    Description / Table of Contents: Zusammenfassung 62 Patienten (14 Knaben, 48 Mädchen), bei denen an insgesamt 85 Niereneinheiten ein vesico-renaler Reflux (Grad 2–4) erfolgreich operiert worden war, wurden im Durchschnitt 9,3 Jahre postoperativ auf das Auftreten von Harnwegsinfektionen (HWI) und von neuen Nierenparenchymnarben (NN) verfolgt. Bei den männlichen Patienten traten postoperativ bis auf einen Knaben keine HWI bzw. NN auf. Ähnliches galt auch für etwa 45% der Mädchen. Beide Gruppen von Patienten wiesen vor Operation hohe Refluxgrade auf. Bei den übrigen Mädchen, deren präoperative Reflux-Grade im Durchschnitt relativ niedrig lagen, traten jedoch postoperativ über Jahre noch HWI auf. NN entwickelten sich vorwiegend in dieser Zeit. Untersuchungen der Fähigkeit von Uroepithelzellen (UEZ), das Bakterienwachstum zu hemmen, zeigten, daß die UEZ dieser Patienten das Bakterienwachstum nicht supprimieren konnten. Somit entsprachen diese Mädchen bezüglich ihrer UEZ-Funktion Patientinnen mit asymptomatischer Bakteriurie. HWI und NN bei Reflux scheinen somit durch unterschiedliche Ursachen ausgelöst zu werden.
    Notes: Summary 62 patients (14 boys, 48 girls) representing 85 refluxive renal units (Grade 2–4) were investigated after successful operation for the development of further urinary tract infections (UTI) and renal scars (RS). The mean follow-up was 9.3 years. With the exception of one boy, none of the male patients developed any UTI or new RS. A similar result was obtained for about 45% of the girls. These two groups of patients presented with high-grade reflux before surgery. The remaining female patients (about 55%), however, presenting with lower-grade reflux before surgical treatment, developed further UTI as well as new RS despite surgical correction of their reflux. Investigations on the capacity of uroepithelial cells (UEC) to suppress bacterial growth revealed a deficient antibacterial effect of UEC in these patients. Such an UEC defect has also been shown in patients with asymptomatic bacteriuria. In conclusion, different reasons seem to be responsible for recurrent UTI and the development of RS in patients with reflux.

    Description / Table of Contents: Zusammenfassung Für die Abklärung und Klassifizierung des vesiko-renalen Refluxes stehen eine Reihe diagnostischer Maßnahmen zur Verfügung, deren Aussagekraft nicht unumstritten ist. Aus diesen Gründen müssen therapeutische Entscheidungen bei der Refluxerkrankung häufig vom klinischen Verlauf abhängig gemacht werden. Durch die operative Refluxkorrektur kann mit hoher Zuverlässigkeit und geringer Komplikationsrate der vesiko-renale Reflux unabhängig vom präoperativ nachgewiesenen Refluxgrad beseitigt werden.
    Notes: Summary Detection and accurate grading of vesico-renal reflux is rendered difficult by the limited realiability of the different diagnostic procedures. Therapeutic decisions are therefore to a large extent influenced by the clinical situation. Properly performed antireflux surgery combines a high cure rate with a low complication rate.

    Description / Table of Contents: Zusammenfassung Zwei von drei Brüdern (13 und 16 Jahre alt) zeigten das klinische Vollbild der Ataxia teleangiectatica Louis-Bar. Der ältere der Knaben leidet zusätzlich an einer autoimmun-hämolytischen Anämie, die im Zusammenhang mit anderen Immunmangel-syndromen, nicht aber bei Ataxia teleangiectatica bislang bekannt ist. Eingehende immunologische Untersuchungen bei den Brüdern zeigten eine Einschränkung von Zahl und Funktion der T-Zellen. Die Zahl der B-Lymphocyten war normal, darunter fanden sich auch IgA-positive Zellen, obwohl Serum-IgA fehlte. Es erscheint möglich, daß dieses Phänomen durch eine Reifungsstörung mit fehlender Differenzierung in IgA-synthetisierende Plasmazellen zustande kommt.
    Notes: Abstract Two of three brothers with the classical signs of ataxia telangiectasia were investigated for their immunological disorders at the ages of 13 and 16 years, respectively. The elder brother also suffers from autoimmune hemolytic anemia, a complication which has not yet been described in the course of ataxia telangiectasia. Immunological investigations made in both brothers showed a reduction in the number and function of T lymphocytes. The number of B lymphocytes was normal, among which there were cells staining for IgA, although serum IgA was absent. It seems possible that this phenomenon is caused by a disturbance in the process of maturation of lymphoid cells with a lack of differentiation into IgA-synthesizing plasma cells.

    Notes: Abstract Bacterial attachment to uroepithelial cells (UEC) and the effect of UEC on bacterial growth was investigated in 15 healthy persons and 12 patients suffering from asymptomatic bacteriuria (ABU) with recurrent urinary tract infections (UTI). Desquamated UEC and mannose-resistant Escherichia coli were co-cultivated for up to 90 min. While no difference in bacterial adherence was observed between healthy controls and patients, 33.4% of the bacteria attached to normal UEC were found to be dead under microscopic evaluation (acridine orange staining), whereas no killing effect could be observed in patients' UEC 5 min after the onset of incubation. This phenomenon was confirmed by investigating the E. coli growth rate in the presence of UEC, measured by counting bacterial colony forming units (CFU) on agar plates. While E. coli showed exponential growth in RPMI medium, the addition of normal UEC suppressed bacterial growth (P〈0.01). UEC from patients with ABU, however, did not show this effect. It has been concluded that bacterial adhesion may initiate an epithelial defence function, present in healthy controls and lacking in ABU patients.

