    Description / Table of Contents: Summary The influence of some substances used in combination therapy of Morbus Hodgkin — Vinblastine, phenylbutazone, prednisolone and procarbazine — on ATP-nucleosidtransferases of leukemic cells have been examinated. Procarbazine and prednisolone do not influence the reactions of nucleosidephosphorylation. In contrary phenylbutazone and especially vinblastine are diminishing the amounts of triphosphates in leukemic cells.
    Notes: Zusammenfassung Einige beim Morbus Hodgkin meist im Sinne einer Kombinationstherapie angewendeten Substanzen—Vinblastin, Phenylbutazon, Procarbazin und Prednisolon — wurden auf ihren Einfluß auf die ATP-Nucleosidphosphotransferasen von Leukosezellen geprüft. Procarbazin und Prednisolon beeinflussen die Phosphorylierungsreaktionen nicht. Hingegen nehmen unter Phenylbutazon und besonders unter Vinblastin die Mengen der gebildeten Triphosphate ab.

Die erreichten spezifischen Aktivitäten erlauben detaillierte Nucleotid- und Oligonucleotidanalysen der verschiedenartigen Ribonucleinsäurespezies in menschlichen Leukämiezellen.
    Notes: Summary Methods are presented which provide the preparation of highly labelled nuclear RNA from cells of the different forms of human leukaemia. An incubation medium is described that is based on Hepes buffer and on the use of exhaustively dialyzed fetal calf serum offering suitable conditions for the uptake of32P-orthophosphate into the RNA of leukaemic nuclei. The specific activities reached may allow more detailed nucleotide and oligonucleotide analyses of the various RNA species present in human leukaemic cells.

    Description / Table of Contents: Summary Phenol-soluble chromatin proteins which may be involved in gene control mechanisms have been isolated from citric acid nuclei of normal and leukaemic human leukocytes derived from freshly obtained venous blood. They were compared by one-dimensional gel electrophoresis and by comparative labelling with14C-leucine or32P-orthophosphate. In addition, the influence of intercalating agents such as adriamycin and daunomycin was studied. Between 20–35 individual proteins were found in the phenol-soluble nonhistone protein fraction of human leukocytes. They were numbered with increasing mobility on 10% polyacrylamide gels. Distinct differences were found between normal lymphocytes and normal granulocytes, with one specific protein (no. 26 a in lymphocytes; no. 25 in granulocytes) for both cell types. Interestingly, protein no. 25 was not present in CML myelocytes but in CML granulocytes.32P-and leucine labels were generally found decreased with increasing cell differentiation. Leukaemic lymphocytes differed from normal lymphocytes by increased protein concentrations in the high molecular weight region. In comparisons of AML and ALL cells, including blast cells of CML blast crisis, no constant differences nor any markers common to leukaemic cells as compared to normal cells were detectable. However, lymphosarcoma cells showed quantitiative and qualitative aberrations from the usual leukaemia pattern. Afterin vitro incubations of cells with adriamycin a selective decrease of an individual protein (no. 30) was noted. This protein may serve as a marker for the intercalation site.
    Notes: Zusammenfassung Phenol-lösliche Chromatinprotein wurden aus Zellkernen normaler und leukämischer menschlicher Leukozyten isoliert. Die Proteine, deren Bedeutung im Rahmen der Kontrolle des Transkriptionsvorganges diskutiert wird, wurden durch Analyse ihrer gelelektrophoretischen Verteilungsmuster und ihrer Markierung mit14C-Leuzin oder32P-Orthophosphat verglichen. Außerdem wurde der Einfluß interkalierender Agentien wie Adriamycin untersucht. Die Fraktion phenol-löslicher Nicht-Histon-Proteine des Chromatins menschlicher Leukozyten enthält mindestens 20–35 individuelle Proteine, welche mit zunehmender Laufgeschwindigkeit auf 10%-Polyacrylamidgelen numeriert wurden. Zwischen normalen Lymphozyten und normalen Granulozyten ergaben sich charakteristische Unterschiede mit jeweils einer spezifischen Proteinbande (Nr. 26a bei Lymphozyten; Nr. 25 bei Granulozyten) für beide Zelltypen. Interessanterweise war das Protein Nr. 25 in Myelozyten (CML) nicht vorhanden, jedoch in reifen Granulozyten desselben Patienten (CML) nachweisbar. Die radioaktiven Markierungen der Proteine mit32P (Phosphorilierung) bzw.14C-Leuzin (Umsatz) nahmen ganz allgemein mit zunehmender Zelldifferenzierung ab. Leukämische Lymphozyten unterschieden sich von normalen Lymphozyten durch erhöhte Protein-Konzentrationen im hochmolekularen Bereich. Beim Vergleich von Zellen aus AML, ALL und der CML-Blastenkrise ergaben sich bei weitgehend ähnlichen Mustern keine typischen Differenzen. Ebenso waren gemeinsame leukämiespezifische Abweichungen gegenüber Normalzellen nicht nachzuweisen. Lymphosarkomzellen zeigten allerdings quantitative und qualitative Abweichungen vom gewöhnlichen Verteilungsmuster der Zellkernproteine. Nach Einwirkung von Adriamycin in vitro kam es bei den verschiedensten Zellinien dosisabhängig zu einer selektiven Abnahme eines bestimmten Proteins (Nr. 30) dessen mögliche Funktion als „marker“ für den Interkalationslocus von Adriamycin an der DNA diskutiert wird.

