    Notes: 1 Electrical field stimulation (EFS) (0.5 ms in train of 2–32 Hz for 300 ms) in smooth muscle of pigeon oesophagus, in the presence of atropine (1 μm) and guanethidine (1 μm), elicited an inhibitory response consisting of a transient hyperpolarization (inhibitory junction potential, IJP) associated with muscle relaxation. 2 Sodium nitroprusside (SNP, 100 μm) induced hyperpolarization correlated to mechanical relaxation. 3 The nitric oxide (NO) synthase inhibitor Nω-nitro-l-arginine (from 0.1 to 100 μm) caused a concentration-dependent reduction of electromechanical response to EFS indicating a role for NO in this response. 4 Apamin (1 μm) reduced both IJP and relaxation to EFS but was without effect on the response to SNP indicating a role for purines, which are also blocked by apamin. 5 Adenosine, AMP, ADP and ATP (all from 1 μm to 1 mm) application caused transient hyperpolarization and muscular relaxation with the following order of potency: adenosine 〉 AMP 〉 ADP 〉 ATP. 6 Inhibitory responses evoked by purines are TTX (1 μm) insensitive but they were inhibited by apamin. This indicates that a purine component for the non-adrenergic non-cholinergic (NANC) response exists but the purine receptor site is not located on the neurone. 7 Overall these results suggest that NANC inhibitory response elicited by EFS presents two different components apamin-sensitive, probably purines-mediated and apamin-insensitive probably NO-mediated as apamin only partially block the response to EFS.

    Notes: Abstract Calcifying discopathy in infancy involving in the cervical spine has already been observed and described by many authors, as a well-defined clinico-radiological syndrome with a benign course. The clinical picture is composed of: pain and functional limitation, sometimes with a stiff neck, more rarely slight fever, increase of the erythrocyte sedimentation rate or leukocytosis. The radiographic picture consists of the association of morphological and structural alterations of vertebral bodies adjacent to one or more disc calcifications usually centrally sited, sometimes associated with anterior or posterior herniations. On the basis of the observation of 7 patients of up to 15 years of age, the authors propose to evaluate the changes of both the vertebral bodies and the discs involved in the disease over a period of time. The repetition of even modest alterations, that persist in time, testify to the involvement of the vertebral growth perhaps, more than the discal alterations connected with the calcification.

    Description / Table of Contents: Résumé La metrizamide, un nouveau produit de contraste hydrosoluble, a été utilisée chez 23 malades soumis à une myélographie cervicale ou dorsale. Pour la réalisation de la première, le produit a été injecté par ponction sous-occipitale; pour la deuxième il a été introduit par ponction lombaire. La metrizamide se mélange rapidement et uniformément avec le liquide céphalo-rachidien, donnant une bonne visualisation de la moelle et des racines. Etant donné sa basse toxicité, ce produit de contraste n'expose à aucune complication. L'effet secondaire que l'on observe le plus fréquemment est la céphalée, qui dans notre série est apparue à peu près chez 40% des malades, mais a été presque toujours légère.
    Notes: Summary Metrizamide, a new water-soluble contrast medium, has been used in 23 patients for cervical and dorsal myelography. It has been injected into the suboccipital space for cervical myelography and into the upper lumbar canal for thoracic myelography. Metrizamide mixes quickly and uniformly with the cerebrospinal fluid giving good visualization of the cord and the nerve roots. Owing to its low neuro-toxicity, this agent is free of serious complications. The most common side effect is headache, which in our series has occurred in about 40% of the patients, usually to slight degree.

Nous rapportons une étude prospective analysant si des possibles facteurs prédisposant à un spondylolisthesis dégénératif (DS) doivent être présents en même temps, afin de provoquer un glissement vertebral. Les clichés standards et la flexion-extension ont été obtenus sur 27 patients avec DS et 27 sans olisthésis. Le niveau de la ligne intercrestal, l’angle lombo-sacré, la présence de sacralization de L5 et la mobilité vertébral au niveau L4 – L5 ont été déterminés. L’orientation des facettes articulaires ont été mesurées sur des tomodensitométries. Seules l’orientation des facettes articulaires et la mobilité vertébrale au niveau olisthétique étaient différentes de fa*on significative chez les patients avec DS en comparaison avec le groupe de côntrole. Les facettes articulaires sont orientées plus sagittalement aux deux niveaux olisthétique et aux niveaux au-dessus et en-dessous. Une corrélation linéaire inverse fût trouvée entre l’orientation sagittale des facettes articulaires et de la mobilité de la vertébre déplacée. L’orientation anormale des facettes articulaires et l’hypermobilité de la vertébre olisthétique semble jouer un role important, prédisposant au DS. Les deux facteurs doivent être pris en consideration pour la planification d’un traitement chirurgical.
    Notes: Summary. We report a prospective study analysing whether possible factors predisposing to degenerative spondylolisthesis (DS) must be present concomitantly in order to cause vertebral slipping. Standard and flexion-extension radiographs were obtained from 27 patients with DS and 27 without spondylolisthesis. The level of the intercrestal line, the lumbosacral angle, the presence of sacralization of L5 and vertebral motion at the L4 – L5 level were assessed. Facet joint orientations were measured on CT scans. Only facet joint orientation and vertebral motion at the spondylolisthetic level were significantly different in patients with DS compared with controls. Facet joints were oriented more sagittally both at the spondylolisthetic level and at the levels above and below. An inverse linear correlation was found between the sagittal orientation of facet joints and the mobility of the slipped vertebra. Abnormal sagittal orientation of facet joints and hypermobility of the spondylolisthetic vertebra appear to play major roles among possible factors predisposing to DS. Both factors should be considered in the planning of surgical treatment.

    Description / Table of Contents: Résumé Chez douze malades présentant une symptomatologie évocatrice d'une hernie discale lombaire, on a découvert une tumeur intradurale. Des neuf patients porteurs d'un neurofibrome, deux avaient subi auparavant une laminectomie du fait d'une erreur de diagnostic avec une hernie discale et un avait trois tumeurs distinctes dont l'ablation nécessita deux interventions successives. Un des trois malades atteints d'épendymome avait subi une résection du coccyx avant que le diagnostic exact ait été posé. Un seul patient, porteur d'un épendymome, présentait des anomalies radiologiques évocatrices d'une lésion néoplasique, mais huit des dix sujets chez lesquels a été pratiqué l'examen du liquidecéphalo-rachidien avaient une albuminorachie supérieure à 50 mg pour 100 ml. La myélographie a permis un diagnostic correct dans tous les cas. L'éxérèse de la tumeur a toujours permis d'obtenir la guérison complète, sauf chez un malade porteur d'un épendymome.
    Notes: Summary Twelve patients with symptoms and signs simulating lumbar disc disease were found to have intradural tumours in the lumbosacral area. Of the nine patients with a neurofibroma, two had previously had a laminectomy for an erroneous diagnosis of disc herniation and one had three separate tumours which were excised in two successive operations. One of the three patients with an ependymoma had a coccygectomy before the correct diagnosis was made. Only one patient who had an ependymoma showed radiographic abnormalities suggesting a neoplastic lesion, but eight of the ten cases in which the cerebrospinal fluid was analysed had a protein content exceeding 50 mg per 100 ml. Myelography provided the correct diagnosis in all cases. Excision of the tumour resulted in full clinical recovery except for one patient with an ependymoma.
