    Notes: Abstract: The potential use of autologous thrombocytic growth factors to accelerate bone regeneration requires improved methods of isolating platelet-rich plasma (PRP). In addition to discontinuous cell separation, a second method by which PRP is produced at the point-of-care has now become available. In this study, growth factor levels in PRP from these two sources were compared. Whole blood was drawn from 115 healthy donors (73 males, 42 females) aged 21 – 62 years (mean 36, SD 10). The PRP was separated by the blood bank (BB) using the discontinuous cell separation method or at the ‘point-of-care’ by the so-called ‘buffy coat’ method (analogous to the Curasan PRP Kit). Growth factor content differed significantly for TGF-β1 (BB 268.65±70.77 ng/ml, Curasan 95.02±60.67 ng/ml (sign test P〈0.001)) and PDGF-AB (BB 133.59±46.26 ng/ml, Curasan 233.70±111.86 ng/ml (P〈0.001)), while the content of IGF-I (BB 85.37±25.58 ng/ml, Curasan 101.72±47.7 ng/ml (P〈0.160)) showed no significant difference. The higher thrombocyte count in the BB PRP (BB 1434300±351960/μl, Curasan 908.500±492.30/μl) seems to result in higher TGF-β1 levels, while the higher leukocyte count in the Curasan PRP (BB 160±320/μl, Curasan 30130±12500/μl) seems to result in higher PDGF-AB levels. The similar IGF-I levels in the two preparations might merely reflect similar amounts of plasma in the PRP produced by each approach.

    Notes: Objectives: Distraction osteogenesis has recently evolved a challenging technique to overcome major drawbacks of conventional augmentation procedures. We, therefore, report the application of miniaturized intraosseous distraction devices for the rehabilitation of mandibular defects due to ablative tumor surgery.Methods: In 10 patients who had undergone box-shaped or segmental resections, augmentation of the residual mandibular bone or of full thickness iliac crest grafts was performed by intraosseous implant-shaped distractors. Distraction and reconsolidation was monitored by ultrasonography. Implants were inserted within 1 week after active distraction. Median follow-up after implant insertion was 38 months.Results: On average, a vertical gain of 7.3 mm was obtained by distraction. Except for one case (local infection), all distraction zones showed complete ossification by radiologic and ultrasonographic evaluation. Overall 28 implants were placed in the distracted bone. Two implants were lost at 2.4 and 22 months after placement. The estimated 4-year implant survival rate in this population was 90%. For the endpoint ‘bone loss’ (〉1.5 mm in the first and 〉1 mm in following years), the estimated 4-year success rate was 59% with four out of seven events occurring in a single patient (patient No. 1 of this series).Conclusion: Vertical distraction by means of implant distractors could be performed with reasonable success in tumor patients with box-shaped resection defects or undercontoured bone grafts. Overall morbidity was very low. Even though blood supply is continuously maintained in distraction osteogenesis, bone resorption remains a critical issue for this reconstruction technique too.

    Notes: Summary Fibro-osteo-cemental lesions of the jaw bones are a heterogeneous group of diseases which present problems in classification. Psammous desmo-osteoblastoma is one of four newly proposed entities (Makek 1983) and has until now been characterized by its light microscopic, clinical and radiological features. On electron microscopy this tumour exhibits fibroblastic (preosteoblastic), osteoblastic and osteocytic cells and a globular mineralization unlike the mineralization of the psammoma bodies. Immunohistological investigations with anti-osteonectin, a bone specific protein linking mineral to collagen, showed positive intracellular staining in all tumour cells and extracellular staining in the osteoid. The psammoma bodies were, however, not stained. These results confirm the view of the osteogenic histogenesis of psammous desmo-osteoblastoma, with an osteogenic differentiation of the tumour cells, bone formation and association of psammoma bodies which are not of bone origin. This combination of findings supports the view that psammous desmo-osteoblastoma represents a new and distinct entity occuring in desmal preformed cranio-facial bones which should be incorporated in a revised WHO-classification.

