    Notes: Abstract In 78 male Wistar rats, the gracilis anterior muscles were devascularized and transplanted into the femoral region. In one experimental group, the muscles were transplanted with their innervation intact, in a second group denervated muscles were transplanted. The ipsilateral saphenous artery and vein were dissected out of their adventitia and implanted axially into the muscle graft. The entire transplant was encased in silicone foil in order to prevent neovascularization and reinnervation emanating from the host area. Neovascularization of the ischaemic innervated and denervated muscle grafts, containing approximately 300 muscle fibers, followed in a centrifugal pattern. The newly formed sinusoidal capillary network originated from the vein. Within the first four days, endothelial cells of the implanted vein had loosened their intercellular contacts, proliferated and migrated through the vascular wall into the muscle graft. The initially solid capillary sprouts opened up, starting at the lumen of the vein. Within two weeks a complete endomysial capillary network was reestablished. Possible and conceivable clinical uses of these findings are discussed.

    Description / Table of Contents: Zusammenfassung Diese Untersuchungen hatten das Ziel, die Gewebereaktion nach Laser-Synovektomie mit einem Holmium:YAG-Laser (Wellenlänge: 2,1 μm) an Kaninchen zu untersuchen und mit jener nach konventionell-mechanischer Therapie zu vergleichen. Bei 48 Kaninchen wurde immunologisch eine chronische Synoviitis in einem Kniegelenk induziert. Zwölf Tiere dienten zur Modellkontrolle. Jeweils 12 weitere Tiere wurden einer Scheinoperation, der Laserbehandlung oder der konventionell-mechanischen Abtragung unterzogen. Bei den Lasereingriffen wurden Pulsenergien vin ca. 600 mJ bei einer Pulsdauer vin 1 ms und einer Wiederholrate von 3 Hz appliziert. Nach Laufzeiten vin einem Tag, einer Woche, einem Monat und 3 Monaten wurden jeweils 3 Tiere jeder Gruppe getötet und die Synovialis makroskopisch, licht- und elektronenmikroskopisch untersucht. Unmittelbar nach der Laserbehandlung entstanden Koagulationsnekrosen, Entzündungen und Ödeme. Nach einer Woche zeigte sich im Stratum synoviale eine der Modellkontrollgruppe vergleichbare leichte Fibrose und nach einem Monat erschien die Oberfläche der Synovialis glatt. Nach der konventionell-mechanischen Abtragung traten Blutungen und Nekrosen auf. Eine vorbestehende Fibrose verstärkte sich in allen Kapselschichten. Nach 3 Monaten erschien die Oberfläche der Synovialis plumpzottig. Aufgrund dieser Ergebnisse könnte die Holmiumlaser-Synovektomie eine Alternative zu bestehenden Verfahren bei der operativen Behandlung der rheumatoiden Arthritis darstellen, gleichzeitig ware bei arthroskopischem Einsatz eine Fibrosierung der Gelenkkapsel durch die mechanische Irritation vermeidbar.
    Notes: Abstract The goal of this study was to evaluate tissue reactions in rabbit knees following laser synovectomy using a holmium:YAG laser (wavelength: 2.1 μm) and to compare these results with those found after conventional mechanical abrasion treatment. Chronic arthritis was immunologically induced in one knee joint each of 48 rabbits. Twelve served as controls, 12 were sham-operated, 12 were exposed to laser radiation, and 12 others were treated according to conventional methods. In the laser group, a pulse length of 1 ms at a repetition rate of 3 Hz resulted in a pulse energy of 600 mJ. After periods of 1 day, 1 week and 1 and 3 months, respectively, three animals from each group were sacrificed and the synovialis was examined macroscopically and histologically using light and electron microscopy. Edema, acute inflammation, and coagulation necrosis occurred immediately following laser therapy. After 1 week, the synovial layer showed slight fibrosis, which was comparable to that found in the controls. One month later the surface appeared to be smooth. The mechanical abrasion caused hemorrhage and necrosis. Fibrosis was pronounced in all capsular layers, and after 3 months the surface appeared coarse and villous in this group. Based on these preliminary findings, holmium-laser synovectomy may offer an alternative to existing therapeutic techniques in the treatment of rheumatoid arthritis. The arthroscopic approach could prevent additional capsular fibrosis associated with mechanical irritation.

