    Notes: During central venous cannulation, percutaneous venous puncture is performed blindly using a variety of surface landmarks on the body for different puncture sites. To avoid vessel transfixion and injury to adjacent structures, the earliest detection of venous puncture is essential. A simple central venous puncture detector is described, which not only efficiently detects entry of the needle into the vein but also prevents air embolism and blood spillage.

    Notes: A method of fixing the epidural catheter by subcutaneous tunnelling and looping was devised. A prospective, randomised, double-blind, clinical trial was conducted in 68 adult patients, where postoperative pain relief was planned by thoracic epidural analgesia. In the tunnelled group (n = 34), the epidural catheter was fixed with a subcutaneous tunnel and loop, whereas in controls (n = 34), a simple loop of epidural catheter was left over the skin without tunnelling. An adhesive dressing was used to secure the epidural catheter. We observed that catheter dislodgement occurred in only one patient in the tunnelled group compared to seven control patients (21%). Despite local inflammation of the skin around the tunnel in nine patients (27%), no catheter infection (positive culture tip) was found in patients with subcutaneous tunnelling for the extended period of 4–5 days. The method described allows the catheter to lie flat on the skin and outward traction of the catheter during movement of patients is dampened by the interposed loop which protects it against dislodgement. At the time of removal, both ends of the catheter can be removed under direct vision. In conclusion, we recommend this fixation method in cases where epidural analgesia is to be used for postoperative pain relief.

    Notes: A number of percutaneous dilational tracheostomy devices are now available for clinical use. Recently, a new percutaneous dilational tracheostomy device, the ‘T-Dagger™’ (Criticure Invasives, India), has been introduced for rapid bedside percutaneous tracheostomy. In a prospective preliminary study, we have performed percutaneous dilational tracheostomy (PDT) using the T-Dagger™ in 20 adult ventilated patients in order to evaluate the safety and efficacy of the new device. The T-Dagger™ facilitated bedside PDT in about 3 min with no untoward incidents. There was no significant bleeding, pneumothorax, pneumomediastinum, tracheal wall injuries or difficulty in ventilation in any of the patients. We conclude that the T-Dagger™ shows early promise in bedside percutaneous dilational tracheostomy. However, controlled studies are required in a larger patient population before it can be recommended for routine use.

    Notes: Summary The endogenous monoamine oxidase (MAO) inhibitor (tribulin) activity of rat brain was investigated during the course of carrageenan induced acute paw inflammation. The increase in rat brain tribulin activity closely paralleled the time course intensity of the inflammation. The study indicates that, like externally induced stress, internal stress caused by the inflammation induced hyper algesia can also induce augmented brain tribulin activity. The findings, thus, support the hypothesis that tribulin may function as an endogenous endocoid marker of stress.

    Notes: Abstract A few linear voltage-controlled sinusoidal oscillators using IC op-amps (OAs) and IC analogue multipliers (AMs) in conjunction with passive components are already known. In present communication two new linear VCO circuits are proposed. The proposed configurations employ two IC OAs, two IC AMs and fewer passive components as compared to any other circuit of this class reported earlier. In these oscillators the condition of oscillation (CO) and frequency of oscillation (FO) are independently tunable. It has also been shown here that both the proposed circuits are reducible to single OA linear VCOs. The workability of all these configurations has been experimentally tested and the practical result conform with the theoretically obtained values.
