    Description / Table of Contents: Zusammenfassung 16 Fälle von teilweise dramatischen Unverträglichkeitsreationen nach Humanalbumininfusionen wurden untersucht. Die Reaktionen manifestierten sich als Fieberanstiege, Schüttelfrost, urticarielle Exantheme und Blutdruckschwankungen bis zum Schock. Sie waren durch Prednisolon günstig zu beeinflussen und klangen nach Absetzen der Humanalbumininfusionen restlos ab. Im zeitlichen Auftreten ließ sich eine Früh- von einer Spätreaktion unterscheiden, wobei die Konzentration des verwendeten Humanalbumins eine Rolle zu spielen schien. Frühreaktionen wurden bevorzugt durch 20%iges, Spätreaktionen immer durch 5%iges Humanalbumin hervorgerufen. Als auslösender Faktor konnten Verunreinigungen der verwendeten Infusionslösungen ausgeschlossen werden. Die Ergebnisse von Intrakutantestungen, Antigeneliminationstechnik, sowie der Nachweis von präzipitierenden Antikörpern gegen Makroaggregate des Albumin-Ultrazentrifugats sprechen dafür, daß eine immunologische Reaktion pathogenetisch von Bedeutung sein könnte. Möglicherweise läßt sich durch Applikation eines desaggregierten Humanalbumins das Risiko von Unverträglichkeitsreaktionen auch bei Gabe großer Humanalbuminmengen an allergisch disponierte Patienten verringern.
    Notes: Summary 16 cases of clinical incompatibility reactions due to human serum albumin (HSA) were recorded. The reactions consisted in increases in body temperature, sometimes together with shivering, urticarial exanthema and blood pressure changes as far as shock. They responded well to prednisolone application and disappeared completely after discontinuing the albumin-therapy. Regarding the onset in time, “early” (1 hour) and “late” (3 days) reactions could be distinguished; early reactions were mostly caused by 20%, late reactions by 5% albumin solutions. Pollutions of the administered HSA-batches could be excluded as an etiologic factor. The results of skin tests, antigen-elimination technique as well as the demonstration of precipitating antibodies directed against the concentrated macroaggregates of the albumin ultracentrifugation sediment suggest a pathogenetic influence of immunological reactions. Ultracentrifugation of the albumin prior to the administration might possibly reduce the risk of incompatibility reactions after high dose application in hyperreactive patients.

    Description / Table of Contents: Zusammenfassung Bei 127 mit ALG vom Pferd behandelten Patienten wurden die möglichen hämatologischen und klinischen Nebenwirkungen erfaßt, ihre prophylaktische Verhinderung und therapeutische Beherrschung geschildert. Durch Anwendung eines exakten Therapieschemas in Kombination mit Azathioprine und Corticosteroiden nach sorgfältiger Patientenauswahl und versuchter Erzeugung einer immunologischen Toleranz gegen Pferde-IgG, ließen sich Häufigkeit und Intensität von Nebenreaktionen signifikant verringern. Dies wird im Vergleich zweier Patientengruppen gezeigt. Die für die Auslösung der Nebenreaktionen verantwortlichen pathogenen Faktoren werden erörtert. Da prinzipiell unvermeidbare, lebensbedrohende Nebenwirkungen bisher nicht aufgetreten sind, wird die Frage nach der ärztlichen Verantwortbarkeit einer ALG-Therapie bejaht, jedoch sorgfältige klinische, klinisch-chemische und immunologische Überwachung gefordert.
    Notes: Summary In 127 patients treated with horse anti-human lymphocytic globulin (ALG) the possible haematological and clinical side effects, their prevention and treatment were recorded. Frequency and intensity of side reactions can be reduced significantly by the application of a strict therapy schedule in combination with azathioprine and corticosteroids, careful selection of patients and induction of immunological tolerance against horse-IgG. This is shown when comparing the two groups of patients. The pathogenic factors of the side-reactions are discussed. Because principally unavoidable, life-threatening side effects have not been observed, the answer to the question of the ethical justification of ALG therapy is positive. Careful clinical, biochemical and immunological controls are postulated.

    Notes: Summary A multicenter randomized controlled clinical trial, which was carried out in 10 hospitals in the Federal Republic of Germany between 1979 and 1983, studied the influence of i.v. immunoglobulin G on the mortality in patients with diffuse acute fibrinopurulent peritonitis. Altogether 288 patients were enrolled in the trial. There was no statistically significant difference in the mortality rates within the treated group (46%) vs the control group (41%). The power of the statistical test to detect a decrease of the mortality by 20% was calculated to be 0.93. This result did not change when we eliminated 50 patients not strictly obeying the entrance criteria of the analysis, or when we focused on a subgroup of patients with initial deficiency of immunoglobulin G. Factors influencing mortality were a preceding laparotomy, serum creatinine level above 2 mg/100 ml, and necessity for artificial respiration. These factors, reflecting the surgical situation and the severity of shock, essentially explain the mortality differences observed between the participating hospitals.

