    Notes: Summary Immunoelectrophoresis of glomerular basement membrane antigens in the urine of 20 Type 1 (insulin-dependent) diabetic and 10 healthy children was performed. In 10 of the diabetic children, there was altered α-1-mobility, while the other diabetic and normal children showed α-2-mobility. After incubation with glucose, glomerular basement membrane antigens in the urine of healthy children showed α-1-mobility. Isolated human kidney glomerular basement membrane split products obtained by proteolytic degradation (papain, trypsin, chymotrypsin) were also investigated by immunoelectrophoresis. A difference was observed in the immunoelectrophoretic pattern of native and glycosylated glomerular basement membrane split products. A distinct increase of thiobarbituric acid assay positive glomerular basement membrane structures after incubation with glucose provides suggestive evidence for the occurrence of non-enzymatic glycosylation of glomerular basement membrane proteins. Glycosylated glomerular basement membrane proteins may contribute to both functional and morphological changes in diabetic glomerulosclerosis.

    Notes: Abstract Successful allogeneic bone marrow transplantation (BMT) for hematologic disorders may be complicated by graft-versus-host-disease (GVHD). Chronic GVHD is a systemic disease, involving, among other organs, the skin, mouth, liver, and esophagus. Esophageal involvement results in mucosal inflammation, leading to submucosal fibrosis and, occasionally, formation of webs and strictures. We investigated 25 allogeneic BMT recipients (17 with and eight without chronic GVHD). All patients had a videofluoroscopic study of the pharynx and esophagus to determine the radiographic abnormalities characteristic of chronic GVHD. Oropharyngeal abnormalities (poor bolus control, pharyngeal retention, or excessive mucous secretions) were found in five patients with and three patients without GVHD. Only one patient with GVHD had a pharyngo-esophageal stricture. There was no significant difference between the two groups with regard to pharyngo-esophageal radiographic abnormalities and esophageal symptoms. Radiographic evidence of esophageal motility disorder is not specific for GVHD involvement. In the absence of specific radiographic features, endoscopy is the most accurate method for the diagnosis of esophageal involvement by GVHD.

    Notes: Abstract The glycaemic response to commonly ingested breakfast with different glycaemic index (cornflakes, white bread, dark rye bread and muesli) was investigated in 14 children with insulin-dependent diabetes mellitus. The blood glucose response after cornflakes was significantly higher than after the other breakfasts, whereas the areas under the blood glucose curve were not statistically different. We observed no difference in blood glucose rise between the other breakfast types. Conclusion Postprandial blood glucose measurements after meals with high glycaemic index may be useful in order to minimize the postprandial rise in blood glucose.

    Notes: Abstract We compared the results of a computerized attention test (TOVA) in 38 children with insulin dependent diabetes mellitus in relation to various spontaneously occurring blood glucose levels. Testing was performed at the following blood glucose levels: 〈3.3 mmol/l (hypoglycaemia), 3.3–8.3 mmol/l (normoglycaemia) and 〉8.3 mmol/l (hyperglycaemia) . The attention (sum of errors and response time) varied significantly with the blood glucose level (P=0.002). The highest number of errors of omission and the longest response time was observed during the test run with hypoglycaemia. Age, sex, age at manifestation of the disease, metabolic control and the results of the intelligence test had no significant influence on these results. We found that attention in children with diabetes was significantly reduced compared to TOVA norms especially during mild hypoglycaemia (P〈0.001). Irrespective of the blood glucose levels, reaction time and the variability of the reaction time differed significantly between TOVA norms and diabetic children (P〈0.01). Conclusion In children with diabetes mellitus a significant reduction in attention was found at mild hypoglycaemia but as well at low normal blood glucose levels. Attention deficits due to transient lowering of blood glucose may therefore occur in diabetic children even before they are aware of hypoglycaemic symptoms.

    Notes: Abstract Acute suppurative thyroiditis is a rare disease, particularly in childhood. We present a case with recurrent acute suppurative thyroiditis due to a pyriform sinus fistula originating from the fourth branchial pouch. The typical symptoms of a piriform sinus fistula are recurrent left-sided pain and swelling of the neck with signs of acute bacterial inflammation. Diagnosis should be made by high resolution ultrasound, barium meal studies and endoscopic examination. During acute exacerbations treatment with antibiotics is indicated, but permanent cure can only be attained by complete fistulectomy.
