    Description / Table of Contents: Summary Aldosterone excretion rate and plasma renin activity were assessed in 4 aeromegalic patients. Aldosterone levels were normal in all instances while renin remained rather low and showed only a very slight increase when measured in the upright position. The possible causes of plasma renin activity suppression are discussed.
    Notes: Zusammenfassung Bei 4 Patienten mit Akromegalie wurden Bestimmungen der Aldosteronexkretionsrate und der Plasmareninaktivität durchgeführt. Bei allen Patienten lag das Aldosteron im Normbereich, während sich das Renin an der unteren Normgrenze bewegte und vor allem nach aktiver Orthostase eine subnormale Stimulation aufwies. Die möglichen Ursachen dieser Suppression der Plasmareninaktivität werden diskutiert.

    Description / Table of Contents: Summary In six anesthetized dogs after injection of 100 mg mefruside (Baycaron®) into the aorta above the renal arteries a statistically significant rise of plasma renin activity, natriuresis, kaliuresis, diuresis and the hematocrit could be observed.51Cr-EDTA-and125J-o-iodine hippurate clearances just as the serum values of sodium, potassium, calcium and magnesium showed no important alterations. Also the heart rate and the arterial blood pressure were in the normal range during the whole time of the experiments. It is suggested, that volume receptors, eventually in the region of the macula densa of the distal tubule, may be more important with regard to the regulation of renin secretion than baroreceptors or chemoreceptors.
    Notes: Zusammenfassung Bei 6 narkotisierten Hunden wurde nach einer Injektion von 100 mg Mefrusid (Baycaron®) in die Aorta oberhalb des Abganges der Nierenarterien ein statistisch signifikanter Anstieg der Plasmareninaktivität, der Natriurese, Kaliurese und Diurese sowie des Hämatokrits beobachtet. Die51Cr-EDTA-und125J-o-Jodhippursäureclearance wiesen ebenso wie die Serumwerte von Natrium, Kalium, Calcium und Magnesium keine wesentlichen Veränderungen auf. Auch die Herzfrequenz und der arterielle Blutdruck blieben während der gesamten Versuchsdauer im Normbereich. Es wird die Auffassung vertreten, daß bei der Regulation der Reninsekretion den Volumenreceptoren eine größere Bedeutung als Baro- oder Chemoreceptoren zuzukommen scheint.

    Notes: Summary Normal sexual development is the consequence of three sequential interrelated processes: establishment of genetic, gonadal and somatic sex. It is the terminal phase of sexual differentiation-the translation of gonadal into somatic sex, which is governed by the presence or absence of both testosterone and Müllerian-inhibiting hormone and of dihydrotestosterone, which is formed in its respective target tissues. Thus, despite a testis, somatic male sexual differentiation will proceed to a normal male phenotype only if all three hormones are synthesized and act during a critical period of uterine development. Many clinically distinct syndromes are the results of abnormalities in the synthesis or action of the above-mentioned hormones; these syndromes are described in detail. In contrast to male somatic differentiation, female somatic development is independent of these hormones.

    Description / Table of Contents: Zusammenfassung Bei 16 Patienten mit Ulcus ventriculi sive duodeni wurde die Wirkung einer Carbenoxolon-Therapie auf das Renin-Angiotensin-Aldosteron-System beobachtet. Die Plasmareninaktivität nach Ruhelage und aktiver Orthostase und die Aldosteronexkretionsrate zeigten einen statistisch signifikanten Abfall. Das Körpergewicht, der arterielle Druck und das Serumnatrium stiegen im Mittel deutlich an, während das Serumkalium und der Hämatokrit abfielen. Diese Veränderungen waren abgesehen von dem Körpergewicht und dem diastolischen Blutdruck ebenfalls statistisch signifikant. Bei 3 Patienten entwickelte sich unter der Behandlung eine Hypertonie und bei 5 Patienten eine Hypokaliämie. Drei weitere Patienten wiesen Ödeme an den Unterschenkeln auf. Bei einem Patienten manifestierten sich die Zeichen einer Herzinsuffizienz. Die Nebenwirkungen, die nach Absetzen des Carbenoxolons reversibel waren, werden auf eine mineralocorticoide Wirkung der Substanz zurückgeführt. Dieser Effekt zwingt zur Beachtung besonderer Kautelen bei der Durchführung einer Carbenoxolon-Therapie.
    Notes: Summary In 16 patients with gastric or duodenal ulcer the influence of carbenoxolone sodium therapy on the renin angiotensin aldosterone system was investigated. Plasma renin activity after recumbency and active orthostasis and aldosterone excretion rate showed a statistically significant decrease. Body weight, arterial blood pressure and serum sodium levels increased, while serum potassium and hematocrit levels decreased. Except for body weight and diastolic blood pressure, these alterations were also statistically significant. Hypertension developed in 3 patients and hypokalemia in five. Three other patients developed edemas of the lower leg. In one patient the signs of congestive heart failure appeared. These side effects, which were reversible after discontinuing the therapy were based on the mineralocorticoid-like action of carbenoxolone. This effect requires observance of more precautions during carbenoxolone sodium therapy.

