    Notes: The prevalences of asthma, allergic rhinitis (AR), and eczema were analyzed in relation to retrospective risk factors from birth in a questionnaire study of schoolchildren in two areas covering the whole climatic span of Sweden: the Göteborg area on the southwestern coast (7-year-olds, n= 1649) and Kiruna, a mining town in the northernmost inland mountains (7–9-year-olds, n= 832). The strongest background factor, a family history of the diseases, was more common in children with another strong risk factor, particularly for asthma: high frequency of upper respiratory tract infection (URTI). Other significant risk factors related to high indoor humidity caused an increased prevalence of both allergic diseases and URTI. Active mechanical ventilation of the homes caused a slight reduction of the prevalence of allergic diseases, and repainting or new wallpaper in the bedroom of the child after birth caused a moderately increased risk of allergic disease. This study illustrates the interaction between genetic and environmental risk factors with special emphasis on factors related to an unventilated indoor climate, which may have substantially contributed to the current increase of the diseases in the country.

    Notes: Background: It is discussed whether exposure to pets during childhood is a risk or a protective factor for sensitization and allergic symptoms. The aim of this study was to investigate the association between pet-keeping at time of birth and allergic symptoms in airways, nose and skin among young children in Sweden.Method: A questionnaire was sent to the parents of 14 077 children (1–6 years), the focus being on allergic symptoms, home environment and other background factors including pet-keeping and avoidance behaviour. The response rate was 79%.Results: Almost one-tenth of the population had got rid of pets because of allergy in the family, and 27.3% reported ‘avoidance’ behaviour towards pets. In a cross-sectional analysis current pet-keeping was ‘protective’, but this may be due to the fact that people avoid exposing their child to something that they believe is a risk factor for allergies. Pet-keeping at the time of birth was associated with ‘wheezing’, ‘asthma’ and ‘rhinitis on pet-exposure’ later in life for children from families with an ‘avoidance’ behaviour, and was not ‘protective’ for other children. There was also an indication of a dose–response relationship between the number of types of furred pets at time of birth and later symptoms in analyses adjusted for avoidance behaviour or current pet-keeping.Conclusion: The distribution of pet-keeping in the population is largely explained by avoidance behaviour, meaning that those who have pets mainly are those who can stand them, indicating a ‘healthy pet-keeping effect’.

    Notes: Background:  Strategies to prevent children from developing allergy have been elaborated on the basis of state-of-the-art reviews of the scientific literature regarding pets and allergies, building dampness and health, and building ventilation and health. A similar multidisciplinary review of infant feeding mode in relation to allergy has not been published previously. Here, the objective is to review the scientific literature regarding the impact of early feeding (breast milk and/or cow's milk and/or formula) on development of atopic disease. The work was performed by a multidisciplinary group of Scandinavian researchers.Methods:  The search in the literature identified 4323 articles that contained at least one of the exposure and health effect terms. A total of 4191 articles were excluded mainly because they did not contain information on both exposure and health effects. Consequently, 132 studies have been scrutinized by this review group.Results:  Of the 132 studies selected, 56 were regarded as conclusive. Several factors contributed to the exclusions. The studies considered conclusive by the review group were categorized according to population and study design.Conclusions:  The review group concluded that breastfeeding seems to protect from the development of atopic disease. The effect appears even stronger in children with atopic heredity. If breast milk is unavailable or insufficient, extensively hydrolysed formulas are preferable to unhydrolysed or partially hydrolysed formulas in terms of the risk of some atopic manifestations.

    Notes: Thirty single-family homes with either high (≥ 2000 ng/g) or low (≤ 1000 ng/g) house-dust mite (HDM) allergen levels in mattress dust were examined for ventilation, thermal climate, and air quality (formaldehyde and total volatile organic compounds (TVOC). Elevated concentrations of HDM allergen in mattress and floor dust were associated with the difference in absolute humidity between indoor and outdoor air, as well as with low air-change rates of the home, particularly the bedroom. No correlation was found between concentration of TVOC or formaldehyde in bedroom air and HDM allergen concentration. In regions with a cold winter climate, the air-change rate of the home and the infiltration of outdoor air into the bedroom appear to be important for the infestation of HDM.

Mite allergens during 18 months of intervention.In Stockholm, Sweden, 17 children with newly diagnosed sensitization to house-dust mites (HDM) and 11 children with previously diagnosed HDM-sensitization were included in a study of HDM-allergen avoidance. Mattress dust was collected on repeated occasions during 18 months and assayed for concentration of major HDM allergens. During the first 12 months, the parents of the intervention group were instructed to intensify cleaning and airing of the child's bedroom. During the last 6 months of the study, the mattresses and pillows of seven children in the intervention group and sibling controls were encased in semipermeable polyurethane covers. The homes exhibited a high absolute indoor humidity throughout the year, and even during the winter the mean levels exceeded 7 g/kg. No mite allergen reduction was seen in the intervention group during the first year. However, among the newly diagnosed HDM-sensitized children, there was a mean reduction of the mattress mite allergen concentration of 83% (P= 0.02), and this was most pronounced in the homes with low humidity. At the end of the mattress encasement period, an average difference of 98% (P〈 0.001) was found between the vacuumed amount of mite allergen on top of the covers and that underneath.

    Notes: Abstract Ten researchers from the Nordic countries have reviewed the literature on early exposure in childhood, to pets and the risk of sensitisation/allergy. A search of the literature resulted in identification of about 1200 articles, of which 89 were selected for further examination. The group concluded that all exposure to pets involves a risk of sensitisation. Exposure in infancy involves an increased risk (normally RR = 1–1,5) of sensitisation and to a lesser degree of the development of symptoms. The group also concluded that pet allergens are present “everywhere”, as a result of people bringing allergens in their clothing, etc. The group concluded as a recommendation, that families with atopic individuals should wait until the child is at least two years old before getting a pet.

    Notes: Abstract The presence of Volatile Organic Compounds (VOC) in indoor air has in past decades often been associated with adverse health effects such as sensory irritation, odour and the more complex set of symptoms called the Sick Building Syndrome (SBS). More recently, a possible link between the increase in the prevalence of allergies throughout the industrialized areas of the world and exposure to elevated concentrations of VOCs has been suggested. In many cases, the total VOC (TVOC) is used as a measure of the concentration of air pollution and, by extension, as a measure of the health risk in non-industrial buildings. However, the TVOC concept has been questioned for a number of reasons, including the facts that it is an ambiguous concept, that individual VOCs making up the whole can be expected to give rise to different effects in people and that researchers have been using different definitions and interpretations of TVOC. This means that simple addition of the quantities of individual VOCs may not be relevant from a health point of view.Twelve researchers from the Nordic countries have reviewed the literature on VOC/TVOC and health. A search of the literature resulted in the identification of about 1100 articles, of which 120 were selected for further examination. A final review of the articles reduced their number to 67 that contained data on both exposure and health effects.The group concluded that indoor air pollution including VOC is most likely a cause of health effects and comfort problems in indoor environments in non-industrial buildings. However, the scientific literature is inconclusive with respect to TVOC as a risk index for health and comfort effects in buildings. Consequently, there is at present an inadequate scientific basis on which to establish limit values/guidelines for TVOC, both for air concentrations, and for emissions from building materials. The group concluded that continued research is required to establish a risk index for health and comfort effects for VOC in non-industrial buildings.
