Summary The antibacterial activity of cephalothin, cefazolin, cefamandole and cefoxitin againstEnterobacteriaceae was investigated comparatively using the tube and the agar dilution tests. The beta-lactamase activity of the bacteria, which had been recently isolated from clinical specimens, was determined at the same time. Depending on the method, 4.4% and 2.6% respectively of 890Escherichia coli strains were resistant to cefoxitin. In vitro, cefoxitin is more effective than cefazolin and cefamandole against cephalothin-resistantKlebsiella sp. AgainstEnterobacter sp. the cephalosporins show the following activity: cefamandole 〉 cefoxitin 〉 cefazolin 〉 cephalothin. On determination of MIC values using the agar dilution test there was a statistically significant difference between beta-lactamase positive and negativeE. coli strains with cephalothin, but not with cefoxitin. The stability of cefoxitin and cefuroxime against beta-lactamases could be demonstrated by investigating the kinetics of the bactericidal effect of beta-lactamase producingKlebsiella strains. Widely varying latent periods of the various cephalosporins, fluctuating between 0.4 h for cefazedone and 1.7 h for cefuroxime, were observed.

Zusammenfassung Die antibakterielle Aktivität von Cefoxitin gegenüberEnterobacteriaceae wurde im quantitativen Reihenbzw. Agarverdünnungstest vergleichend mit Cephalothin, Cefazolin und Cefamandol untersucht. Gleichzeitig erfolgte die Bestimmung der Beta-Laktamase-Aktivität der frisch aus klinischem Untersuchungsmaterial isolierten Bakterien. Von den 890Escherichia-coli-Stämmen waren je nach Untersuchungsmethode 4,4% bzw. 2,6% gegenüber Cefoxitin resistent. Bei Cephalothin-resistentenKlebsiella sp. ist Cefoxitin in vitro besser wirksam als Cefazolin und Cefamandol. BezüglichEnterobacter sp. dagegen ergibt sich für die antibakterielle Aktivität folgende Reihe: Cefamandol 〉 Cefoxitin 〉 Cefazolin 〉 Cephalothin. Bei der Bestimmung der MHK mit dem Agardilutionstest zeigt sich ein statistisch signifikanter Unterschied bei Beta-Laktamase-positiven und-negativenEscherichia coli-Stämmen mit Cephalothin, dagegen nicht mit Cefoxitin. Die Beta-Laktamase-Stabilität von Cefoxitin und Cefuroxim wird besonders deutlich bei der Untersuchung der Kinetik der Bakterizidie von Beta-Laktamase-bildendenKlebsiella-Stämmen. Dabei wird auch eine sehr unterschiedliche Latenzzeit der einzelnen Cephalosporine festgestellt, schwankend von 0,4 h für Cefazedon bis 1,7 h für Cefuroxim.