    Description / Table of Contents: Zusammenfassung Seit Juli 1978 wurden 103 Patienten mit inoperablem kleinzelligem Bronchialkarzinom mit der Zytostatikakombination Adriamycin, Cyclophosphamid und Vincristin (ACO) behandelt. Im Stadium „limited disease“ (n=64) erfolgte während des zweiten Chemotherapiekurses eine prophylaktische Schädelbestrahlung, nach dem vierten eine konsolidierende thorakale Bestrahlung. Nach Erreichen einer kompletten Remission erhielten die Patienten prospektiv randomisiert Etoposid oder keine weitere spezifische Therapie. Ein objektives Ansprechen konnte bei 88/100 auswertbaren Patienten erzielt werden. Im Stadium „limited disease“ fanden sich 72%, im Stadium „extensive disease“ nur 33% komplette Remissionen. Im Stadium „limited disease“ betrug die hochgerechnete mediane Überlebenszeit 15,8, im Stadium „extensive disease“ 9,3 Monate (p〈0.005). Es leben noch 29 Patienten, 4 rezidivfrei länger als 24 Monate. Patienten mit kompletter Remission hatten eine statistisch signifikant (p〈0.001) längere Überlebenszeit als Patienten mit geringerem Ansprechen. Regelmäßig traten gastrointestinale und hämatologische Nebenwirkungen auf, drei Patienten starben während der Induktionsphase an Infektionen. Die kurzzeitige Induktionsbehandlung verbesserte jedoch den Krankheitsverlauf subjektiv und objektiv. Bisher ist kein positiver Effekt der zyklischen Etoposid-Gabe nach ACO festzustellen.
    Notes: Summary Since July 1978 one hundred and three consecutive patients with unresectable small cell bronchogenic carcinoma were treated with a combination of doxorubicin, cyclophosphamide and vincristine (ACO). In limited disease patients (64) the second chemotherapy course was followed by prophylactic cranial irradiation, the fourth by irradiation towards primary disease sites. Complete responders were randomised to either receive etoposide or no further maintenance therapy. Objective responses were reached in 88/100 evaluable patients, with 72% of complete remissions in limited-stage disease and 33% in extensive disease, respectively. The actuarial median survival time for limited disease patients was 15.8 months compared to 9.3 months in extensive disease (p〈0.005). 29 of the 100 patients remain still alive, 4 for more than 24 months without disease recurrence. The survival advantage of patients reaching complete remissions relative to those who did not is highly significant (p〈0.001). Acute gastrointestinal and hematological side effects were common, with possibly three drug-related deaths from infections during transient granulocytopenia (mean nadir: 600–900 cells/mm3). The present induction regimen using only four courses of chemotherapy produces high complete remission rates on roentgenography and bronchoscopy and improved survival in the majority of patients. Thus far any effectiveness of etoposide-maintenance therapy following ACO could not be substantiated.