    Notes: Summary The ability of various natural and synthetic steroids (some of which are widely used in clinical practice) to compete with dihydrotestosterone receptor binding in human genital skin fibroblasts was studied. Binding was assessed in fibroblast monolayers after incubation for 1 h at 37 °C with 2 nM 3H-dihydrotestosterone in the presence or absence of increasing concentrations of the steroid to be tested. Inhibition constants (Ki) were determined as the concentration of competitor-required for 50% inhibition of3H-dihydrotestosterone binding. In addition, relative binding activity (RBA) of each test compound was calculated. Each competitor was tested in at least two different cell strains. The concentrations of unlabeled methyltrienolone (a synthetic nonmetabolizable androgen) and dihydrotestosterone for 50% inhibition of3H-dihydrotestosterone binding were in the same order of magnitude, namely, 2 nM (2.2 respectively, 2.4 nM), whereas the affinity of testosterone was approximately one-fifth that of dihydrotestosterone. Other potent competitors for dihydrotestosterone binding were three progestins (norgestrel, gestoden, and medroxyprogesterone acetate) which have Ki values similar to testosterone. An order of magnitude lower Ki values (around 10−7 M) were found for the androgen 17α-propylmesterolone, the antiandrogen cyproterone acetate, and the progestin norethisterone acetate. Binding affinities of all other steroids to the androgen receptor were markedly lower and showed the following order of potency: estrogens (estradiol, ethinyl estradiol, diethylstilbestrol) 〉 glucocorticoids as well as aromatase inhibitors and potassium canrenoate. We conclude that (a) among the compounds tested some progestins are very potent in their ability to interact with human skin fibroblast receptors and thus may affect endogenous androgen action; (b) estrogens are relatively weak androgen receptor binders; and (c) this receptor assay in combination with pharmacokinetic and metabolic studies appears to be a useful screening test to evaluate the potency of various steroids for androgen and antiandrogen therapy.

    Description / Table of Contents: Zusammenfassung Bei 10 Patienten mit primärer und sekundärer Polycythämie wurden Bestimmungen von Erythropoetin, Renin, Aldosteron, Erythrocyten- und Plasmavolumen durchgeführt. Der Erythropoetingehalt im Serum war bei einem Patienten mit Polycythaemia vera, 2 Patienten mit Hypoxämie und einem Patienten mit metastasierendem Lebercarcinom deutlich erhöht. Die Plasmareninaktivität lag bei allen Patienten im Normbereich. Sie wies jedoch nach aktiver Orthostase und Gabe eines Saluretikums eine zum Teil exzessive Stimulation auf. Die Aldosteronexkretionsraten wurden bei 4 Patienten erniedrigt gefunden; die übrigen Werte waren normal. Die Erythrocytenvolumina waren insgesamt vermehrt, die Plasmavolumina bei 7 Patienten vermindert. Die Beziehungen zwischen den verschiedenen Parametern werden diskutiert. Dabei wird betont, daß Renin bzw. Aldosteron und der renale erythropoetische Faktor höchstwahrscheinlich verschiedenen Regulationsmechanismen unterliegen.
    Notes: Summary Erythropoietin, renin, aldosterone, red cell mass, and plasma volume were assessed in 10 patients with primary and secondary polycythemia. The serum erythropoietin content was clearly elevated in one case of polycythemia vera, two cases of hypoxemia, and one case of liver carcinoma. —Plasma renin activity was within the normal range in all patients, but it increased excessively after upright posture and administration of saluretics. — Aldosterone excretion rates were decreased in 4 patients; the other values were normal. — Red cell mass was increased in all, while plasma volume was reduced in 7 patients. — The relationships between the parameters studied are discussed. It is suggested that renin, respectively aldosterone and the renal erythropoietic factor can be stimulated or depressed by different regulatory mechanisms.
