    Notes: Summary Cell-free translation of salivary gland RNA or of purified Balbiani ring RNA (75S) in a reticulocyte lysate system gives rise to high molecular weight translational products (HMTP). In addition to a common size (approx. 1×106 daltons) HMTP share imunogenic determinants with the giant secretory proteins of salivary glands. This suggests that HMTP correspond to in vivo secreted proteins and thus, corroborates the notion that 75S-RNA is the messenger for these proteins. The time course of HMTP synthesis and the lack of appearance of lower molecular weight components as translational products of 75S-RNA indicate that the synthesis of HMTP (and of secretory proteins) occurs in one piece by an uninterrupted process. HMTP are regarded the largest polypeptides so far synthesized in a cell-free system.

The preform of the rabbit sterol carrier protein 2 (pre-rSCP2) was cloned, the uniformly 15N-labelled protein expressed in Escherichia coli and studied by three-dimensional 15N-resolved nuclear magnetic resonance spectroscopy. In spite of its low solubility in aqueous solution of only ∼0.3 mM, sequential 15N and 1H backbone resonance assignments were obtained for 105 out of the 143 residues. From comparison of the sequential and medium-range nuclear Overhauser effects (NOEs) in the two proteins, all regular secondary structures previously determined in mature human SCP2 (hSCP2) [Szyperski et al. (1993) FEBS Lett. 335: 18–26] were also identified in pre-rSCP2. Near-identity of the backbone 15N and 1H chemical shifts and 1 : 1 correspondence of 24 long-range NOEs to backbone amide groups in the two proteins show that the residues 21 – 143 adopt the same globular fold in pre-rSCP2 and mature hSCP2. The N-terminal 20-residue leader peptide of pre-rSCP2 is flexibly disordered in solution and does not observably affect the conformation of the polypeptide segment 21 – 143.

    Notes: Conclusions Our studies showed that at very rapid cooling rates of more than 100° C. per second in liquid helium the platelets are damaged and after thawing do not circulate in the peripheral blood. We don't have any explanation for this. But it seems that ice crystals are not the only reason for cell damage. The experiments with different thawing rates indicate that approximately 45° C. per second is optimum. While glycerol in a concentration of 10 per cent was not suitable for preservation of rabbit platelets at the high freezing and thawing rates used, the results with DMAC were encouraging. Toxicity of DMAC for human platelets and for animals and man should be determined. In addition, it should be clarified whether human platelets behave similarly to those of the rabbit frozen in liquid nitrogen without protective agents. If platelets could be frozen without cryophylactic additives, all questions of toxicity both to the platelets and the recipient would be eliminated.