    Notes: Summary The evolution of medical oncology so far owes much to the preclinical and clinical development of antineoplastic agents. Prognostic factors and empiric treatment strategies have guided the clinician in his choice of drugs. In the light of increasing ethical restrictions met with phase I–II clinical trials and major advances in propagating human tumor cells outside the donor patient, a reappraisal of predictive tests in cancer chemotherapy is warranted. Among ‘short-term assays’ only the determination of steroid-hormone receptor content in tumor tissues has gained clinical acceptance, whereas other methods still suffer from theoretical or practical shortcomings. Both the human tumor stem cell assay and the xenograft model have revealed unique patterns of sensitivity for each individual tumor line. While interindividual heterogeneity among tumors sharing a common site of origin justifies efforts to develop predictive tests, microheterogeneity among tumor samples from the same donor patient limits the potential of this approach. Predictive tests should be performed in conjunction with clinical trials to ensure optimal extraction of information. As additional prognostic factors, they should in the near future accelerate drug development and reduce the hazard of unnecessary drug toxicity without therapeutic benefit.

    Description / Table of Contents: Zusammenfassung Seit 1975 wurden 140 Patienten mit retroperitoneal-metastasierten nicht-seminomatösen Hodentumoren nach Orchiektomie und retroperitonealer Lymphadenektomie sequentiell alternierend mit den Zytostatika-Kombinationen Velbe/Bleomycin und Adriamycin/Cisplatin plus/minus Radiotherapie behandelt. Davon erhielten 68 Patienten nach totaler retroperitonealer Lymphadenektomie mit postoperativ normalisierten Tumormarkern (Stadium IIA) 6 Chemotherapie-Kurse, woran sich bei 35 Patienten eine Strahlentherapie anschloß. Vierzig Patienten wurden nach subtotaler retroperitonealer Lymphadenektomie oder bei postoperativ erhöhten Tumormarkern (Stadium IIB) und 32 Patienten nach palliativer Lymphadenektomie (Stadium IIC) mit mindestens 12 Chemotherapie-Kursen und fakultativer intermittierender Radiotherapie und/oder Relaparotomie behandelt. Der Vergleich der Behandlungsergebnisse bei den Stadien IIA und IIB ergab unabhängig von der zusätzlichen Radiotherapie nach der „Life-table“-Methode Vier-Jahres-Überlebensraten zwischen 80 und 100%. Diese günstigen Resultate sind mit den Ergebnissen bei 34 nicht adjuvant behandelten Patienten ohne histologisch nachweisbare retroperitoneale Metastasierung (Stadium I) vergleichbar. Ausdruck einer statistisch signifikant schlechteren Prognose bei fortgeschrittener retroperitonealer Metastasierung ist eine Vier-Jahres-Überlebensrate von 12% bei den Patienten im Stadium IIC.
    Notes: Summary Following orchiectomy and retroperitoneal lymph node dissection (RND) 140 patients with stage II non-seminomatous testicular cancer were treated by sequential combination chemotherapy consisting of vinblastine/bleomycin and adriamycin/cis-dichlorodiammineplatinum(II) (DDP), plus/minus radiotherapy. 68 stage IIA-patients (complete RND and normal tumor-markers thereafter) received 6 courses of chemotherapy, followed by radiotherapy in 35 patients. 40 stage IIB-patients (minor residual disease after RND or elevated tumor-markers after RND) and 32 stage IIC-patients (advanced residual disease after RND) were treated by at least 12 chemotherapy courses and optional intermittent radiotherapy and/or relaparotomy. In stage IIA and IIB disease the actuarial 4-year survival rates were between 80 and 100%. These favourable results were not significantly influenced by additional radiotherapy and corresponded to the survival rates for 34 stage I-patients. For stage IIC-patients the prognosis was significantly worse with a 12% 4-year survival rate.