    Description / Table of Contents: Summary As a trial to induce immunological unresponsiveness to horse-IgG, 6 patients received two intravenous injections of the ultracentrifuged serum protein antigen in doses of 10 mg/kg and 30 mg/kg body weight respectively prior to the ALG-therapy. During and after ALG-treatment no antibodies against horse-IgG could be detected in the serum of the ALG-treated patients using the passive hemagglutination technique. The antigen-elimination tests, carried out after the offset of ALG, showed a typical non-immune elimination of the131J-labelled horse-IgG from the circulation. These findings suggest the possible induction of an immunological tolerance to horse-IgG, which may prevent foreign serum reactions in patients receiving anti-lymphocyte globulin.
    Notes: Zusammenfassung Als Versuch zur Erzeugung einer spezifischen immunologischen Toleranz gegen Pferdeimmunglobulin G (PIgG) erhielten 6 Patienten vor der geplanten ALG-Therapie 2 intravenöse Injektionen des ultrazentrifugierten Serumproteinantigens in einer Dosierung von 10 mg/kg bzw. 30 mg/kg Körpergewicht. Während und nach Absetzen der ALG-Behandlung konnten mit der passiven Hämagglutinationsmethode keine zirkulierenden Antikörper gegen PIgG im Serum der behandelten Patienten nachgewiesen werden. Der nach Beendigung der ALG-Behandlung durchgeführte Antigen-Eliminationstest zeigte, daß intravenös injiziertes131J-markiertes PIgG in allen Fällen im Sinne eines Nichtimmunabbaus aus der Blutbahn eliminiert wurde. Diese Befunde deuten darauf hin, daß möglicherweise der Zustand einer immunologischen Toleranz gegen PIgG induziert wurde, der die Patienten vor anaphylaktischen Reaktionen oder Erscheinungen einer Serumkrankheit als Folge der Fremdserumbehandlung zu schützen vermag.

    Notes: Summary A fatal case of overwhelming postsplenectomy pneumococcal sepsis is presented occurring in a 37-year-old female 11 years after removal of the spleen because of traumatic rupture. The patient died 11 h after admission to hospital and about 32 h after sudden onset of illness. At necropsy splenic tissue, splenosis, disseminated intravascular coagulation, and thrombi within the arterioles consisting of gram-positive cocci and adrenal hemorrhage were found. The clinical, laboratory, and postmortem findings are described. Reports had been published of 41 other cases of overwhelming postsplenectomy infection (OPSI) in patients aged 20 years ore more, but only three of these cases of OPSI syndrome occurred in spite of remaining splenic tissue. The longest interval between extirpation of spleen and subsequent sepsis was 42 years, indicating a small but lifelong risk of severe infection in asplenic patients. In view of the literature, the role of spleen in infection defence, the splenic function in blood clearance, and the prevention of postsplenectomy infections by antibiotical prophylaxis, pneumococcal vaccine, and reimplantation of autochthonous splenic tissue or infrared contact coagulation are discussed.

    Notes: Abstract A single dose of clofibrate (400 mg/kg), given to rats, increased the incorporation of (3H)thymidine into liver DNA, in a period of 20–30 h after administration. However, (3H)thymidine incorporation into hepatic DNA of rats treated repeatedly was identical to that of control animals. After the administration of a single dose of clofibrate a small increase in (3H)thymidine incorporation also occurred in kidney DNA; repeated doses, however, resulted in a marked suppression of labeling.

    Notes: Abstract Blood volume was estimated using 51chromium labelled red cells and 125iodinated human serum albumin in 5 children with sepsis, in 6 burned children and 7 children with acute lymphoblastic leukaemia. Studies of the equilibration pattern demonstrated that the mixing time of labelled red cells was prolonged to 40 minutes or more in 5 children, indicating the existence of slowly circulating red cells. Mixing of labelled albumin was complete within 10 minutes in 15 patients and within 20 minutes in all the children studied. In a burned patient with severe sepsis, exchange transfusion improved the clinical state and normalized the equilibration pattern of labelled red cells. The mean body/venous haematocrit ratio was 0.893±0.018 (SD) in the children with sepsis, 0.859±0.052 in the burned patients, and 0.916±0.078 in the children with acute lymphoblastic leukaemia, increasing with spleen size in the latter group.
