    Notes: Abstract —The study presents the results of the statistical relationship between seasonal northeast monsoon rainfall over Tamil Nadu state of India (TNR) and southeast India (SER) and mid-latitude circulation indices viz., zonal index (ZON) meridional index (MER) and the ratio of meridional to zonal index (M/Z) between the geographical area 35°N to 70°N at 500 hPa level over three sectors and hemisphere, based on 19 years (1971–1989) of data. The results indicate that northeast monsoon rainfall over India shows a strong antecedent relationship with the strength of ZON over all the sectors and hemisphere. The best association is observed during antecedent March over sector I (45°W–90°E) where direct and strong correlation coefficients of 0.69 and 0.64 are obtained with TNR and SER, respectively. Antecedent MAM (spring) season over sector I also shows a significant positive correlation with TNR/SER. Thus, the mid-latitude zonal circulation index may have possible use for the long-range forecasting of northeast monsoon rainfall over India.

    Notes: Abstract The therapeutic effect of boswellic acids and salai guggal in adjuvant induced arthritic rats in relation to urinary excretion of connective tissue metabolites viz. hydroxyproline, hexosamine and uronic acid was thoroughly investigated. Compared to controls, the arthritic animals showed an increase in the excretion of these metabolites in urine. The elevated levels of urinary hydroxyproline (free, total, nondialysable and dialysable), hexosamine and uronic acid in the arthritic animals were found to be slightly decreased in the acute phase and significantly decreased in the chronic phase of the disease following the administration of boswellic acids or salai guggal. The results of the investigation indicated that both these anti-inflammatory drugs could offer a partial protective action against changes induced by adjuvant induced arthritis.

    Notes: Abstract Aims/hypothesis. A computer model was developed to determine the health outcomes and economic consequences of different combinations of diabetes interventions in newly diagnosed patients with Type I (insulin-dependent) diabetes in Switzerland.¶Methods. We modelled seven complications of diabetes: hypoglycaemia, ketoacidosis, acute myocardial infarction, stroke, lower extremity amputation, nephropathy, and retinopathy. Transition probabilities and costs were taken from published literature. The Swiss health insurance payer perspective was taken. Various combinations of diabetes management strategies, including intensive or conventional insulin therapy and screening and treatment strategies for renal and eye disease were defined. Life expectancy, cumulative incidences of complications, and mean expected total lifetime costs per patient were calculated under six different management strategies. Incremental cost-effectiveness ratios were calculated in terms of costs per life-year gained compared with conventional insulin therapy alone.¶Results. The addition of screening for microalbuminuria and retinopathy followed by appropriate treatment, if detected, were cost saving, with reduction in cumulative incidence of end stage renal disease and blindness respectively, and, in the case of microalbuminuria screening and treatment, an improvement in life expectancy. Intensive therapy improved life expectancy but increased total lifetime costs.¶Conclusion/interpretation. Optimal management of Type I diabetic patients, including secondary and tertiary prevention, leads to reduced complications and improved life expectancy, with the increased costs of prevention offset to varying degrees by cost savings due to complications avoided.     Notes: Abstract This study examines the effects of acute in vitro acid-base disorders on Na+/H+ and H+-ATPase transporters in rabbit kidney proximal tubules (PT). PT suspensions were incubated in solutions with varying acid base conditions for 45 min and utilized for brush border membrane (BBM) vesicles preparation. BBM vesicles were studied for Na+/H+ exchange activity (assayed by 22Na+ influx) or abundance (using NHE-3 specific antibody) and H+-ATPase transporter abundance (using antibody against the 31 kDa subunit). The Na+/ H+ exchanger activity increased by 55% in metabolic acidosis (pH 6.5, HCO 3 − 3 mm) and decreased by 41% in metabolic alkalosis (pH 8.0, HCO 3 − 90 mm). The abundance of NHE-3 remained constant in acidic, control, and alkalotic groups. H+-ATPase abundance, however, decreased in metabolic acidosis and increased in metabolic alkalosis by 57% and 42%, respectively. In PT suspensions incubated in isohydric conditions (pH 7.4), Na+/H+ exchanger activity increased by 29% in high HCO 3 − group (HCO 3 − 96 mm) and decreased by 16% in the low HCO 3 − groups (HCO 3 − 7mm. The NHE-3 abundance remained constant in high, normal, and low [HCO 3 − ] tubules. The abundance of H+-ATPase, however, increased by 82% in high [HCO 3 − ] and decreased by 77% in the low [HCO 3 − ] tubules. In PT suspensions incubated in varying pCO2 and constant [HCO 3 − ], Na+/H+ exchanger activity increased by 35% in high pCO2 (20% pCO2, respiratory acidosis) and decreased by 32% in low pCO2 (1.5% pCO2, respiratory alkalosis) tubules. The NHE-3 abundance remained unchanged in high, normal, and low pCO2 tubules. However, the H+-ATPase abundance increased by 74% in high pCO2 and decreased by 69% in low pCO2 tubules. The results of these studies suggest that the luminal Na+/H+ exchanger is predominantly regulated by pH whereas H+-ATPase is mainly regulated by [HCO 3 − ] and/ or pCO2. They further suggest that the adaptive changes in H+-ATPase transporter are likely mediated via endocytic/exocytic pathway whereas the adaptive changes in Na+/H+ exchanger are via the nonendocytic/exocytic pathway.     Notes: Abstract. Na+/H+ exchanger isoforms NHE-2 and NHE-3 demonstrate distinct tissue expression patterns in renal epithelial cells. NHE-2 is predominantly expressed in the inner medulla whereas NHE-3 is highly expressed in the proximal tubule cells. The purpose of the current experiments was to study the characteristics of NHE-2 upon its own expression in cultured proximal tubule cells, LLC-PK1. Toward this end, LLC-PK1 cells were subjected to six cycles of proton suicide. The mutant cells, when grown to confluence and assayed for Na+/H+ exchanger by 22Na+ influx, showed significant reduction in NHE activity as compared to the parent cells (10.4 nmole/mg prot/4 min in parent cells vs. 1.8 in mutant cells, P 〈 0.001, n= 4). This remaining exchanger activity was mostly mediated via NHE-3 as shown by inhibition of the Na influx following PKC stimulation (65% with PMA vs. 100% without PMA. P 〈 0.05, n= 4). The mutant cells were transiently transfected with a pCMV/NHE-2 expression vector using calcium phosphate precipitation method. Northern blot analysis showed the expression of a 3.4 kb transcript only in the transfected cells. The expression peaked at 48 hr and diminished by 96 hr. The exchanger activity at 48 hr after transfection was mostly due to NHE-3 (as shown by inhibition in the presence of PMA) but was significantly lower than in sham transfected cells (1.2 nmoles/mg prot. in NHE-2-transfected and 2.1 in sham-transfected, P 〈 0.05, n= 4). At 60 hr after transfection, the cells exhibited PMA-stimulated Na influx (〉28%) indicating functional expression of NHE-2. Increasing the osmolality of the media to 510 mOsm/l stimulated the Na+/H+ exchanger in NHE-2 transfected cells but inhibited the exchanger activity in sham transfected cells. In conclusion, NHE-2 appears as a 3.4 kb transcript in transfected LLC-PK1 cells and functional expression of NHE-2 is preceded by inhibition of endogenous NHE-3 activity. The NHE-2 is stimulated by hypertonicity, indicating a likely role for this isoform in cell volume regulation.