    Notes: Summary Thirty-eight pretreated patients with primarily resistant [6] or relapsed [32] small-cell lung cancer were treated with a combination of vindesine (3–4 mg/m2) and cisplatin (60–100 mg/m2). Eight patients responded to this therapy with three (8%) complete and five (13%) partial remissions. Minor responses were noted in 12 (32%) additional patients. Chemotherapeutic response was rare in regions of prior irradiation. In the complete remission group survival from start of vindesine/cisplatin therapy lasted 61, 48 and 38 weeks, respectively. In the “less-than-complete-remission” group median survival was 12 weeks. Nausea and vomiting were the prominent side-effects, while only mild to moderate myelosuppression was noticed in most cases. The vindesine/cisplatin combination showed significant activity in heavily pretreated small-cell lung carcinoma. However, the remission rates remain low in this unfavourable condition, which might be due to pronounced chemotherapeutic resistance in previously irradiated areas.

    Description / Table of Contents: Zusammenfassung Aus Zellen des Burkitt-Lymphoms, eukämischer Lymphosarcomatose, chronisch-lymphatischer Leukämie, chronisch-myeloischer und akuter myeloischer Leukämie wurden nach sechsstündiger32P-Markierung in einem phosphatarmen Medium Zellkerne mit Hilfe des Zitronensäure-Verfahrens isoliert und die nucleare RNA mit der heißen Phenol-SDS-Methode extrahiert. Nach Fraktionierung der kernspezifischen Nucleinsäuren über Zucker-Dichtegradienten fanden sich markante Unterschiede in der32P-Radioaktivitätsverteilung. Insbesondere war eine differente Markierung der nuclearen 45S RNA, welche als Vorläufer ribosomaler 28S und 18S RNA gilt, festzustellen. Die niedrigsten spezifischen Aktivitäten des ribosomalen Vorläufers fanden sich bei der CML, die höchsten bei AML, leukämischem Lymphosarkom und Burkitt-Tumor. Bemerkenswert erscheint die aktive Synthese präribosomaler (45S/35S) RNA in Zellen der CLL, deren DNA-Syntheserate äußerst niedrig ist. Zur Klärung der Frage, ob die im Nucleolus der unreifen Zellen gebildeten hochmolekularen Nucleinsäuren quantitative oder qualitative Unterschiede zwischen myeloischen und lymphatischen Zellen aufweisen, sind strukturchemische Untersuchungen im Gange.
    Notes: Summary Nuclear RNA was isolated from citric acid nuclei derived from AML, CML, CLL, leukaemic lymphosarcoma and BURKITT lymphoma cells after 6 hours incubation with32P-orthophosphate in a phosphate-free medium. In fractionations on sucrose density gradients, marked differences were found in the distribution of the32P-radioactivity mainly in the 45S fraction containing the ribosomal precursor RNA. The lowest specific activities of nuclear 45S RNA were found in CML; very high labelling accurred in cells of AML, leukaemic lymphosarcoma and BURKITT lymphoma. In CLL cells which are known for lack in DNA synthesis, pre-ribosomal 45S and 35S RNA were labelled to a remarkable extent. Studies are in progress in order to define possible differences in nuclear RNA structures between lymphocytic and granulocytic cell lines.