    Description / Table of Contents: Abstract Gamma-hydroxybutyric acid (GHB) is a naturally occurring transmitter in the mammalian brain, related to sleep regulation and possibly to energy balance in diving or hibernating animals. It has been used for almost 35 years as an intravenous agent for induction of anaesthesia and for long-term sedation. Its convincing pharmacological properties, without serious adresse effects on circulation or respiration, are compromised by its unpredictable duration of action. This is not a major problem with long-term sedation during ICU treatment. GHB has been used with good results for sedation of patients with severe brain injury, where it compares favourably with barbiturates. In animal studies, it seems to possess a protective action against hypoxia on a cellular and whole organ level. However, in some experimental animals GHB has been shown to produce seizure-like activities, and the compound is being used to produce absence-like seizures. GHB has been used in our ICU for years to provide adequate sedation for patients under controlled ventilation or for patients figthing the respirator during spontaneous respiration. No serious side effects were observed in these patients, while in some patients under haemodialysis hypernatraemia and metabolic alkalosis developed; both were reversible after discontinuation of GHB and restriction of additional sodium input (Somsanit, the commercially available GHB preparation in Germany, contains 9.2 mmol sodium/g; the daily dose averages 20–40 g GHB, i.e. 180–370 mmol sodium). Patients and methods. In 31 patients after major abdominal surgery, sedation was established with GHB 50 mg/kg BW injected via perfusion pump over a 20-min period. No centrally acting medication had been given for at least 2 h. A computer-based multichannel EEG system (CATEEM, MediSyst, Linden) was used, allowing for fast Fourier transformation, spectral analysis and topographical brain mapping. EEG during induction of sedation was followed after a baseline EEG (10 min) had been recorded. Patients receiving long-term sedation were studied daily for an additional 15-min period. Corresponding well to the clinical findings, EEG pattern changed to a slow delta-theta or delta-only rhythm within 10 min of the start of injection. Alpha and beta power decreased, while delta activity exhibited an increase. All changes were most obvious in frontal and central areas of the brain. In about one out of three patients, a burst – suppression pattern developed. Since automatic processing of EEG may fail to detect special patterns like the looked-for 3/s spikes and waves, the raw EEG was analysed visually by an expert neurologist. Both processed and conventionally analysed EEG were free of any seizure-like electrical activity. Conclusion. We conclude that animal data may not apply to the use of GHB in humans, provided the dose is limited to the clinical needs. GHB is used in clinical practice in doses twice as high, or even higher, than the one we use for induction, without obvious side effects. However, the suppression of theta rhythm we observed in about half of the patients studied may indicate that even less than 50 mg/kg BW might be sufficient for adequate sedation.
    Notes: Zusammenfassung Gamma-Hydroxybuttersäure (GHB) als zentraler Neurotransmitter wird zur Sedierung in der Intensivmedizin eingesetzt. Trotz insgesamt sehr günstigen pharmakologischen Profils wurde seine Eignung in Frage gestellt, da es beim Tier in sehr hoher Dosierung Krampfpotentiale auslösen kann und in dieser Absicht sogar als Modellsubstanz für die Absenzen-typische Attacke dient. Nach eigenen positiven Erfahrungen mit GHB im klinischen Einsatz wurde bei 31 postoperativen Patienten, die zur Beatmung mit GHB sediert wurden, während der Induktionsphase mit 50 mg/kg KG kontinuierlich ein Multikanal-EEG abgeleitet. Als Ausdruck der Sedierung fand sich, verglichen mit der Referenzphase, einheitlich eine Verlangsamung des EEG-Grundrhythmus mit Ausbildung eines δ/θ- bzw. reinen δ-Rhythmus. Die EEG-Veränderungen korrelierten zeitlich gut mit dem klinischen Vigilanzzustand. Weder in der rechnergestützten Auswertung noch in der klassischen visuellen Auswertung der Original-EEG waren Hinweise auf die Ausbildung von Krampfpotentialen aufzufinden, insbesondere auch keine 3/s-Rhythmen, wie sie bei Absenzen-typischen Attacken beschrieben werden. Die Unterdrückung auch des Theta-Rhythmus bei einem Teil der Patienten darf als Hinweis auf eine relative Überdosierung gewertet werden und zeigt, daß– wie bei allen Anästhetika – auch bei GHB die Dosierung am klinischen Effekt zu orientieren ist.

