    Notes: Summary The characteristics of the increased calcium (Ca) influx observed in metabolically depleted red blood cells (RBCs) of hypertensive patients were investigated. Twenty-four normotensives, 16 untreated essential hypertensives, and 10 essential hypertensives under sufficient blood pressure control by 50–100 mg/day atenolol were studied. Free intracellular concentrations of Ca, sodium (Na), and potassium (K) were assessed using ion-selective electrodes in freeze-thawed RBCs, which were metabolically depleted by 30 mM desoxy-glucose at 37°C for 48 h. In the treated hypertensives values for Ca and K at 24 and 48 h were not different from values for the normotensives, whereas elevated Ca was found in RBCs of untreated hypertensives. Na in treated hypertensives was significantly increased at 0 and 48 h, thus, being similar to values for untreated hypertensives. Additionally, RBCs of six normals were stressed in a glass/teflon potter. Before metabolic depletion electrolytes were not affected by this procedure, while Ca at 24 and 48 h of metabolic depletion increased to significantly higher values for the hypertensive patients as compared to the controls. These results suggest that the altered Ca metabolism in the RBCs of hypertensives may reflect a secondary phenomenon due to the mechanical damage to RBCs by the elevated blood pressure.

    Description / Table of Contents: Zusammenfassung Bei einem 35jährigen Patienten mit rheumatischen Aortenvitium, Schweregrad III–IV, wurde im Rahmen einer diagnostischen Linksherzkatheterisierung auf transseptalem Wege eine bipolare Schrittmachersonde in den linken Vorhof gelegt. Durch elektrische Reizung einer eng umschriebenen Stelle an der linken, lateralen, oberen Vorhofwand trat bei einer Frequenz von 125/min reproduzierbar und sofort ein WPW-Syndrom auf, das nach Abschalten des Schrittmachers ohne Zeitverzögerung wieder verschwand. Durch Stimulation anderer Partien des linken und rechten Vorhofs mit gleicher Frequenz ließ sich kein WPW-Syndrom auslösen. Es kam lediglich zu den bekannten bei artifizieller Vorhofstimulation auftretenden PQ-Verlängerungen. Es wird diskutiert, daß eine bis dahin „stumme“, akzessorische Herzmuskelbrücke zwischen linkem Vorhof und linker Herzkammer aktiviert wurde. Die bei Vorhofstimulation mit höheren Frequenzen beobachtete Verzögerung der av-Überleitung kann die Aktivierung der Kammer über das akzessorische Bündel ermöglichen.
    Notes: Summary During diagnostic transseptal heart catheterism, a bipolar pacemaker catheter was placed into the left atrium of a 35 year old man suffering from rheumatic aortal stenosis of degree III–IV. Electrical stimulation at a rate of 125/min of a well circumscribed small area of the left lateral superior atrial wall was repeatedly and immediately followed by a Wolff-Parkinson-White syndrome which disappeared with disconnecting the pacer. Stimulation at other parts of the left and right atrium at even the same rate did not show any WPW syndrome, besides of the well known PQ prolongation occurring in artificial atrial pacing. It is discussed that an up to now “silent” accessory heart muscle bridge between atrium and ventricle of the left heart is activated by the pacer. The delay of the AV-conduction seen in atrial stimulation at higher rates may possibly allow the activation of the ventricle via the accessory bundle.

