    Notes: Summary Tellurium is one of the rarest elements on earth. Intoxications are rare and almost exclusively occupationally exposed workers are affected. Only a few cases of non — occupational poisoning have been reported so far. Severe poisoning results in respiratory depression and circulatory collapse. After occupational exposure main symptoms and signs include loss of appetite, dryness of the mouth, suppression of sweating, a metallic taste in the mouth, and most notable, a sharp garlic odor of the breath, sweat and urine. We report our findings in a 37 year old, non-occupationally exposed woman with tellurium intoxication.

    Notes: Summary In 45 patients with carotid sinus syndrome, the heart rate-independent vasodepressor component under ventricular and av-sequential pacing of the heart was investigated both in supine and in standing patients. Under both forms of pacing, the carotid pressure determination has led to a marked lowering of systolic and diastolic arterial blood pressure which is significantly more pronounced under orthostasis. The blood pressure values reached both in supine and in standing patients are significantly higher under av-sequential pacing than under ventricular pacing. Accordingly, the proportion of symptomatic patients is less under av-sequential pacing in the supine position (17% versus 29%) and in standing (65% versus 83%). The heart rate-independent vasodepressor reaction is maximal on average between 11 s and 16 s after the beginning of the carotid pressure test and persists for 3 s to 7 s. The blood pressure reaches initial values again after 14 s to 20 s. To summarize, these data document that a clinically relevant heart rate-independent vasodepressor reaction is to be reckoned with in the majority of patients with carotid sinus syndrome. This shows a different time course than the vagally determined effect of the carotid sinus reflex on heart rate. Orthostasis intensifies the vasodepressor reaction so that the hemodynamically more favorable avsequential pacing which is actually more favorable in hemodynamic terms cannot guarantee freedom from symptoms in the majority of patients in the upright standing position.

    Notes: Summary There is a statistically significant order in the tangential orientation of stellate cell dendrites, both spiny and smooth, in layer IV of the barrelfield of the mouse parietal cortex. Neurones situated in a barrel side have most of their dendrites oriented towards the barrel hollow; those situated in the hollow preferentially orient their dendrites parallel to the long axis of the barrel. A quantitative measure of the orientation of individual dendrites in barrelfields of 60-day old mice was obtained using a semiautomatic computer-microscope and a minicomputer. In the same manner, the dendrite orientation of layer IV stellate cells was determined in barrelfields, whose (cytoarchitectonic) pattern had been experimentally altered through lesions of the middle row of the mystacial vibrissal follicles at birth. The dendrites of these cells are oriented in harmony with the novel parcellation of the cortex. Consequently, for cells in the altered areas of the barrelfields, the dendrite orientation is different from that of cells with identical positions in a normal field (see Fig. 8). We tentatively interpret these findings as an adaptation of dendrite orientation to an altered pattern of thalamic input to layer IV that, in turn, is a consequence of the peripheral manipulation.

    Description / Table of Contents: Zusammenfassung 6 Kinder und Adoleszenten unter intermittierender Hämodialyse und ein transplantiertes Kind wurden im Abstand von 8–20 Monaten zweimal psychologisch-psychiatrisch untersucht. Ihre Stimmung, Reaktivität, Körperschema, Reaktionsbildung sowie Anpassung an die eingreifenden Behandlungsmethoden werden dargestellt. Der Adaptationsprozeß wird günstig beeinflußt durch eine komplikationsarme Behandlung, Vertrauen zum behandelnden Team und eine realistische, angstarme Einstellung der Eltern gegenüber der Krankheit und Behandlung ihres Kindes. Die Rehabilitation im psychosozialen Bereich hängt weitgehend von der Ausprägung dieser Faktoren ab.
    Notes: Abstract Six children and adolescents on intermittent dialysis and one child transplant recipient were subjected to both psychiatric and psychological examinations twice over 8 to 20 months. Mood, reactivity, body image, reaction formation and adjustment to the radical method of treatment are presented. The process of adaptation seems to be positively influenced by a treatment with few complications and also by confidence in the therapeutic team and a realistic attitude with a minimum of anxiety on the part of the parents towards the illness and treatment of their child. Rehabilitation in the psychosocial sphere depends largely on the successful achievement of these goals.

    Description / Table of Contents: Zusammenfassung An Hand der Krankengeschichte eines zum Zeitpunkt des Todes 14 Jahre alten Mädchens werden einige somatische und psychologische Probleme bei intermittierender Hämodialysebehandlung und Nierentransplantation im Kindesalter besprochen. 7 Jahre nach Beginn eines steroidresistenten nephrotischen Syndroms wurde wegen einer progredienten Niereninsuffizienz zunächst eine chronische Hämodialysebehandlung, nach weiteren 4 Monaten eine Nierentransplantation durchgeführt. Durch medizinische Komplikationen wurde die Rehabilitation stark verzögert, jedoch 2 1/2 Jahre nach Nierentransplantation begann eine kontinuierliche Besserung im medizinischen, sozial-psychologischen, schulischen und familiären Bereich. Nach 1 1/2jährigem Wohlbefinden und weitgehender Rehabilitation starb das Mädchen an einer akuten Cryptococcusmeningitis. Die Problematik der Entscheidung zur Dialyse und Nierentrasplantation, die Vorbereitung zur Operation und Rehabilitation werden diskutiert.
    Notes: Abstract Presentation of some somatic and psychological problems of intermittent hemodialysis and kidney transplantation in children, with reference to the history of a girl 14 years old at the time of her death. Seven years after the onset of a steroid-resistant nephrotic syndrome, chronic hemodialysis and (4 months later) a kidney transplant were performed because of progressive kidney failure. Rehabilitation was greatly retarded by medical complications, but 2 1/2 years after the transplant a steady improvement in the medical, sociopsychological, academic, and family spheres was possible. After 1 1/2 years of well-being, and extensive rehabilitation, the girl died of acute cryptococcus meningitis. The questions of the decision on dialysis and kidney transplantation, preparation for surgery and rehabilitation are discussed.
