    Notes: Summary Sera from three patients with porphyria cutanea tarda (PCT) were examined for evidence of complement activation. The irradiation of sera in vitro with 405-nm light resulted in a dose-dependent diminution of total hemolytic complement activity and the hemolytic titers of C1, C4, C2, C3, and C5. Furthermore, such irradiated sera showed immunoelectrophoretical C3 conversion, chemotactic activity for rat polymorphonuclear leukocytes inhibited by incubation with anti-C5 antisera but not anti-C3 antisera, and C5a generation as measured by radioimmunoassay. Factor-B conversion did not occur in such irradiated sera. Using Sephadex G-75 chromatography, the irradiated sera showed a multiphasic elution profile of chemotactic activity similar to that of zymosan-activated serum. The generation of C5a even occurred in factor-B-depleted serum. These studies indicate that the irradiation of PCT serum with 405-nm light induces the activation of the complement system via the classical pathway, resulting in the development of a chemotactic factor.

    Notes: Summary We measured the electrical conductance at 3.5 MHz of a sheet of normal stratum corneum (SC) mounted with either the upper surface or the lower surface downward in simulated in vivo conditions. In this way, we assessed the water-binding capacity of the upper and lower portions of the horny layer. Measurements were made with the SC model in environments of various relative humidities. Between 30% and 90% relative humidity, the conductance of the upper surface was always significantly higher than that of the lower surface. In contrast no significant difference was observed in transepidermal water loss (TEWL) with the SC sheet placed upwards or downwards. After exposure to lipid extraction with acetone/ether, a significant decrease in conductance and increase in TEWL occurred, particularly in the upper surface. The amount of extractable amino acids was much higher from the middle layer than from the outer layers of the SC, and was lowest from the deepest part of SC. These results, indicating a lower efficiency of the lower surface of the SC for water-binding than the desquamating upper surface, suggest that newly formed immature SC does not have the water-holding capacity of the mid portion of the SC, which is probably the layer with the greatest water holding capacity.

    Notes: Summary Tumor necrosis factor (TNF)-α is a cytokine with multiple biological properties, particularly proinflammatory, apart from the induction of tumor necrosis. In order to elucidate the role of TNF-α in the pathogenesis of psoriasis, we have carried out bioassay and enzyme-linked immunosorbent assay for TNF-α in suction blister fluids and horny tissue extracts from psoriatic skin. Although bioassay showed some activities in the suction blister fluids and horny tissue extracts, there were no significant differences between the levels of activities from normal and psoriatic skin. They were at the background level and pretreatment of the samples with anti-TNF-α antiserum failed to abrogate the activities. ELISA confirmed the absence of TNF-α. Therefore, the present study could not verify that TNF-α plays a major role in the pathogenesis of psoriasis.

    Notes: The face is composed of complicated anatomical components, presenting unique portions, such as the eyes, nose and mouth in a relatively narrow area. Moreover, the facial skin is densely populated by the pilosebaceous units and sweat glands, and its stratum corneum (SC) is much thinner than that of the trunk and limbs, although it is always exposed to the environment. Among various portions of the facial skin, some are more easily irritated than others by environmental stimuli, or are more often affected by certain dermatoses. However, the functional aspects of the different portions of the facial skin have not been studied in detail under a strictly controlled environment in sufficiently large numbers of subjects covering different age groups. Thus, we conducted studies in winter with various biophysical techniques, such as transepidermal water loss (TEWL), as a parameter for SC barrier function, high-frequency conductance as that for skin surface hydration state, skin surface lipids, pH, blood flow and skin surface temperature on the forehead, mid-portion of the cheek (cheek in short), nasal tip (nose in short), nasolabial fold and chin of 20 healthy Japanese females aged 22–37 years (average 25 years) in a climate chamber adjusted to 21 °C and 50% relative humidity. Thereafter, we studied the influence of ageing on these biophysical parameters by collecting data of TEWL, high-frequency conductance and size of superficial corneocytes on the cheek, nasolabial fold and chin of 303 healthy Japanese female volunteers of different ages. The obtained results showed that the barrier function of the SC was best on the cheek, presenting the lowest TEWL, which was significantly higher on the nasolabial fold and chin than on the cheek. TEWL showed a decrease with age. In contrast, skin hydration state was higher on the nose, but it tended to be lower on the nasolabial fold, showing a mild age-related increase. The corneocytes on the nasolabial fold and chin were smaller than those on the cheek. They revealed a clear increase in size with age. Skin surface lipids were richest on the nose, whereas the superficial pH on the nose was the lowest among the regions tested. The skin temperature was lowest on the cheek than on other areas of the face; although, together with the nose, its blood flow was higher than that of the others. These data indicate great regional differences observable in SC functions on the face. In general, the SC barrier function increases with age, probably because of a decreased epidermal turnover rate as recognized by the increase in corneocyte size. Among the various sites, the skin of the nasolabial fold and chin, whose SC consisted of the smallest corneocytes, showed poorest SC properties in barrier function, suggesting the presence of mild invisible inflammation. It is understandable that this area easily develops not only the complaint of sensitive skin to cosmetics but also dermatitis because of various external agents.