    Description / Table of Contents: Zusammenfassung An 40 Kindern mit akuten Leukämien und soliden Tumoren sowie 31 Kontrollpatienten wurde das Verhalten der cutan-allergischen Spätreaktion auf 2,4-Dinitrochlorbenzol (DNCB) und Tuberkulin untersucht. Dabei ergaben sich bei Verlaufsbeobachtungen der Hautreaktionsstärke signifikante Unterschiede zwischen den beiden Malignomgruppen und dem Kontrollkollektiv. Kinder mit Leukosen oder soliden Tumoren ließen sich weit weniger gut gegen DNCB sensibilisieren als Patienten ohne neoplastische Erkrankungen. Der negative Testausfall wird zum großen Teil durch die cystostatische bzw. Strahlentherapie erklärt. Da jedoch Leukosepatienten in der Remissionsphase trotz cytostatischer Therapie häufiger positive Cutanreaktionen zeigten als im akuten Schub, ferner bei Kindern mit neurogenen Geschwülsten signifikant weniger positive Hautreaktionen auszulösen waren als bei Kindern mit sarkomatösen Tumoren, müssen noch andere Faktoren Einfluß auf die immunologische Reaktionslage nehmen. Ein Plasmafaktor wird diskutiert.
    Notes: Abstract The delayed type of hypersensitivity on the skin was studied after application of 2,4-dinitrochlorbenzol (DNCB) or tuberculin to 40 children who had either acute leukemia or solid tumors. Thirty-one patients were used as a control group. There was a significant difference in the results between the neoplasm group and the control group. It proved more difficult to sensitize the leukemia and tumor groups with DNCB. The negative reaction is thought to be caused primarily by irradiation therapy or by cytostatic agents. However, on remission of leukemia a higher amount of positive skin reactions was observed in spite of the cytostatic therapy. Children suffering from primary neurogenic tumors also showed significantly less positive reactions than those who had sarcoma. Therefore other factors must exist that influence the immunologic reaction. A plasma factor is to be discussed in this study.

    Notes: Abstract Serum levels of IgE were studied in 30 children with minimal-change nephrotic syndrome and 32 children with mesangioproliferative glomerulonephritis during different stages of the disease and treatment. In addition, tissue obtained by renal biopsy was investigated by immunofluorescence histology; no deposits of IgE could be found. The serum IgE levels, however, were increased, particularly in patients with minimal-change nephrotic syndrome. It is concluded that IgE does not play a pathogenic role in the development of the renal disease, but that increased IgE levels are an indication of a disturbance of the immune system.

    Notes: Abstract Compensatory growth in 27 undiseased congenital solitary kidneys and in 31 solitary kidneys in patients with Wilms tumour was monitored in long-term follow-up studies by ultrasound volume biometry. In congenital solitary kidneys hypertrophy was not detectable at the time of birth. Parenchymal mass increase achieved 188% of the volume of a healthy kidney within at least 4 years of life and afterwards paralleled the physiological growth documented in healthy kidney pairs. Disease-free kidneys in Wilms tumour patients all developed a similar 180% volume augmentation within 2–4 years after nephrectomy, irrespective of the chosen mode of radiation and single or tripte chemotherapy. The age of the patient at onset of surgical and concomitant conservative therapy determined slight differences in kinetics but not in degree of compensatory growth.

    Notes: Abstract The effect of buccal epithelial cells (BEC) on bacterial growth was investigated in healthy subjects as well as in patients with recurrent urinary tract infections (UTI) and compared to the antibacterial capacity of uroepithelial cells (UEC) of the same individuals. Epithelial cells were obtained from the following groups: healthy female controls; females without further UTI after reflux operation; females with asymptomatic bacteriuria (ABU); females with further UTI despite successful reflux operation; and patients with meningomyelocele (MMC) and recurrent UTI due to significant residual urinary volume. Cocultivation of Escherichia coli with BEC as well as UEC from healthy females or patients with MMC resulted in significant suppression of bacterial growth. However, neither type of epithelial cell showed an antibacterial effect when they were obtained from patients with recurrent UTI in the absence of urological abnormalities (ABU patients; reflux-corrected patients with further UTI). From these results it is concluded that a generalised epithelial defence defect is one important pathogeneic factor for recurrent idiopathic UTI.

    Notes: Abstract In thirty-two patients with nephrotic syndrome (NS) phytohemagglutinin (PHA)-induced lymphocyte proliferation was studied at various stages of the disease. We demonstrated that lymphocyte transformation during acute exacerbation is markedly decreased, especially if cells are cultured in patient serum. During treatment with steroids PHA-stimulation improves. During full remission all patients showed their maximal lymphocyte stimulation. On the basis of these results and reports from the literature we postulate that patients with the NS have a T cell clone which inhibits the transformation capacity of the remaining lymphocytes through production of a heat stable serum factor. The same or a second factor produced by these lymphocytes could at the same time exert a toxic effect on the glomerular basement membrane.
