    Notes: Abstract Histamine assays can be unreliable in individual subjects or samples even though the particular method is in general working very well. Therefore the specificity and accuracy of histamine determination in the gastric aspirate of individual duodenal ulcer patients was thoroughly examined and shown to be satisfactory. Pitfalls of the fluorometric assay were investigated. A native (non-histamine) fluorescence in gastric aspirate which occurs before the addition of OPT was not removed by the original Shore procedure. In the combined assay (Dowex 50+ butanol extraction) this fluorescence no longer interferes with the assay. For the identification of histamine in a single gastric aspirate of an individual duodenal ulcer patient, the reversed blank (3M HCl added to the reaction mixture before OPT instead after OPT), excitation and fluorescence spectra, the heating test with spectra recorded and the HMT test were found to be reliable. The formaldehyde test and the heating test without recording the spectra were useless since they gave false negative results. Since the HMT test was regarded as a reference method it was thoroughly investigated both by theoretical considerations (enzyme kinetics) and by a series of measurements in a single patient as well as in a group of nine subjects. Samples from the period of peak acid output in response to pentagastrin showed an average histamine concentration of about 8 ng/ml and a histamine output of 1.5 μg/30 min.

Jeder 5. Kranke mit Zeichen der aktiven Blutung, jeder 6. mit Zeichen der vorausgegangenen Blutung, aber nur jeder 11. ohne Blutungszeichen verstarb in der Klinik. Arteriell Blutende hatten eine höhere Letalität als solche mit Sickerblutung. Blutungsaktivität und Blutungstyp haben Prognoserelevanz und sind bei Studienplanung zu berücksichtigen.
    Notes: Summary To evaluate different treatment attitudes in upper GI bleeders we must know the factors that might influence outcome. In a 2.5-year period 248 patients had early endoscopy. Every four to five patients had more than one lesion. Every fifth patient with an actively bleeding lesion, every 6th with a recent lesion, and every 11th with no bleeding lesion died during hospital stay. Arterial bleeding was combined with 33 %, but capillary bleeding showed only 23 % mortality. Therefore, bleeding activity and bleeding type are relevant prognostic factors to be incorporated in trial designs.

    Description / Table of Contents: Zusammenfassung In einer prospektiven, konsekutiven Studie wurden 26 Kranke wegen Magencarcinom gastrektomiert und nach Longmire-Gütgemann eine 40 cm lange Jejunumschlinge End-zu-End isoperistaltisch durch SPTU-Klammernaht (12 Kranke) bzw. Cerny-Handnaht (Polyglykoleinzelknopfnähte, 14 Kranke) interponiert. Die Gruppen waren vergleichbar hinsichtlich Alter, Geschlecht, Gewicht, TNM-Stadium und Anzahl der Operateure. Die Insuffizienzrate betrug bei der SPTU-Naht 6/12, bei der Handnaht 2/16. 2mal versagte das Gerät intraoperativ. Eine generelle Empfehlung des SPTU ist daher nicht möglich.
    Notes: Summary Gastrectomy and gastric replacement by an isoperistaltic 40-cm jejunal loop with end-to-end anastomosis between the esophagus and jejunum were performed on 26 patients with adenocarcinoma of the stomach. This anastomosis was performed by the SPTU in 12 and by hand (Cerny interrupted sutures) in 14 cases. Patients were comparable according to age, sex, weight, TNM stage, and number of surgeons. Rate of anastomosic insufficiency was 6/12 in the SPTU and 2/16 in the Cerny group. Technical defects occurred twice intraoperatively with SPTU. Therefore, regular use of the SPTU is not recommended.

    Description / Table of Contents: Zusammenfassung 10–12 Wochen alte weibliche Mäuse der Stämme C3H und (101xC3H)F1 wurden mit 2,3,5-Triäthyleniminobenzochinon-1,4 (T) kurz vor der Ovulation behandelt. Danach wurden die ♀ ♀ mit unbehandelten ♂ ♂ des gleichen Typs verpaart. Die aus dieser Paarung resultierenden Embryonen wurden zum Zeitpunkt des 2-Zellstadiums aus dem Eileiter ausgespült und einer Chromosomenanalyse unterworfen. Es fanden sich signifikante Unterschiede zwischen Versuch und Kontrolle in beiden Stämmen bezüglich der Aneuploidieraten. Die Raten struktureller Aberrationen waren gegenüber den Kontrollen zwar auch in beiden Stämmen erhöht, die Werte waren jedoch nur im C3H-Stamm signifikant.
    Notes: Summary 10–12-week-old female mice of the C3H strain and (101xC3H)F1 hybrids were treated with 2,3,5-triethyleneiminobenzoquinone-1,4 (T) shortly before induced ovulation. Then the animals were mated with untreated males of the same type. The embryos from these matings were flushed out of the oviduct usually during the 2-cell-stage and subjected to chromosome analysis. We found significant differences between both experimental series and matching controls, especially with the frequency of aneuploidy. Thus, we could demonstrate that chromosome aberrations can be chemically induced in the pre-ovulatory phase of oogenesis.