    Description / Table of Contents: Zusammenfassung Fragestellung: Untersuchung einer akustischen Erfassung der komplexen strukturellen Anomalien bei Lippen-Kiefer-Gaumenspalt-assoziierten Nasendeformitäten und ihre Auswirkung auf die Planung von operativen Korrekturen. Patienten: 23 Patienten mit einseitigen LK(G)-Spalten im Rahmen der Routinesprechstunde, drei davon vor Nasenkorrekturen. Messmethode: Akustische Vermessung der Querschnittsfläche des nasalen Luftweges vor und nach Schleimhautabschwellung mit 0,3 mg Xylometazolin pro Nasenseite. Bestimmung der Querschnittsfläche und Lage des Isthmus, sowie des endonasalen Volumens in verschiedenen Abschnitten des nasalen Luftweges. Ergebnisse: Auf der Spaltseite fanden sich bei 85% Stenosen unter 0,4 cm2 Restfläche, auf der Gegenseite nur bei 15%. Eine starke Schleimhauthypertrophie war typisch auf der Spaltseite, mit einer deutlich höheren Abschwellkapazität, besonders im hinteren Teil der Nasenhöhle (P 〈 0,02). Eine zweite Stenose hinter dem physiologischen Isthmus fand sich bei 16 Patienten. Schlußfolgerung: Die akustische Rhinometrie ist eine nicht invasive, objektive Methode, die es erlaubt, Grad und Lage von Hindernissen im Luftweg zu bestimmen. Die einzige Voraussetzung für die Durchführung ist, neben der apparativen Ausstattung, ein kooperativer Patient. Die Messung hat eine direkte therapeutische Konsequenz. Mit Kenntnis der exakten Ursache einer Luftwegsstenose und ihrer genauen Lage ist es leichter, die notwendige Korrekturoperation zu individualisieren. Die Entscheidung für zusätzliche chirurgische Maßnahmen kann abgesichert werden, und postoperative Reststenosen lassen sich besser vermeiden.
    Notes: Summary Objective: To investigate acoustic assessment of complex structural anomalies in cleft lip and palate associated nasal deformities and its impact on the planning of corrective rhinosurgery. Patients: Twenty-three consecutive patients with unilateral CL(P) during routine consultations, three of them prior to corrective surgery. Intervention: Acoustic measurements of the nasal airway cross-sectional area were obtained for both nasal cavities. The size of isthmus area and isthmus location as well as the nasal volume and decongestion capacity in various regions of interest were determined. Results: Obstructions in the area of the nasal valve (〈 0.4 cm 2 ) were detected in 85% of the patients on the cleft side, but only 15% on the contralateral side. Severe mucosal hypertrophy was a common finding on the cleft side with a significantly higher capacity of decongestion, especially in the posterior region of the nasal cavity (P 〈 0.02). A second stenosis located behind the physiological isthmus was found in 16 patients. Conclusion: Acoustic rhinometry is a noninvasive, objective method. It measures the degree and the exact location of airway obstructions. Apart from the equipment, the only prerequisite is a cooperative patient. The measurement has direct therapeutic consequences. If the exact cause and location of airway stenosis are known, it is easier to adapt the corrective surgical procedures to suit the individual patient. The decision in favour of additional therapeutic measures can be made on a more rational basis to prevent residual airway problems postoperatively. Preoperative acoustic rhinometry offers valuable information concerning the severity and location of nasal airway obstructions and thus proves to be a helpful tool in the planning of corrective operative measures.

