    Notes: A patient is described who, despite severe pre-operative respiratory disability, had her persistent pneumothorax successfully managed by thoracoscopic pleurectomy. The technique causes considerably less pain and interference with respiratory function postoperatively than does conventional thoracotomy. Potential anaesthetic problems arise because of the necessity of insufflating carbon dioxide at pressures of up to 1 kPa to maintain a pneumothorax during surgery.

    Notes: In a double-blind placebo-controlled trial of patients undergoing elective abdominal surgery (n=91), a single intravenous infusion of ceruletide (2.5 ng kg−’min−1’for 1 hour) resulted in audible bowel sounds in 42/47 patients as opposed to 30/44 receiving placebo (P 〈 0.025). Excessive bowel sounds were noted in 16 patients in the ceruletide group and four receiving placebo (P 〈 0.01). Significantly more patients (P 〈 0.01) in the ceruletide group (22/45 versus 9/44) passed flatus per rectum between the second and third post-operative day. Ceruletide infusion was accompanied by a significant increase in the incidence of nausea and vomiting (P 〈 0.005, P 〈 0.0025) but these side effects were short-lived. These results indicate that ceruletide is likely to be a useful therapeutic agent for acute intestinal adynamic motility disorders.

    Description / Table of Contents: Résumé Les auteurs décrivent une technique particulière de cholédoco-duodénotomie qui consiste en la section transversale de la voie biliaire principale au niveau de son passage en arriére du pancréas et la constitution d'une anastomose cholédoco-duodénale terminolatérale. Ce type d'opération a été réalisé chez 46 malades au cours d'une période supérieure à 14 ans, 26 opérés ayant été suivis plus de 5 ans. Aucun cas de sténose postopératoire de l'anastomose n'a été constaté. Un malade seulement a présenté un épisode ictérique 3 ans après l'intervention.
    Abstract: Resumen Se describe una técnica de coledocoduodenostomía que implica la transección del canal colédoco a nivel de su unión con el segmento trans-pancreático y anastomosis término-terminal con el duodeno. La operación ha sido realizada en 46 pacientes sobre un período de 14 años, con 26 pacientes seguidos por períodos de 5 años o más. No se ha presentado caso alguno de estenosis anastomótica en la serie, aun cuando un paciente tuvo un episodio único de ictericia 3 años después de la operación.
    Notes: Abstract A technique of choledochoduodenostomy involving transection of the bile duct at its junction with the trans-pancreatic segment and an end-to-side anastomosis with the duodenum is described. The operation has been performed in 46 patients over a period of 14 years, 26 patients having had a follow-up of 5 years or more. There has been no instance of anastomotic stenosis in the series, although one patient had a single episode of jaundice 3 years after operation.

    Notes: Summary The effects of colchicine on the mouse gallbladder followed a course depending on the dosage given (0.4–4 mg/100 g body weight). Following 0.5 mg/100 g, by 16 h there was a marked cholestasis with dilatation of the gallbladder and steatosis. There were progressive alterations in the Golgi apparatus and accumulation of vesicles. The apical mucous droplets decreased in number and became pleomorphic and dispersed throughout the cytoplasm. Lipid droplets appeared in numbers on the epithelial cytoplasm. By 48 h the tissues had reverted to normal appearances. When cholecystokinin, pilocarpine or ceruletide were given to animals which had received colchicine 18 h previously, the excess bile from the dilated gallbladder was discharged into the duodenum, remaining apical mucous droplets secreted and electron dense material accumulated in the lateral intercellular space. This formed a quasi-regular array between the epithelial bases and the basement membrane. Biochemically there was a significant decrease in alkaline phosphatase activity and a significant increase in acid phosphatase activity.

    Notes: Summary The occurrence of secondary deposits in the esophagus from cutaneous melanoma has rarely been reported in medical literature. Since esophageal deposits may present as dysphagia, it is important that the clinical picture be defined. Such a case is presented, and the typical radiological appearance described. Surgical treatment for secondary melanoma deposits may be dictated by the need for palliation of symptoms, but the long-term results are poor.

    Notes: Abstract As part of a prospective diagnostic protocol, patients suspected of having pancreatic cancer had systemic and portal venous blood samples assayed, in coded batches, for peptide hormones and enzymes thought to be of potential value as tumor markers. An average of 111 patients were tested for each candidate marker. Results were analyzed by dividing patients into three groups according to the definitive diagnoses. These were pancreatic cancer (32% of patients), other cancers (27%), and benign diseases (41%). Although elevated mean levels of fasting plasma glucose and serum alkaline phosphatase were found in the pancreatic cancer group, there were no significant differences in the mean levels of any of the candidate markers studied in the three groups. The diagnostic values of normal and elevated levels of each candidate marker studied have been calculated. None has proven to be as useful as the serum level of pancreatic oncofetal antigen, fasting plasma glucose, or serum alkaline phosphatase in the diagnosis or exclusion of pancreatic cancer.