Nach 4 Tagen waren in den gefässnahen Regionen des Muskels kleinkalibrige Gefässe anzutreffen, nach 3 Wochen war der Muskel bis zur bindegewebigen Kapsel unterhalb der Gore-Tex Surgical Membrane-Ummantelung perfundiert. Der Revascularisierung ging eine Invasion motiler Zellen voraus, die fischzugähnlich das Muskeltransplantat besetzten. Die Gefässsprosse entstammten vorwiegend der V. femoralis, Narbenbildung im Transplantat konnte nach 4 Wochen nicht beobachtet werden.
    Notes: Summary Implantation of the femoral artery and vein into the free-transplanted, isolated, medial capita of the gastrocnemius muscle in rats led to revascularization of this part of the muscle. After 4 days, in the muscle part lying near the implanted vessels, small vessels were seen that came from the femoralis vein. After 3 weeks, the whole free graft was revascularized up to the surrounding tissue beneath the Gore-Tex surgical membrane. An invasion of motile cells preceded the revascularization; the cells occupied the peri- and endomysium of the graft. After 4 weeks no muscle scars could be observed in the transplant.

Tusche- und Ausgußpräparate zeigten im Licht- bzw. Rasterelektronenmikroskop ein Gefäßnetz, das die Corticalis vollständig durchdrang. An Semidünnschnitten wurden in den Gefäßkanälen der revascularisierten Corticalis Osteoblasten und Osteoklasten sichtbar, während im Kontrollpräparat ohne Gefäßimplantation nur noch die Grundsubstanz zur Darstellung kam. Die revascularisierten Knochen enthielten keine Nekrosezonen. Das neugebildete Gefäßgeflecht bestand aus Arterien und Venen und deren Anastomosen.
    Notes: Summary Microsurgical implantation of saphenous vascular bundles into free autogeneous, isolated rib grafts in beagles resulted in revascularization of the bone after 2, 3, and 4 weeks. The newly formed vessels were demonstrated by injection of India ink, and in corrosion casts. Semithin sections revealed osteoblasts and osteoclasts in the vascular channels of the cortex, whereas in control rib grafts without implantation of blood vessels, only the matrix was observed. We saw no necrotic zones in the revascularized bones. The newly formed vascular networks consisted of arterial and venous vessels and their anastomoses.

Indian ink and Mercox-injected specimens showed a vascular network which penetrated the entire cortical layer. Dupuytren ist eine chronisch progrediente Fibromatose der Palmaraponeurose und der Digitalfascien unbekannter Genese mit einer Morbidität von 1–3%. Der beste Zeitpunkt für die Operation ist gegeben, wenn die Summe der Streckdefizite an einem Finger 20–30° nicht überschreitet. Die Methode der Wahl ist die komplette Entfernung der Palmaraponeurose und der erkrankten Digitalfascien. Bei fortgeschrittenen Kontrakturen an den Fingern sind evtl. Z-Plastiken, freie Hauttransplantationen and Eingriffe an Bändern and Gelenkkapseln erforderlich. Ein Modell der Dokumentation zur Qualitätssicherung wird vorgestellt.
    Notes: Summary Dupuytren's contracture is a chronic and progressive fibromatosis of the palmar aponeurosis and digital fascia. Morbidity is about 1%-3% and the cause is not yet known. The best time for prophylactic surgery is when the degree of deformity has reached a finger flexion of 20°–30°. Complete fasciectomy and excision of the involved digital fascia is the best method. If there is severe extension of contracture, Z-plasties, full-thickness grafts, or operation at the articular capsules or ligaments may be necessary. A documentation model and control of the surgical success are demonstrated.

    Notes: Abstract Spectroscopic results obtained on multiply charged few electron ions by using laser stimulated two-step recombination in ion storage rings are presented in an overview. Line splittings in transitions between Rydberg levels were observed for helium like and beryllium like ions. Emphasis is given to results on the beryllium like system N3+ where information about the splitting between high angular momentum levels (l⩾4) was obtained for the Rydberg levels n=13 and 8 with experimental errors of about ±0.02 meV.

    Notes: Abstract Recombination of highly charged ions with free electrons is studied in merged-beams experiments at the UNILAC accelerator in Darmstadt and at the heavy-ion storage ring TSR in Heidelberg. Unexpected high recombination rates are observed for a number of ions at very low energiesE cm in the electron-ion center-of-mass frame. In particular, theoretical estimates for radiative recombination are dramatically exceeded by the experimental recombination rates of U28+ ions nearE cm=0 eV. The observations point to a general phenomenon in electron ion recombination depending onE cm, on the ion charge state, and possibly also on electron density, electron beam temperature, and strength of external magnetic fields.