    Description / Table of Contents: Abstract Object of the study: The aim of the study was to assess, whether the pneumatic pressure of an antishock-trouser (AST) of 20–40 mm Hg induces a decreased oxygenation of the anterior tibial muscle and attenuates muscular response potential (MRP) of n. peronaeus profundus? Methods: Among 22 normotensive, healthy volunteers the AST were tested by applying pressure values between 0 and 100 mm Hg and measuring the intracompartmental pressure, the muscular oxygen pressure as well as the MRP by electroneurographic means within a period of 6 hours. Results: The median initial intracompartmental pressure value of the m. tibialis anterior was 12.0 mm Hg (Q25%/Q75%: 8.9/17.3), the muscular oxygen pressure 14.8 mm Hg (Q25%/Q75%: 11.5/22.0). Transmission of the pneumatic AST-leg segment pressure to the muscle: 97.7% (Q25%/Q75%: 89.2/99.8). Already in the low AST pressure field (20–40 mm Hg) a severe hypoxia occurred in one case. A reduction of MRP was noticed at an AST pressure rate of 10 mm Hg. In 5 of 6 cases AST pressure values of 60 mm Hg led to pathological pO2-values within 5–20 minutes. Almost without exception AST-pressure rates 〈 60 mm Hg resulted in an anoxia of the muscle and loss of the MRP. Conclusions: We should demand that the AST are only applied with models where the pressure generated within the single segments can be controlled by pressure gauge. The application of the AST seems to be justified for polytraumatised in severe haemorrhagic shock where the risk of a local tissue ischemia with systemical consequences must deliberately be accepted.
    Notes: Zusammenfassung Fragestellung und Ziel der Studie: Ziel der Arbeit ist, durch Messung des intramuskulären Sauerstoffpartialdrucks den kritischen Bereich des pneumatischen Drucks der Antischockhose (ASH) zu definieren, ab dem sich eine Muskelischämie an der unteren Extremität entwickelt. Bei gleichzeitiger intrakompartimenteller Druckmessung sollte neben der Gewebeoxygenation auch die auf Druck empfindlich reagierende Impulsfortleitung im N. peronaeus profundus beurteilt werden. Methodik: Bei 22 normotensiven gesunden Freiwilligen wurden mit einem ASH-Beinsegment im M. tibialis anterior Druckwerte zwischen 0 und 100 mm Hg erzeugt. Über einen Zeitraum von bis zu 6 h erfolgte die Messung des Muskelgewebedrucks, der Sauerstoffspannung und des Muskelsummenaktionspotentials (MSAP) des N. peronaeus profundus. Ergebnisse: Ausgangsdruckwert im M. tibialis anterior im Median 12,0 mm Hg (Q25%/Q75%: 8,9/17,3), pO2: 14,8 mm Hg (Q25%/Q75%: 11,5/22,0). Übertragung des pneumatischen Drucks zu 97,7% (Q25%/Q75%: 89,2/99,8) auf den Extremitätenmuskel. Bereits bei niedrigen ASH-Druckwerten (20–40 mm Hg) trat im Einzelfall eine schwere Hypoxie auf. Eine Reduktion des MSAP war ab einem ASH-Druck von 10 mm Hg zu beobachten. ASH-Druckwerte von 60 mm Hg (n = 6) führten in 5 Fällen innerhalb von 5–20 min zum Abfall des pO2 auf pathologische Werte. Inflationsdruckwerte über 60 mm Hg resultierten nahezu ausnahmslos in einer Anoxie des Muskels und im Verlust des MSAP. Schlußfolgerungen: Der Einsatz der ASH sollte nur mit Modellen erfolgen, bei denen der Inflationsdruck manometrisch kontrolliert werden kann. Der ASH-Einsatz erscheint dann gerechtfertigt, wenn bei einem Polytraumatisierten im schweren hämorrhagischen Schock das Risiko einer sich auch systemisch auswirkenden lokalen Gewebeischämie bewußt in Kauf genommen werden muß.

    Notes: Summary Leu-19 antigen is a 200–220 kDa surface glycoprotein, initially detected on natural killer (NK) cells exhibiting non-major histocompatibility complex-restricted cytotoxity. Using a monoclonal antibody (mAb) directed against the Leu-19 molecule, we were unable to identify NK cells immunohistologically in cryostat sections of neuroectodermal tumors. Instead, mAb Leu-19 cross-reacted with the surfaces of the tumor cells, except for melanomas. It stained mesodermal tumor cells less intensely and did not stain those of carcinoma metastases. In three plasmocytomas, cells were observed, most likely NK cells, which extend Leu-19-positive cell processes towards to the unstained tumor cells. Furthermore, the Leu-19 antigen was identified on physiological tissues, especially on all neuroendocrine cells analyzed. The cross-reactions observed with Leu-19 mAb were confirmed using mAb NKH-1, which is also directed against the Leu-19 molecule. Thus, the expression of Leu-19 on neuroectodermal cells is another example of antigen sharing between specialized immune cells and the nervous system.