    Description / Table of Contents: Zusammenfassung Fragestellung: Es wird untersucht, ob die akustische Pharyngometrie Unterschiede der velopharyngealen Muskelfunktion zwischen Patienten mit Lippen-Kiefer-Gaumenspalten und gesunden Probanden aufzeigen kann. Als Maß der Funktion wird die epipharyngeale Volumenveränderung bei aktiver Tonisierung des Velums gemessen. Studien-Design: Fall-Kontroll-Studie. Studieneinrichtung: Behandlungszentrum für Lippen-Kiefer-Gaumenspalten. Patienten: Untersuchungsgruppe: 33 Pat. mit (LK)G-Spalten, darunter 10 nach Velopharyngoplastik. Kontrollgruppe: 32 Patienten ohne velopharyngeale Erkrankungen. Messmethode: Transnasale akustische Vermessung des Atemwegsquerschnitts bei entspannter und tonisierter Muskulatur des velopharyngealen Verschlußapparates. Bestimmung der epipharyngealen Volumenänderungen durch Integration der Differenz der Atemwegsprofile ab den Choanen über eine Eindringtiefe von 5 cm. Eine Referenzierung erfolgte durch Messung defininierter epipharyngealer Volumenänderungen über Ballonkatheter. Ergebnisse: Durch akustische Pharyngometrie konnten Unterschiede der Velumbeweglichkeit (P 〈 0,05; U-test, Mann & Whitney) zwischen Spaltpatienten (6,5 cm3) und Kontrollgruppe (8,0 cm3) anhand der Volumenveränderungen aufgezeigt werden. Das Bewegungsmuster wird durch die Velopharyngoplastik deutlich verändert. Schlußfolgerung: Die akustische Pharyngometrie erscheint grundsätzlich geeignet, die velopharyngeale Muskelfunktion anhand der Volumenveränderung des Epipharynx zu erfassen. Diese nicht invasive Untersuchungsmethode könnte klinische Bedeutung in der objektiven Therapiekontrolle bei velopharyngealen Funktionsstörungen gewinnen.
    Notes: Summary Objective: To investigate whether acoustic pharyngometry is capable of discriminating velopharyngeal mobility in cleft and non-cleft subjects by determination of epipharyngeal volume changes with active muscle function. Design: Case control study, consecutive sample. Setting: Cleft palate rehabilitation centre. Patients: Thirty-three consecutive cleft palate patients, among them 10 following pharyngeal flap surgery and 32 controls without velopharyngeal pathology. Intervention: Transnasal acoustic measurements of airway cross-sectional area in the state of relaxed and tensed pharyngeal muscles. Determination of epipharyngeal volume changes by integrating the difference of the airway profile from the choane for a distance of 5 cm (EV 0–5 ). Measurements of defined changes in the epipharynx served as reference. Results: Acoustic pharyngometry is capable of discriminating (P 〈 0.05; U test, Mann-Whitney) the volume effect of velopharyngeal mobility in CP patients (6.5 cm 3 ) from the control group (8.0 cm 3 ). A ‘pharyngeal flap type’ and a ‘non-pharyngeal flap type’ of restriction was observed. The individual effect of velopharyngoplasty on pharyngeal mobility can thus be determined. Conclusion: Acoustic pharyngometry is a non-invasive, quantitative investigation technique which seems well suited for the evaluation of velopharyngeal mobility. We expect it to be a helpful tool in objectively monitoring the effect of therapeutic intervention on velopharyngeal mobility, and it may provide a better understanding of the pattern of movement in CP patients.

    Description / Table of Contents: Summary Aims: The predominant cause of death due to oral cancer is the failure to control local tumor due to regional tumor recurrence. The sequelae of surgical resection and high-dose irradiation cause substantial changes in head and neck anatomy, leading to considerable problems in the early morphological detection of recurrent disease. Therefore, this study evaluates the verification of cancer recurrence by means of its pathologic glucose metabolism. Materials and methods: We reviewed a total of 50 [18F]-2-fluordeoxyglucose positron emission tomography (18FDG-PET) investigations performed in 44 patients who had undergone surgical resection of oral cancer. In 23 cases, re-staging (group A) was indicated due to suspicion of recurrent or secondary tumor manifestation. In 27 cases, PET served as a screening procedure (group B). Statistic evaluation included sensitivity, specifity, positive/negative predictive value and accuracy of 18FDG-PET for the detection of tumor manifestation. Results: 18FDG-PET correctly identified 23 of 26 tumor sites (88%) in the re-staging group and 9 of 10 tumor sites (90%) in the screening group. We encountered a total number of 16 false-positive foci with an increased 18FDG-uptake. In six patients, 18FDG-PET detected tumor recurrence several months before a morphological correlative could be identified. In 5 of these 6 patients, the PET findings for the latter tumor sites determined the patient’s fate. Specificity was 63% for local recurrence, 97% for secondary lymph node involvement and 90% for distant metastasis. Conclusion: According to these data, 18FDG-PET is the most effective diagnostic tool in the follow-up of oral cancer patients to date. Due to the high prevalence of recurrent disease in the follow-up of oral cancer, either the detection of early recurrence or the identification of additional, incurable tumors may add substantially to a rational therapeutic management. We therefore recommend 18FDG-PET for screening and re-staging of recurrent oral cancer.
