    Notes: Summary Epstein-Barr virus (EBV)-IgM antibodies could be detected in the sera of 36 children, most of whom had symptoms of Infectious Mononucleosis (IM). The detection of these antibodies indicates recent EBV infection. Heterophile antibodies could not be demonstrated in 17 of the 36 children. In young children, 〈 3 years, no heterophile antibodies could be detected at all. In two babies with EBV-IgM-antibodies bronchopneumonia and in a third baby a severe hepatosplenomegaly was diagnosed. In the serum of an 11-year-old boy with a Burkitt Lymphoma, EBV-IgM antibodies were present.

    Notes: Abstract In this double-blind, randomised, placebo-controlled cross-over study the respiratory effects of Mr 2264 Cl2 × 5 mg IV, a new partial opiate receptor agonist, were investigated and compared with the respiratory effects of morphine 2 × 10 mg IV and placebo. As primary end-points, the slope of the rebreathing curve (dV'/dPCO2ET) and V55 (ventilation at PCO2ET=55 mm Hg) were determined by Read's rebreathing method. The incidence of adverse events was also documented and compared. The respiratory depression after the intravenous administration of 5 mg and 10 mg Mr 2264 Cl was comparable to the decreased sensitivity of the respiratory centre after the 20 mg morphine IV. In contrast to morphine, a ceiling effect of Mr 2264 Cl was found. The tolerability of Mr 2264 Cl was comparable to that of morphine.

    Description / Table of Contents: Summary Hemorrhage after cardiopulmonary bypass (CPB) is associated with increased fibrinolytic and clotting activity. As pathogenic factors artificial (nonbiologic) surface contact and damage of corpuscular blood components are discussed. The aim of this study was to evaluate the impact of heparin-coated circuits on coagulation and fibrinolytic parameters, platelet count and postoperative blood loss in low-risk cardiac patients.¶ Fifty-two patients undergoing coronary artery bypass grafting (CABG) were randomly assigned to totally heparin coated (BiolineTM, Jostra, Hirrlingen, Germany) or identical uncoated extracorporeal circuits in a double blind protocol. Initial heparin dosage was equal for coated and uncoated circuits. Systemic anticoagulation was maintained with heparin in both groups.¶ Coagulation and fibrinolytic parameters showed a parallel perioperative course. Postoperative platelet count was significantly higher with heparin coating (152000/μl±46000 vs. 125000/μl±44000, p=0.029). Blood loss was lower with heparin coating but did not reach statistical significance. Heparin consumption was not reduced in heparin-coated circuits.¶ Heparin coating of cardiopulmonary bypass circuits does not improve hemostatic disorders in low risk CABG patients. Higher postoperative platelet counts suggest clinical benefit for high-risk patients with extended duration of bypass. Our data do not support reducing systemic heparin doses with heparin-coated circuits.
    Notes: Zusammenfassung Einer verstärkten Blutungsneigung nach Herzoperationen unter Einsatz der extrakorporalen Zirkulation (EKZ) können verschiedene Ursachen zugrundeliegen. Diskutiert werden u.a. Gerinnungsaktivierung und erhöhte fibrinolytische Aktivität durch Fremdoberflächenkontakt sowie Schädigung korpuskulärer Blutbestandteile. In einer doppelblind randomisierten Studie an 52 Koronarbypasspatienten haben wir den Einfluß eines komplett heparinbeschichteten Schlauch- und Oxygenatorsystems auf Gerinnungs- und Fibrinolyseparameter, Thrombozytenzahl und -funktion, den postoperativen Blutverlust sowie den Substitutionsbedarf an Fremdblutprodukten untersucht. Ausgeschlossen wurden Patienten mit abnormen Parametern der plasmatischen Gerinnung oder nach Einnahme von Thrombozytenaggregationshemmern weniger als 7 Tage präoperativ. 18 Patienten wurden mit heparinbeschichteten Systemen (Bioline®, Fa. Jostra, Hirrlingen, Deutschland) operiert, 34 Patienten dienten als Kontrollgruppe (identes, unbeschichtetes System). Zur systemischen Antikoagulation wurden initial die gleichen Dosen von Heparin verabreicht, nach Maßgabe der „activated clotting time“ (ACT) wurde bei Bedarf Heparin nachgegeben.¶ Die Parameter der Gerinnungsaktivierung und der Fibrinolyse zeigten in beiden Gruppen parallele Verläufe mit Maximalwerten am Bypassende bzw. frühpostoperativ. Heparinbeschichtete Systeme zeigten einen geringeren Abfall der Thrombozytenzahl postoperativ (152000/μl±46000 vs. 125000/μl ±44000, p=0,029). Die Thrombozytenfunktion zeigte in beiden Gruppen eine postoperative Einschränkung. Der Drainagenverlust zeigte eine Tendenz zugunsten heparinisierter Systeme ohne statistische Signifikanz, dieser Trend wirkte sich nicht auf den Verbrauch von Fremdblutprodukten aus. Der systemische Heparinbedarf war in beiden Gruppen gleich.¶ Bei Verwendung eines heparinbeschichteten EKZ-Systems bei elektiven Koronarbypasspatienten ließ sich keine klinisch relevante Minderung des pathogenen Einflusses der extrakorporalen Zirkulation auf das Gerinnungssystem nachweisen. Bei Verwendung heparinbeschichteter Systeme an Risikopatienten mit längerer Bypasszeit könnte der geringere Thrombozytenverbrauch in verminderter Blutungsneigung resultieren. Eine Reduzierung der systemischen Heparingabe ist nicht möglich.

