    Description / Table of Contents: Zusammenfassung Bei 17 Patienten mit angeborenen Immunmangelkrankheiten wurden die T- und B-Lymphocyten quantitativ untersucht. Gleichzeitig wurden zur funktionellen Beurteilung der beiden Zellpopulationen die Antwort auf Stimulation mittels Phytohämagglutinin und die Serumkonzentration der Immunglobuline untersucht. In einer vorbereitenden Studie wurde die Zuverlässigkeit der verwendeten Methoden kontrolliert; dafür wurden die Zellmarker (Rosettenbildung mit Schafserythrocyten für T-Zellen, Membranimmunfluorescenz für B-Zellen) anhand von zwei unterschiedlichen Lymphoidzellpopulationen genauer definiert: als nahezu reine T-Zellen wurden frisch zubereitete Suspensionen aus Thymusbiopsien von drei jungen Kindern verwendet, als B-zellreiche Präparate die isolierten Lymphocyten von drei Patienten mit chronisch lymphatischer Leukämie. Zur Klassifizierung der angeborenen Immunmangelsyndrome wurden die Vorschläge eines Expertenkomitees der Weltgesundheitsorganisation befolgt [12, 13]: 2 Patienten mit Agammaglobulinämie, 5 mit variablem Immunmangel, 4 mit Ataxia telcangiectatica, 3 mit schwerem kombiniertem Immunmangel und 3 mit Wiskott-Aldrich-Syndrom konnten ausführlich untersucht werden (Ergebnisse s. Tabelle 2). Die kritische Beurteilung der Resultate trägt zum besseren Verständnis der normalen Differenzierung lymphoider Zellen und der zur Immunmangelkrankheit führenden Störungen bei (vgl. dazu Abb. 1).
    Notes: Summary The T- and B-lymphocytes of 17 patients with immunodeficiency diseases were determined quantitatively. Simultaneously, their functional behavior, i.e. the response to stimulation by phytohemagglutinin and the immunoglobulin levels were studied. In a preliminary study devised to check the accuracy of the methods used, the cell markers (rosette formation with sheep erythrocytes for T-cells and membrane immunofluorescence for B-cells) were defined by means of two different lymphoid cell populations: as nearly pure T-cells the fresh suspensions prepared from thymic biopsies of 3 young children were used, as B-cell rich preparations the isolated lymphocytes of 3 patients with chronic lymphatic leukemia. Immunodeficiency syndromes were classified according to the recommendations of a World Health Organization expert committee [12, 13]: 2 patients with agammaglobulinemia, 5 with variable immunodeficiency, 4 with ataxia teleangiectatica, 3 with severe combined immunodeficiency and 3 with Wiskott-Aldrich syndrome were extensively studied (Table 2). The results are relevant for the better understanding of normal differentiation and its disturbances leading to the different deficiency syndromes (cf. Fig. 1).

    Description / Table of Contents: Zusammenfassung Auf 14 permanenten lymphoblastoiden Zellinien und auf primären Lymphoblasten wurden mit einem gepoolten Serum von 4 ausgewählten Multipara und mit einem Kaninchenantiserum ein B-Zell-allo-antigen nachgewiesen. Dabei erwiesen sich beide Antiseren als spezifisch, wobei jedoch das heterologe Antiserum mit B-Zellinien von Mononucleosepatienten eine breitere Reaktion zeigte. Die primären Lymphoblasten waren durch den Nachweis von B-Zell-allo-antigen bei gleichzeitigem Fehlen der Membranimmunglobuline und der Fc-Rezeptoren gekennzeichnet. Untersucht wurde das Knochenmark von 8 Patienten mit leukämischem Schub, von 13 Patienten mit hämatologischer Teilremission und von 30 Patienten in Vollremission der akuten Lymphoblastenleukämie. Dabei wurde die Remissionsqualität durch diese immunologische Charakterisierung besser erfaßt als durch eine knoventionelle, cytologische Auswertung. Besonders groß war die Diskrepanz zwischen den beiden Auswertungsmethoden 4 Wochen nach Initialtherapie, wobei sich immunologisch noch immer ein erhöhter Blastenanteil nachweisen ließ, obwohl cytologisch regelmäßig bereits die Vollremission erzielt war.
    Notes: Summary B-Cell-allo-antigen was demonstrated on lymphoblasts of children with acute lymphoblastic leukemia and on 14 permanent growing lymphoblastoid cell-lines. Pooled sera of 4 selected multiparas and a rabbit antiserum were used as specific antisera. Antigen was more effectively detected with heterologous antibodies than with human antisera. Primary lymphoblasts were characterized by B-cell-allo-antigen and lack of membrane immunoglobulins and Fc-receptors on the same cell. The quality of remission, i.e. the early diagnosis of relapse was better surveyed by immunological characterization than by cytological assessment. The bone marrow was examined in 8 patients with initial leukemia, in 13 patients with partial remission and in 30 patients in complete hematological remission. A striking discrepancy between immunological and cytological examination was observed in the bone marrow 4 weeks after initial anti-leukemic chemotherapy. At that time an increased number of blast-typed cells could still be counted in the immunological evaluation, although the cytological evaluation revealed already a complete hematological remission.

Membrangebundene Immunglobuline (B-Zell-Marker) wurden dabei nie gefunden, unerwarteterweise auch nicht bei einem Jungen mit leukämisch transformiertem Lymphosarkom. Dagegen zeigten die Blasten bei einem anderen Patienten, der frühzeitig verstarb, in verschiedenen Zellkompartments wiederholt T-Zell-Eigenschaften (Rosettenbildung durch Schafserythrocyten).
    Notes: Abstract The blast cells of 8 patients with acute lymphatic leukemia were investigated for B- and T-cell properties. Membrane-bound immunoglobulins (B-cell marker) were not found, unexpectedly also not on the blasts of a boy with leukemic transformed lymphosarcoma. In contrast, in another patient, who died shortly after diagnosis, the blasts of different compartments repeatedly showed T-cell properties (spontaneous rosette formation with sheep red blood cells).

    Notes: Abstract Immune complexes (ICs) participate in the pathogenesis of various diseases and can be shown in 18% of all hospitalized patients (excluding those with infectious diseases) by means of a sensitive method such as the Raji-cell radioimmune assay. However, before this test can be applied to quantify disease activity in renal, connective tissue and neoplastic diseases, it must be recognized that febrile infections of the upper respiratory tract also induce ICs in 86% of all patients. The immune complexes containing microbial antigens can be reduced or removed by a single injection of human immunoglobulin. This is a simple method to distinguish between the immune complexes of different specifities. The resulting removal of some immune complexes may be the explanation for the claimed therapeutic effect of gammaglobulin injection in normogammaglobulinemic patients.

    Notes: Abstract Focal and generalized seizures occurred in 4 patients with acute lymphoblastic leukemia and non-Hodgkin-lymphoma. The etiology of the neurological complications could be established by cranial computerized tomography (CT): i.e., 1. localized metastasis with calcification and 2. acute intracerebral hemorrhage during induction therapy in two patients with malignant lymphomas; 3. diffuse cerebral infiltration with blast cells and 4. cerebral atrophy in two children with acute lymphoblastic leukemia who were in relapse. Accurate diagnosis of cerebral complications in hemoblastoses is essential for appropriate therapy and CT may lead to more effective treatment in patients with lymphoid malignancy and seizures.