    Description / Table of Contents: Zusammenfassung Die jüngsten molekularbiologischen Untersuchungen an Lymphocyten der chronisch-lymphatischen Leukämie bestätigen den neoplastischen Charakter dieser Zellen. Funktionelle und strukturelle, z.T. qualitative Unterschiede in der Biochemie von DNA, RNA und Proteinen belegen die biologischen Sonderstellung dieser Erkrankung, bei deren Pathogenese Oncornaviren nach den bisher gesicherten Befunden wahrscheinlich keine Bedeutung haben. Zellen der chronisch-lymphatischen Leukämie sind durch verschiedenste makromolekulare Abnormalitäten ausgezeichnet: multiple DNA-Polymerasen nach PHA-Stimulation, vermindertes Aktinomycin-Bindungsvermögen, veränderte Transcription des Chromatins, erhöhten DNA-repair, erhöhte RNA-Synthese, vermehrte Synthese ribosomaler Vorstufen bei mangelhafter Ribosomenreifung, veränderte32P-Nucleotidkomposition und veränderte Oligonucleotidfrequenzen in nuclearer 45S RNA und ribosomaler 28S RNA, mangelhafte Stimulierung des nucleolaren „processing“ durch PHA, verminderten Lysosomen- und Polysomengehalt, Funktionseinbuße der Ribosomen in zellfreien Systemen, neue Polyribonucleotidsequenzen in der nuclearen heterogenen RNA sowie durch qualitative und quantitative Veränderungen des Kernproteinmusters nach zweidimensionaler Gel-Elektrophorese. Die Interpretation des CLL-Lymphocyten als Bähnliche Zelle erscheint im Zusammenhang mit diesen neuen biochemischen Daten als nicht ausreichend.
    Notes: Summary Recent studies on the molecular biology of the lymphocyte of chronic lymphocytic leukemia confirm the neoplastic nature of these cells. Functional and structural differences in the biochemistry of DNA, RNA and proteins define the biological peculiarity of this disease in which oncorna viruses are probably of no pathogenetic importance. Cells of chronic lymphocytic leukemia are characterized by various macromolecular abnormalities: multiple DNA-polymerases after PHA-stimulation, decreased actinomycin binding capacity, altered transcription of the chromatin, elevated DNA-repair, increased RNA-synthesis, increased synthesis of ribosomal precursors combined with a failure in ribosomal maturation, different32P nucleotide composition and oligonucleotide frequencies of nuclear 45S RNA and ribosomal 28S RNA, impairment in stimulation of nucleolar “processing” by PHA, decreased lysosome and polysome content, functional impairment of ribosomes in cell-free systems, new hybridizable polyribonucleotide sequences in nuclear heterogeneous RNA as well as qualitative and quantitative differences in the nuclear proteins after two-dimensional mapping by gel electrophoresis. The interpretation of the CLL-lymphocyte as a B-like cell does not seem to be sufficient with respect to the new biochemical data.

    Description / Table of Contents: Zusammenfassung Prätherapeutische Sensibilitätsbestimmungen an menschlichen Tumoren auf der Basis von Nukleinsäurevolräufer-Inkorporationsstudien sind von verschiedenen Arbeitsgruppen beschrieben worden. Diese Übersicht will zeigen, weshalb solche Tests für eine dringend erforderliche individuelle Chemotherapieplanung vorerst nur von geringem Nutzen zu sein scheinen. Einer der wesentlichsten Gründe ist, daß es gegenwärtig für viele Zytostatika noch keinen Nachweis einer Korrelation zwischen biochemischer und klinischer Wirkung gibt. Darüber hinaus wird der Aussagewert von Sensibilitätsbestimmungen beeinträchtigt durch methodische Probleme bei der In-vitro-Dosierung und der Zellaufbereitung, die unterschiedliche Pharmakologie und Toxikologie der einzelnen Zytostatika sowie deren Wechselwirkungen bei der kombinierten Chemotherapie.
    Notes: Summary Praetherapeutic sensitivity tests of human solid tumours based on incorporation studies of nucleic acid precursors have been described by various authors. This review intends to show that such tests may only be of rather limited value for the design of individual chemotherapeutic regimens. In many drugs, a correlation between biochemical and clinical effects has not yet been established. The reliability of praetherapeutic sensitivity tests is further impaired by general methodological difficulties of short-term cultures, individual pharmacology and toxicology of cytostatic agents as well as pharmacological and biochemical drug interactions in combination chemotherapy. Praetherapeutic testing with individual cytostatic drugs is discussed.
