsants at average dose levels and who developed evidence of overdose, or who failed to respond to therapy. The most important feature is that combined administration of several drugs may increase or inhibit metabolisation of the various substances, so that inadequate or excessively high serum concentrations results. Furthermore, irregular intake of the necessary medication must always be taken into account in the case of treatment on an out patient basis. Routine determinations of serum levels of anticonvulsant drugs in these patients are called for because of this.