    Notes: Basal cell carcinomas arising from epidermal cysts are rare. A 76-year-old Japanese man had had a blackish nodule on his right knee for 15 years, under which he later noticed the development of a subcutaneous nodule. On histological examination masses of tumour cells showed the feature of adenoid and solid patterns of basal cell carcinoma that were connected to the wall of epidermal cysts in many places as well as with the overlying epidermis.

    Notes: When we attain a certain level of mental development, it is natural for us to wish to show our bodily features as attractive as possible to others, particularly to those of opposite sex. Thus, cosmetics have been playing an important role in our cultural activities. In their long history, human beings have made all possible efforts to utilize various natural substances as ingredients for the formulation of desirable make-up products and fragrances. However, some of them caused unexpected skin problems or even a hazard to health. Hence, in the past, dermatologists tended to regard cosmetics as unnatural and sometimes undesirable substances applied to the skin.In old days, those individuals who had conspicuous skin diseases were discriminated in their society under a collective term of leper. Even until recently, patients who had congenital skin malformations or keratinization disorders, such as severe cases of ichthyosis, were cruelly put on exhibition to the public. Studies conducted by dermatologists have greatly contributed to eradication of such a prejudice against skin disfigurements and skin diseases. At the same time, through various trials and errors, they have developed many therapeutic measures for them. However, it is only in the later half of the last century that medicine and cosmetic science have begun to make the best use of the rapid progress in biological sciences, uniting their working sphere of activity. It has become clear that dermatology and cosmetology, both concerned not only with improvement of the skin appearance but also with maintenance and enhancement of skin health, share the same background of skin science. Their expertise has begun to be shared each other, contributing greatly to the recent rapid progress of the science related to the skin. They include the development of various non-invasive biophysical methods that are employed for objective and quantitative in vivo evaluation of morphological and functional properties of the skin that are easily overlooked even by astute eyes of expert clinicians and that of acceptable and truly effective skin care products. Science has now become a strong driving force to accelerate development of desirable cosmetics.

    Notes: We report an unusual case of sebaceous gland hyperplasia manifested clinically as a large solitary intracutaneous nodule, which was at first diagnosed as an epidermoid cyst. Histologically, the nodule consisted of a single large central cavity connected with numerous fully-matured sebaceous glands. We discuss the unique clinical appearance and its histopathological differential diagnosis including sebaceous adenoma, sebaceous trichofolliculoma, organoid nevus and sebaceous gland hyperplasia.

    Notes: We report a 24-year-old male patient with sarcoidosis who developed subcutaneous nodules on his thighs and buttocks at the sites of repeated intramuscular injections. Histologically, there were extensive areas of caseation necrosis surrounded by typical sarcoidal granuloma. The nodules regressed spontaneously over a period of 4 months, concurrently, both his bilateral hilar adenopathy and the results of immunological tests, returned to normal. We think that these lesions developed at the sites of subcutaneous scar tissue induced by frequent injections in childhood. We believe the pathomechanism to be similar to that for scar sarcoidosis, and that the extensive caseation necrosis reflected a regressing stage of the disease.
