    Notes: Summary An excessive glucagon secretion to intravenous arginine infusion was found in obese hyperinsulinaemic patients with glucose intolerance. This study was designed to determine whether the glucagon hyperresponsiveness to arginine in these patients would improve by insulin infused at a high enough dose to overcome insulin resistance. By infusing high dose insulin during arginine infusion, the previously exaggerated glucagon response to arginine could be normalized. To normalize the abnormal glucagon response, insulin doses of 4.2±0.7 and 3.8±0.5 IU were required during arginine infusion in obese hyperinsulinaemic patients with impaired glucose tolerance and Type 2 (non-insulin-dependent) diabetes mellitus, respectively. This achieved plasma peak insulin levels 3 to 4 times higher than those observed in non-obese healthy subjects. Furthermore, we clarified whether or not the effect of normalizing insulin action and/or glycaemic excursions contributed to normalizing the exaggerated glucagon response to arginine in these patients. Blood glucose was clamped while high dose insulin was infused at the same levels as observed during the arginine infusion test with no insulin infusion. As a result, normalization of the exaggerated plasma glucagon response was achieved, whether hyperglycaemia existed or not. These results clearly demonstrate that, similar to non-obese hypoinsulinaemic Type 1 (insulin-dependent) and Type 2 (non-insulin-dependent) diabetic patients, the exaggerated Alpha-cell response to arginine infusion in obese hyperinsulinaemic patients with glucose intolerance is secondary to the reduction of insulin action on the pancreatic Alpha cell, and that the expression of insulin action plays an important part in normalizing these abnormalities.

    Notes: Abstract Background: Patients with papillary adenocarcinoma survive longer than do patients with other histologic types of bile duct tumors. We evaluated the usefulness of intraductal ultrasonography (IDUS) for predicting the histology. Methods: Preoperative tumor assessment was performed by using IDUS through a percutaneous tract or the transpapillary route in 37 patients with extrahepatic bile duct cancer. In 30 of 37 patients, imaging results were compared prospectively with histologic findings in resected specimens. Probes 2.0 mm in diameter and 20 MHz in frequency were mainly used. When IDUS showed a “narrow-based polypoid pattern” or a “papillary surface pattern,” the patients were judged as having papillary adenocarcinoma. Results: The accuracy, sensitivity, and specificity of IDUS in predicting papillary adenocarcinoma were 90%, 89%, and 90%, respectively. When intraductal ultrasonography showed a papillary surface pattern or a narrow-based polypoid pattern, lymph node metastases and perineural invasion were rarely seen when compared with other patients with bile duct cancer (p 〈 0.05). Conclusion: IDUS is useful for assessing the histologic type of bile duct cancer.

    Notes: Abstract Background: To identify the appropriate puncture points in the bile duct to avoid catheter dislodgement. Methods: Percutaneous transhepatic biliary drainage catheters (n = 300) were placed in 242 patients. The frequency of dislodgement (complete dislodgement or bending of the catheter) was prospectively investigated. The puncture site of the bile duct was classified on the ultrasonographic findings as follows: Main-B3, main branch of the lateral inferior segment; peripheral-B3, peripheral branch of the lateral inferior segment; B2, lateral superior segment; left hepatic duct, proximal portion of the left hepatic duct; B8, anterior superior segment; B5, anterior inferior segment; B5 + 8, main bile duct of the anterior segment; B6, bile duct of posterior inferior segment; and right hepatic duct, proximal portion of the right hepatic duct. Results: When a catheter without an outer sheath was used, catheter dislodgement in peripheral-B3 (2/11, 18%) was more common than in main-B3 (0/32, 0%; p 〈 0.05). In B5, catheter dislodgement (6/12, 50%) was more frequent than in B8 (3/20, 15%; p 〈 0.05) and in B6 (0/14, 0%; p 〈 0.005). When a catheter with an outer sheath was used, catheter dislodgement (2/207, 1%) was rare. Conclusion: Drainage from B5 and peripheral-B3 is associated with a high risk of dislodgement of the catheter. A catheter with an outer sheath was useful to prevent catheter dislodgement.     Notes: Abstract Hereditary ceruloplasmin deficiency with hemosiderosis (aceruloplasminemia) is a new disease characterized by systemic hemosiderosis, diabetes mellitus, neurological abnormalities and pigment degeneration of the retina. Loss of the ferroxidase activity of ceruloplasmin results in systemic iron deposition and tissue damage. Neuroimaging studies reveal iron deposition in basal ganglia and in the red and dentate nuclei. Cerebellar ataxia, extrapyramidal signs and dementia develop after middle age. We report a patient with undetectable serum ceruloplasmin levels and the above clinical manifestations. Sequence analysis of the cDNA of ceruloplasmin from this patient revealed an insertion of adenine in exon 3; this produced a premature stop codon.