    Notes: Abstract Adhesion molecule CD11b/CD18 expressed by neutrophils (PMNs) participates in cell migration and phagocytosis of C3bi derivatized bacteria. It is this phagocytic function that eliminates some of the known periodontal pathogens to periodontal pockets. In patients with advanced periodontitis, homotypic aggregation of crevicular fluid PMNs (CF-PMNs) may occur due to overexpression of CD11b/CD18 and this may lead to ineffective elimination of periodontal pathogens. We have previously shown that CF-PMNs isolated from the periodontal pockets overexpress CD11b compared to PB-PMNs. This study tested the hypotheses that (1) overexpression of surface CD11b correlates with expression of CD11b mRNA in CF-PMNs isolated from advanced periodontitis subjects, and (2) the intrinsic capacity of CD11b mRNA upregulation by PB-PMNs from periodontitis patients differs from that of control subjects. CF-PMNs and peripheral blood PMNs (PB-PMNs) were isolated from 13 subjects with healthy gingiva (control group) and 13 subjects with advanced periodontitis (patient group). The surface expression of CD11b was determined by flow cytometry and CD11b mRNA was determined by extraction of mRNA and reverse transcription to cDNA followed by DNA amplification using primers to detect a segment of the cDNA which encodes CD11b. The results of this study confirm that the surface expression of CD11b on CF-PMNs is significantly higher in periodontitis subjects vs control subjects (p=0.03), whereas surface CD11b expression on PB-PMNs does not differ significantly between groups (p=0.06). The level of surface CD11b expression on CF-PMNs did not correlate with the amount of mRNA present in CF-PMNs in either group (p=0.056, 0.07) for control and periodontitis patients, respectively). Most (9 of 13) individuals in the patient group expressed CD11b mRNA whereas very few control subjects (2 of 11) had CD11b mRNA in their CF-PMNs, This difference between groups was statistically significant (p=0.004). The capacity to upregulate CD11b mRNA upon stimulation with fMLP and/or GM-CSF was highly variable and there was no statistical difference between the 2 groups.

    Notes: Sand ripples, familiar from ocean floors, are studied by oscillating an annular sand–water cell. Empirical rules for the onset and disappearance of ripples are uncovered and compared with simple dimensional arguments, and with prior experimental and theoretical work. In addition the dynamical behavior of sand ripples are investigated, and the existence of a secondary instability where small and large wavelengths coexist in space is shown. Finally, by visualizing the fluid flow above the sand ripples the paper describes vortices which are an integral part of the ripple formation and show that there is a strong coupling between two different types of vortices and the sand ripple pattern itself. © 1999 American Institute of Physics.

    Notes: Two-step thermal annealing processes were investigated for electrical activation of magnesium- doped galliumnitride layers. The samples were studied by room-temperature Hall measurements and photoluminescence spectroscopy at 16 K. After an annealing process consisting of a short-term step at 960 °C followed by a 600 °C dwell step for 5 min a resistivity as low as 0.84 Ω cm is achieved for the activated sample, which improves the results achieved by standard annealing (800 °C for 10 min) by 25% in resistivity and 100% in free hole concentration. Photoluminescence shows a peak centered at 3.0 eV, which is typical for Mg-doped samples with high free hole concentrations.© 2001 American Institute of Physics.