    Description / Table of Contents: Zusammenfassung 103 Patienten mit sichtbar aktiver Blutung bei der Notfallendoskopie wurden von Januar 1978 bis Dezember 1981 durch Lasercoagulation behandelt. Die Blutstillungsrate betrug 90 %, die Rezidivblutungsquote 38 % (35 Patienten). Die Analyse der Therapieversager (= nicht stillbare Blutung, Rezidivblutung) zeigt für die arterielle Blutung oder den intermittierend blutenden Gefäßstumpf eine Versagerquote beim U. duodeni (n = 24) von 80 %, beim U. ventriculi (n = 24) von 50 %. Sie lag damit 5- bzw. 3mal höher als bei Sickerblutungen. Das arteriell blutende U. duodeni sollte daher primär operiert werden.
    Notes: Summary From January 1978 to December 1981 laser therapy was carried out on 103 of 163 patients with visibly active hemorrhage. Control of bleeding was achieved in 90 %; 35 patients experienced recurrent bleeding (38 %). Arterial bleeding or intermittent bleeding of visible vessels showed an 80 % failure rate with laser therapy (uncontrollable or recurrent bleeding). Duodenal ulcer with arterial bleeding should be primarily treated by operation. The prognosis of patients with upper GI bleeding can possibly be improved if therapeutic concepts rely more on urgent endoscopic findings.

In 65% kam es zum spontanen Blutungsstillstand. Bei 97% der aktiv blutenden Läsionen (n=33) konnte die Blutstillung endoskopisch erzielt werden, nur ein Patient mußte operativ behandelt werden. Die Rezidivblutungsquote lag bei 4%, in allen Fällen konnte durch erneute endoskopische Verfahren Blutstillung erreicht werden. Die Letalität lag bei 0%. Damit hat das blutende Mallory-Weiss Syndrom eine günstige Prognose. Operative Behandlung ist nur in Ausnahmefällen erforderlich.

    Description / Table of Contents: Zusammenfassung Zur frühen Erfassung von Rezidivblutungen wurde mit Methoden der medizinischen Entscheidungsfindung ein auf klinischen and laborchemischen Parametern basierendes Überwachungssystem entwickelt and prospektiv an 80 Patienten validiert. Die Endoskopie entschied über die Patienten-Aufnahme and wurde als Kontrolle bei Alarm auf Rezidivblutung eingesetzt. Die Sensibilität dieses Systems betrug 63%, die Spezifität 86%. Bei weiterer Verbesserung der Sensibilität sind sowohl unnötige Kontrollendoskopien vermeidbar, als auch eine frühzeitige Erfassung der Rezidivblutung möglich.
    Notes: Summary Using medical decision-making methods, a monitoring system for the early recognition of recurrent bleeding was developed. This system was validated in a prospective study on 80 patients. Endoscopic findings determined which patients were admitted to the study. Endoscopy was repeated if there was a suggestion of recurrent bleeding. The sensitivity of the system was 63%, the specificity 86%. Further improvement of the sensitivity could help avoid unnecessary endoscopies, as well as allow early diagnosis of recurrent hemorrhage.

Am Beispiel der Indikation zum operativen Eingriff beim chronischen Ulcus duodeni werden die Kennzeichen des Stellens der Operationsindikation erläutert: Ein Erkenntisprozeß und Sich-Entscheiden zur Operation, unter Bedingungen der Unsicherheit, zu einem momentanen Zeitpunkt nur mit einer Arbeitsdiagnose, mit dem Ziel des größtmöglichen Nutzens für den Patienten, unter Beachtung der Folgen der Folgen, und dies, bitte, möglichst schnell.
    Notes: Summary Several types of clinical studies provide important decision aids for surgical indications. They include quantitative data on outcomes and utility analysis, graphic illustration of the various possibilities for decisions by decision trees, controlled clinical trials with data used in decision trees and - if these trials fail for several reasons -carefully conducted and documented prospective and retrospective trials. Items for defining the term “surgical indication” were illustrated by constructing a decision tree for treatment of chronic duodenal ulcer.