    Description / Table of Contents: Zusammenfassung Für die Vorhofstimulation ist charakteristisch, daß es mit steigender Stimulationsfrequenz zu einer Verzögerung der atrioventriculären Überleitung kommt. In der vorliegenden Mitteilung wird über 5 Beobachtungen an Herzkranken berichtet, bei denen es während Vorhofstimulation mit erhöhter, aber gleichbleibender Stimulationsfrequenz zu phasischen Verlängerungen und Verkürzungen der av-Überleitungszeit kam. In einem dargestellten Fall schwankte die Überleitungszeit bei einer Reizfrequenz von 92/min zwischen 0,26 und 0,40 sec mit einer Phasenlänge von 9–60 sec. Das Phänomen trat nur bei einer bestimmten und individuell unterschiedlichen Stimulationsfrequenz auf und war reproduzierbar. Diese kritische Frequenz lag bei verschiedenen Patienten zwischen 92 und 150/min, und zwar dreimal dicht unterhalb der zu Blockierung führenden Frequenz, während zweimal eine weitere Frequenzsteigerung wieder zu einer konstanten verlängerten Überleitung führte.
    Notes: Summary The prolongation of the AV-conduction induced by atrial pacing with increasing heart rate is well known. Atrial pacing was performed in 22 patients with cardiac disease. In 5 of these, atrial pacing at a special constant rate was followed by phasic prolongation and shortening of the AV-interval. In a typical case (pacing rate 92/min) AV-conduction ranged phasically from 0.26–0.40 seconds with a phase length of 9–60 seconds. The phenomenon of phasic prolongation and shortening was reproducible. It could be demonstrated only at a certain pacing rate which differed from one patient to the other, and it was between 92 and 150 per minute. In 3 patients the rate required was slightly below the rate causing Wenckebach's cycles, in 2 others further increase of the pacing rate was followed by a constant prolonged AV-conduction.

    Notes: Summary This study was performed to assess the dependence of the effect of the duration of stimulation on the effective refractory period (ERP). ERP was determined in 12 patients after four consecutive stimuli as well as after continuous stimulation for 1 min applying a constant stimuli interval of 600 msec. After 1 min stimulation all patients showed a significant (P〈0.01) decrease of the ERP with 236±23 SD msec in comparison to the ERP of 271±26 SD msec after four consecutive stimuli. Although it is well established that ERP depends on the previous cycle length, it is additionally influenced by the stimulation duration, as shown in this study. This must be considered when ERP or other electrophysiological parameters of the right ventricle are investigated.

    Notes: Summary A 39-year-old male with homozygous familial hypercholesterolemia confirmed by tissue culture suffered from mild aortic insufficiency and valvular stenosis with a gradient of 20 mm Hg across the aortic valve. Plasmapheresis carried out every 2 weeks for 4 years resulted in a marked reduction in the serum cholesterol level and in a regression of the valvular stenosis, as shown by echocardiography and by left heart catheter.

    Description / Table of Contents: Zusammenfassung Bei Herzkatheteruntersuchungen am Menschen wurde die effektive Refraktärzeit (RZ) des rechten Ventrikels mit der Doppelstimulationsmethode gemessen. Bei Frequenzsteigerung, mittels elektrischer Stimulation erzeugt, kommt es physiologisch zu einer Verkürzung der RZ. Im Akutversuch wurde die Wirkung von Aldadiene-K als Kurzinfusion untersucht. 10 min und in einigen Fällen 45 oder 60 min nach 800 mg Aldadiene-K wurde die RZ erneut bestimmt, und zwar jeweils bei verschiedenen, bereits im Vorversuch am gleichen Patienten geprüften Frequenzen. Bei 5 in dieser Weise untersuchten Patienten wurde eine Verkürzung der RZ nach Aldadiene gefunden, verglichen mit den vorausgegangenen Kontrollmessungen bei den entsprechenden Herzfrequenzen. Diese beim Menschen gefundene Verkürzung der RZ nach Aldadiene-K spricht für eine positiv inotrope Wirkung der Substanz. Ein antiarrhythmischer Effekt wäre über eine Verbesserung der elektrischen Dispersion des Myokards denkbar.
    Notes: Summary The effective refractory period (RP) of the right ventricle was measured in patients during routine heart catheterisation by the method of paired stimulation. With increasing heart rate, induced by electrical stimulation, the RP shortens physiologically. The effect of Aldadiene-K (Canrenoate-K) was studied in acute infusion experiments. 10 min and in some cases 45 or 60 min after 800 mg Aldadiene-K, the RP was measured at different heart rates as it was done in the same patient before administration of the substance. In 5 patients studied in such way, there was a decrease of the RP after administration of aldadiene-K, compared to the control measurements at the corresponding heart rates. This decrease of the RP in man suggests a positive inotropic action of the substance. An antiarrythmic activity might be possible by means of an improvement of the electrical dispersion of the myocardium.