    Notes: Zusammenfassung Fragestellung: Die Letalität des Mundhöhlenkarzinoms resultiert überwiegend aus sekundären, regionalen Tumormanifestationen. Therapiebedingte Veränderungen der Anatomie erschweren die klinische Tumornachsorge und Interpretation morphologisch bildgebender Verfahren so erheblich, dass die Dignität suspekter Gewebeformationen häufig nicht zuverlässig beurteilbar ist. Die vorliegende Studie untersucht daher die Wertigkeit eines Nachweises sekundärer Tumormanifestationen anhand ihrer Glukosestoffwechselaktivität. Material und Methode: Bislang wurden 50 PET-Untersuchungen mit [ 18 F]-2-Fluordeoxyglukose ( 18 FDG) bei 44 Patienten nach primär kurativ intendierter Therapie von Mundhöhlenkarzinomen vorgenommen. Bei 22 Patienten (Gruppe A) wurden 23 Untersuchungen als Rezidiv-Staging durchgeführt. 27 Untersuchungen wurden als Screening-Maßnahme (Gruppe B) eingesetzt. Für den Nachweis von Tumorgewebe wurden Sensitivität, Spezifität und die positive bzw. negative Prädiktion bestimmt. Ergebnisse: Bei 18 von 22 Patienten aus Gruppe A wurden von 26 Tumormanifestationen 23 durch die PET dargestellt (Sensitivität 88%). Bei 6 Patienten aus Gruppe B wurden im PET-Screening 9 von 10 Tumorlokalisationen erkannt. Insgesamt wurden 16 falsch-positive Anreicherungen auffällig. In 6 Fällen konnte die PET Tumorformationen bereits Monate vor dem Nachweis eines morphologischen Korrelats aufzeigen. Die zusätzlichen Befunde erwiesen sich bei 5 dieser 6 Patienten als prognosebestimmend. Die Spezifität des Tumornachweises lag für Lokalrezidive bei 63%, für Lymphknotenfiliae bei 97% und für Fernmetastasen bei 90%. Schlussfolgerungen: Nach diesen Ergebnissen stellt die 18 FDG-PET das derzeit effektivste Diagnoseverfahren in der Nachsorge des Mundhöhlenkarzinoms dar. Bei einer Tumorprävalenz um 60% in der untersuchten Risikogruppe ist mit einer substanziellen Verbesserung der Früherkennung und auch mit einer rechtzeitigen Darstellung therapielimitierender Befunde zu rechnen. Die vorliegenden Daten rechtfertigen daher eine Ausdehnung der Indikation zum ¶Screening und zum Rezidiv-Staging im Rahmen der onkologischen Nachsorge des Mundhöhlenkarzinoms.

    Description / Table of Contents: Aim: Prospektive, randomized placebo-controlled double-blind study to prove the efficacy of Coumarin/Troxerutine (Venalot® Depot) for protection of salivary glands during a head and neck irradiation. Patients and Method: Forty-eight radiotherapy patients (60 Gy) with head and neck cancer were included in this trial. During radiotherapy the salivary glands were located in the core irradiation field. Primary efficacy parameters were sialometry, quantitative salivary gland scintigraphy and clinical evaluation of early effects of radiotherapy (RTOG-score, Table 1). All data were collected at 6 assessments: 1 week pre-radiation (U1), at start (U2), half time (U3) and end (U4) of irradiation, 8 days (U5) and 28 days (U6) after the end of irradiaten (Figure 1). Results: Twenty-three patients (11 verum, 12 placebo) completed the study with all assessments. Sialometrically, all patients were severely (half of radiotherapy) or completely (end of radiotherapy) xerostomatic (Figure 2). In a global efficacy measure according to O'Brien combining scintigraphy and RTOG there was a tendency for a higher efficacy of verum compared to placebo (p = 0.068). After start of irradiation therapy, the RTOG-score showed continuously and significantly lower early radiation effects under verum than under placebo (U3 vs U6: p 〈 0.05, area under curve: p = 0.032; Table 2, Figure 3). The scintigraphically determined excretion fraction was slightly less impaired in the verum group compared to the placebo treatment (p = 0.12. Figure 4). There was no difference in drug safety between placebo and verum for adverse events, changes in the activity of liver enzymes and for global impression of tolerability. Conclusions: The results give support for an advantageous effect of Venalot® Depot in the treatment of radiogenic sialadenitis and mucositis. In even a small number of evaluable patients, early clinical effects of irradiation (RTOG-score) were less pronounced in the active treatment group than in the placebo group, but the sample size was too low to prove statistically also the benefit of coumarin/troxerutine with the scintigraphic method. Sialometry seems not suitable for the assessment of early radiation effects.
