    Notes: Summary The effect of alloxan on insulin and glucagon secretion was investigated in perifused isolated rat islets. Five minutes of exposure to 1.4 mmol/l alloxan in a low-glucose medium (5.6 mmol/l) abolished subsequent leucine stimulated insulin and glucagon secretion. In a medium containing 19 mmol/l arginine and 3.3 mmol/l glucose, insulin secretion was only slightly diminished by alloxan pretreatment, whereas glucagon secretion was reduced to about 60% of controls. Exposure to alloxan in a high glucose medium (27.8 mmol/l) did not effect insulin or glucagon secretion.

    Notes: Summary A new neurological mutant mouse shows a gracile axonal dystrophy (GAD). The degenerative lesion develops by postnatal day 80, first appearing in the most rostral portion of the gracile fascicles. This lesion then extends caudally to involve the entire gracile fascicles. Many axonal swellings (dystrophies) also appear in the degenerative lesions in proportion to their severity. The clinical findings develop in keeping with these pathological changes, and are characterized by tremor, ataxia and difficulty in moving the hind limbs. These start around day 80, and progress gradually to death about day 150. The lumbar dorsal roots, their spinal root ganglia and peripheral nerves are normal. Electron microscopic study shows dystrophic axons packed with neurofilaments, mitochondria and tubulovesicular structures. These may reflect some stagnation of axonal transport. The distribution of the lesions suggest that the GAD mouse has a central distal axonopathy involving primary sensory neurons of the lumbar dorsal root ganglia.

    Notes: Summary Double tumors were found in the anterior and posterior lobes of pituitary gland at autopsy in a patient who presented with progressive deterioration of mental status. The chromophobe adenoma of anterior lobe consisted of a mixture of non-immunoreactive hormone containing cells and a few prolactin (PRL) and growth hormone (GH) cells in the mid portion whereas the periphery of the tumor contained immunoreactive cells for PRL, GH, thyroid-stimulating hormone (TSH), luteinizing hormone (LH), and follicle-stimulating hormone (FSH). A microscopic focus of granular cell myoblastoma (GCM) was found in the posterior lobe. Differentiation of tumor cells into anterior pituitary cells and GCM is discussed.

    Notes: Abstract A boy with homocystinuria due to cystathionine synthase deficiency was found to have hypermethioninaemia by neonatal blood screening, but was not diagnosed as homocystinuric until 3 months of age because urinary homocystine was not detected by the cyanidenitroprusside test or on two examinations with a sensitive amino acid autoanalyser. These findings indicate that tests for urinary homocystine should be made repeatedly with an amino acid auto-analyser in newborn infants with hypermethioninaemia until the enzyme defect is identified.

    Notes: Summary Fifty-one patients with medulloblastoma have been treated at the Children's Memorial Hospital during the past 11 years. The ratio between males and females was 2∶1. The ages ranged from 4 months to 12 years; 7 patients were less than 12 months old at the time of diagnosis. After 1969, prior to definitive surgery, all patients were given a ventriculo-peritoneal shunt to decompress the hydrocephalus which was invariably present. There were no cases of systemic metastasis related to these shunts. Subsequently, posterior fossa craniotomy was performed, with total or radical resection of tumour in 13 cases, subtotal resection in 20 cases, and partial resection or biopsy in 14 cases. After completion of surgical treatment and radiotherapy, the three-year survival rate was 45.6 per cent and the five-year survival 34.5 per cent. The survival of children given total resection of the tumour was significantly longer, and females survived longer than males. Local recurrence and spinal cord or systemic metastases are discussed. Reexploration of the posterior fossa for recurrences failed to improve the prognosis. Recurrent or metastatic medulloblastoma should be treated by radiotherapy or chemotherapy, or both.

    Notes: Summary Six patients with multifocal glioma are presented. Computerized tomography revealed multiple, discrete, contrast-enhancing lesions in the cerebral hemispheres, suggestive of multiple intracranial metastases. The most accessible lesion was resected at craniotomy in each patient, confirming the diagnosis of primary malignant glioma. Postoperative radiation therapy and chometherapy were instituted according to current protocols. Since neuroradiological studies may not allow distinction of multifocal gloima from multiple brain metastases, surgical biopsy is suggested in those patients who have no history of cancer.

    Notes: Summary A 12-year-old white male presented with an ossifying fibroma of the maxillary sinus. The tumour recurred following incomplete excision through a maxillary approach. Computerized tomography revealed an extensive tumour involving the paranasal sinuses, which extended intracranially through the cribriform plate. The tumour was successfully excised through a combined craniofacial approach. This approach should be used whenever there is radiological involvement of the base of the skull from paranasal sinus tumours. Accurate radiological delineation of the extent of the tumour is only possible by computerized tomography in the axial and coronal planes.