    Notes: Abstract Gilbert syndrome was diagnosed in a girl with anorexia nervosa and unconjugated hyperbilirubinaemia. Since the patient was starved and hyperbilirubinaemic, the loading test was not used for the diagnosis but analysis of the bilirubin UDP-glucuronosyltransferase gene (UGT1A1) instead. The patient was homozygous for a missense mutation that replaced guanine with adenine at nucleotide number 211 (211G→A: G71R). The unconjugated hyperbilirubinaemia was apparently induced by the fasting state. Homozygous missense mutations of the gene have been generally recognized as responsible for Crigler-Najjar syndrome type II; the results obtained here, however, confirm that Gilbert syndrome may also be caused by a homozygous missense mutation of UGT1A1. Conclusion Since anorexia nervosa patients are in a fasting state, they may show moderate unconjugated hyperbilirubinaemia if they have Gilbert syndrome. Gene analysis of such cases will rule out hepatic damage. Homozygous missense mutations of the bilirubin-UDP-glucuronosyltransferase gene cause not only Crigler-Najjar syndrome type II but also Gilbert syndrome.

    Notes: We have developed a new electron–ion coincidence apparatus combined with synchrotron radiation in order to examine the various ion desorption mechanisms related to the Auger process induced by core excitation. Photon stimulated ion desorption (PSID) of a poly-methylmethacrylate (PMMA) thin film has been investigated by this apparatus. The PSID of PMMA induced by carbon core excitation has been examined using Auger electron yield, total ion yield, resonant Auger electron, and Auger electron–photoion coincidence (AEPICO) spectra. The spectrum of the total ion yield divided by the Auger electron yield shows that the desorption efficiency is largely increased at the resonant excitation of carbon 1s electron in the O–CH3 side chain to σ*(O–CH3) orbital. In AEPICO measurement, H+ and CHn+ (n=1–3) ions are observed at various resonant excitations. The AEPICO signal intensity depends on the Auger electron energy. Particularly, the CH3+ ion desorption in coincidence with Auger electron at 270 eV shows strong enhancement with σ*(O–CH3) resonant excitation. The results of the resonant Auger spectra and AEPICO yield spectra demonstrate the relation of the ion desorption mechanism to the bonding/antibonding character and localized character of the excited σ*(O–CH3) orbital and the Auger final state. © 2001 American Institute of Physics.

    Notes: This study was conducted to isolate the constituent, which had a preventive effect on D-galactosamine-induced rat liver injury, from the 70% ethanol-soluble fraction of Japanese green tea. Theanine (glutamic acid γ-ethylamide) was identified as the active compound, and the liver injury-preventive effect of theanine was dose-dependent. L-Glutamic acid γ-ethyl ester, but not glutamine, also brought about a significantly preventive effect on liver injury when added to the diet at equimolar levels to that of 1% theanine. The results indicate that theanine is one of the effective constituents of Japanese green tea in preventing D-galactosamine-induced liver injury.

    Notes: New meat products were developed from beef using the process of dried-skipjack (katsuobushi) manufacturing, which employs boiling, smoke drying and molding. Boiling and molding affected the constituent nitrogen compounds of the meat. These changes affected the flavor of the new meat products. Lipid constituents were altered in the molding process, where lipase produced from the mold increased the amount of free fatty acids, which produced a mild flavor in the new product. Differences in the processing method produce a difference in the organoleptic acceptance and flavor.In particular, the molding process was responsible for production of a mild sourness and acceptable flavor.

    Notes: The effects of spawning migration and extrusion cooking on o-3 polyunsaturated fatty acids (PUFAs) in chum salmon muscle were studied. Total lipid content in the muscle of the fish during spawning migration was lower than that during feeding migration. However, the content increased to double those in the raw materials by pretreatment of extrusion cooking. Lipids of muscle and outer layer during spawning migration contained higher percentages of docosahexaenoic acid (22:6) than those during feeding migration. Moreover, PUFAs were not lost by extrusion cooking and its pretreatment. These results suggest that production of foods containing a rich amount of W-3 PUFAs is possible by extrusion cooking of the muscle of chum salmon during spawning migration.