    Notes: Fragestellung: Eine kausale Behandlung der radiogenen Sialadenitis ist bislang nicht etabliert. In einer prospektiven, randomisierten, plazebokontrollierten Doppelblindstudie sollte die pharmakologische Wirksamkeit von Cumarin/Troxerutin (Venalot® Depot) zur Speicheldrüsenprotektion unter einer Kopf-Hals-Bestrahlung bei Malignompatienten überprüft werden. Patienten und Methode: 48 Patienten mit Strahlentherapie (60 Gy) eines Kopf-Hals-Malignoms wurden in die Untersuchung eingeschlossen. Die Speicheldrüsen lagen überwiegend im Herdvolumen der Bestrahlung. Primäre Zielparameter waren Sialometrie, Speicheldrüsenfunktionszintigraphie und frühe Strahlenfolgen (RTOG-Score). Alle Befunde wurden zu sechs Untersuchungszeitpunkten erhoben: eine Woche vor Bestrahlung (U1), zu Beginn (U2), zur Hälfte (U3) und am Ende der Bestrahlung (U4) sowie sieben Tage (U5) und 28 Tage post radiationem (U6). Ergebnisse: 23 Patiente (elf mit Verum-, zwölf mit Plazebobehandlung) beendeten die Studie prüfplankonform mit allen Untersuchungen. Sialometrisch zeigten alle Patienten ab der Hälfte der Strahlentherapie (U3) stark verminderte Mundflüssigkeitsfließraten, danach betrugen die Mediane der Flußraten 0 ml/min für beide Gruppen bis zur U6 (28 Tage nach Bestrahlung). Im Gesamtwirkungsmaß nach O'Brien aus Sialoszintigraphie und klinischer Beurteilung war eine tendenzielle Überlegenheit von Verum gegenüber Plazebo zu beobachten (p = 0,068). Der RTOG-Score zeigte bei allen Untersuchungen nach Bestrahlungsbeginn unter Verum signifikant niedrigere Befundausprägungen früher Strahlenfolgen als unter Plazebo (U3 bis U6: jeweils p 〈 0,05; “Area under the Curve”: p = 0,032). Die szintigraphisch bestimmte Exkretionsfraktion zeigte für Verum eine tendenziell geringer Funktionsminderung der Speicheldrüsen als für Plazebo (p = 0,12). In der Arzneimittelsicherheit bestanden keine Unterschiede zur Plazebogruppe bezüglich der Rate unerwünschter Ereignisse, der Veränderungen leberspezifischer Enzymaktivitäten und der Gesamtbeurteilungen zur Verträglichkeit. Schlußfolgerungen: Die Ergebnisse verweisen auf einen Vorteil von Venalot® Depot gegenüber Plazebo in der Behandlung der radiogenen Sialadenitis und Mukositis. Frühe Strahlenfolgen (RTOG-Score) waren schon bei geringer Fallzahl unter Verum signifikant geringer ausgeprägt. Für den Nachweis von Therapiewirkungen in der Szintigraphie ist die erreichte Fallzahl offensichtlich zu niedrig. Die Sialometrie eignet sich zur Beurteilung früher Strahlenfolgen nicht.

    Notes: Abstract Between 1987 and 1991 we performed a unilateral or bilateral frontoorbital advancement to correct trigonoor plagiocephaly in 10 children: Three-dimensional CT provides an exact basis for operation planning. Titanium miniplates allow an already primarily rather stable osteosynthesis. The best time for this intervention is the end of the third month of life.
