Zusammenfassung. In den letzten Jahren mehren sich Berichte über das Auftreten schwerwiegender Infektionen durch Streptokokken der Gruppe A, vor allem des „streptococcal toxic shock-like syndromes“ bei ansonsten gesunden Erwachsenen. Es ist gekennzeichnet durch Fieber, Hautausschlag, Hypotension und das frühzeitige Auftreten von Schock mit konsekutivem Multiorganversagen im Anschluß an Bagatellverletzungen. Anhand der Krankheitsverläufe zweier Patienten werden verschiedene Aspekte der Klinik, Pathophysiologie, Diagnostik und Therapie angesprochen. Nur durch sehr frühe, aggressive und evtl. wiederholte chirurgische Interventionen parallel zur antibiotischen Therapie kann die Menge der zirkulierenden Bakterientoxine wirksam reduziert und so die sehr hohe Letalität dieser Erkrankung gesenkt werden.

Summary. In recent years there has been an increasing incidence of streptococcal toxic shock-like syndrome (TSLS) in otherwise healthy adults. It is characterized by fever, rash, hypotension and early shock with consecutive organ failure and a mortality rate of about 30 %. This paper describes the history of two patients with severe streptococcal TSLS. Various aspects of the pathophysiology, diagnosis and therapy are discussed. Only early, aggressive and repeated surgical debridement can reduce the bacterial load and decrease circulating exotoxins and thereby, in combination with antibiotic therapy, decrease the high mortality of this entity.
