    Notes: Summary We recorded spinal cord evoked potentials (SCEPs) and spinal somatosensory evoked potentials (spinal SEPs) in 30 operations following stimulation of the epidural spinal cord and the peripheral nerve, respectively, to compare their feasibility as an intraoperative technique for spinal cord monitoring. SCEPs produced quicker responses and had larger amplitudes with simpler waveforms. SCEPs could reflect residual function of the pathological spinal cord and predict the postoperative clinical outcome, findings which are not observed with spinal SEPs. Moreover, SCEPs had a much higher sensitivity to spinal cord insult. Therefore, we conclude that the SCEPs were more appropriate indicator than the spinal SEPs as an intra-operative monitoring method for spinal cord function.

    Notes: Abstract Aminopeptidase activity was demonstrated inMycoplasma salivarium (ATCC 23064) cells disrupted by sonic vibrations and lyophilized (crude enzymes), and weak endopeptidase or carboxypeptidase activity was also suggested. The crude enzymes were suspended in 0.1 M borate buffer, pH 8.0, containing 0.5% (w/v) sodium deoxycholate, and then the suspensions were centrifuged at 100,000g for 2 h. Thus separated, the supernatants were applied to a column of Sephacryl S-300. As a result, aminopeptidase activity was separated from caseinolytic activity, which had already been demonstrated in this organism. The aminopeptidase activity was inhibited byo-phenanthroline and stimulated by Mn2+, and the enzyme exhibited a strong affinity for leucine and arginine. On the other hand, the caseinolytic activity was inhibited considerably by o-phenanthroline and Ni2+ and slightly by diisopropyl fluorophosphate and Co2+. The caseinolytic activity was therefore believed to be due mainly to metalloproteinases and partly to serine proteinases.

    Notes: Abstract Objective: To evaluate the role of surfactant in the mechanism and treatment of acute lung injury caused by inhalation of fabric protector. Design: Prospective, randomized study. Setting: University laboratory. Interventions: In vitro experiment: a porcine surfactant suspension (10 mg · ml−1) was exposed to a fabric protector aerosolized with an ultrasonic nebulizer for 1 min. Minimum surface tension (γ min) was sequentially measured using pulsating bubble equipment. Animal experiment: 14 adult rats were anesthetized with pentobarbital and mechanically ventilated with pure oxygen. Then, all rats inhaled fabric protector aerosolized with the nebulizer for five breaths. Three hours after inhalation, the rats were randomly assigned to two groups: a surfactant group (n = 7), in which surfactant (100 mg · kg−1) was replaced, and a control group (n = 7), in which no substance was given. Measurements and results: In vitro experiment: exposure to fabric protector aerosol increased the mean γ min of the surfactant from 1.7 to 19.2 mN · m−1 (n = 5, p 〈 0.05). Animal experiment: the mean partial pressure of oxygen in arterial blood (PaO2) in all rats decreased from 62.8 to 17.1 kPa at 3 h after inhalation. The PaO2 in the surfactant group increased to 49.8 ± 11.1 (SD) kPa at 30 min after surfactant replacement (p 〈 0.05), while the PaO2 in the control group remained below 20 kPa. Conclusions: Impairment of surfactant is a factor involved in the development of acute lung injury caused by inhalation of fabric protector. Surfactant replacement may be therapeutic for such injuries.

    Notes: Abstract Objective The pharmacokinetics of amikacin were studied in patients undergoing slow hemodialysis (HD). Design Slow HD was performed at the dialysate flow rate of 30 ml/min. After a single intravenous dose of amikacin 5 mg/kg, pharmacokinetic variables were calculated by fitting indivdual concentration-time curves to a two-compartment open model. Patients 6 critically ill patients with renal failure were entered into the study. Results The volume of distribution was 0.35±0.03 l/kg. Total body clearance was 35.1±2.3 ml/min with an elimination half-life of 10.5 h. During a 10.5 h session of slow HD, the serum amikacin concentration decreased from the peak level of 21.3±1.2 mg/l to 7.2±0.9 mg/l. Conclusion Slow HD eliminate amikacin more efficiently than other types of slowly performed renal replacement therapy and had profound effects on the pharmacokinetics. Amikacin elimination by this approach should be taken into consideration for designing a dosage schedule during the treatment.

    Notes: Abstract Seven cases of phalangeal microgeodic syndrome in childhood are reported. Reviewing the literature, we have found a total of thirty Japanese cases, including the cases of our own. On the other hand, only ten cases have been reported outside Japan. The mean age of onset of the Japanese cases was six and a half years, which is older than that in the initial description. Of further interest is the fact that all the cases in Japan occurred sporadically and exclusively in winter, and regressed spontaneously and fairly rapidly within six months. Furthermore, most of the cases reported in Europe also occurred in the winter months. These facts imply an etiological relationship between the lowered temperature and the bone lesions.

    Description / Table of Contents: Zusammenfassung Es wurde über 6 Patienten mit Edwards-Syndrom berichtet. Ein Kind (Fall 2) zeigte außer Extrachromosom Nr. 18 eine strukturelle Anomalie eines B-Chromosoms: 47,XY,18+,inv.(Bp+q-). Bei Fall 3 waren einige beim Edwards-Syndrom unübliche Mißbildungen vorhanden. Bei dem 4. Kind handelte es sich um eine Zwillingsgeburt, die Zwillingsschwester war klinisch und chromosomal normal. Ein Fall von Trisomie 18, der uns nach der Entdeckung der Bandentechnik bekannt wurde, konnte, mit dieser Technik einwandfrei als Trisomie 18 diagnostiziert werden. Einige Auffälligkeiten im Bereich der Dermatoglyphen wurden demonstriert.
    Notes: Abstract Six patients with Edwards' syndrome were studied. One child (case 2) was found to have a structural anomaly of a B chromosome as well as an extra chromosome of pair No. 18: 47,XY,18+,inv.(Bp+q-). Case 3 showed additional malformations not commonly present in Edward's syndrome. Case 4 was a twin; the twin sister was clinically and chromosomally normal. In case 6, which came to our attention after the discovery of the banding technique, it was possible to diagnose as trisomy 18 unequivocally by this method. Some striking features of the dermatoglyphics and creases are discussed.
